Comparisons of Epitheliotrophic Factors in Autologous Serum Eyedrops from Sera of Chronic Renal Failure Patients with Those Prepared from Normal Controls

Abstract

Aims: To investigate the composition and biological activity of epitheliotrophic factors in serum from chronic renal failure (CRF) patients and to compare these with healthy controls. Methods: 20%, 50% and 100% autologous serum eyedrops (ASE) were prepared from blood samples of 16 patients with CRF and 16 normal subjects. Epithelial growth factor (EGF), platelet-derived growth factor-AB (PDGF-AB), transforming growth factor-β1 (TGF-β1) and fibronectin levels in serum were quantified using commercially available enzyme-linked immunosorbent assay kits. We performed MTT assay to compare the proliferative effects of the serum from CRF patients and healthy donors in a SV 40 immortalized human corneal epithelial cell (HCEC) culture model. In an effort to evaluate the migratory potential of ASE, migration assays were conducted via the manual scraping of HCECs. The morphologic changes were also observed with transmission electron microscopy (TEM). Results: EGF, PDGF-AB and TGF-β1 levels in ASE from healthy donors significantly higher than in serum of CRF patients. The pattern of cellular proliferation in patient group was similar to that in normal group. Effects of ASE on cell migration in vitro were significantly more prominent in normal group. Compared with negative control, ASE in both groups facilitated better proliferation and migration in vitro assay. It showed more noticeable enhancement after incubation with diluted serum in comparison with undiluted serum. In TEM analysis, HCECs incubated with patients’ 50% ASE showed some loss of microvilli without alterations of cytoplasmic organelles. Conclusions: Concentrations of epitheliotrophic factors and their biologic activities of serum from CRF patients were differed from those of healthy controls. ASE in patients with CRF have been also helpful in corneal healing process, especially when applied to a 50% concentration.;연구목적: 만성신부전 환자의 혈청 내 상피영양인자의 농도와 그 생물학적 기능을 정상인과 비교분석 하고자 하였다. 연구방법: 16명의 만성신부전 환자와 16명의 정상인의 혈액을 채취하여 20%, 50%, 100%의 자가혈청점안액을 제조하였다. Epidermal growth factor (EGF), platelet-derived growth factor-AB (PDGF-AB), transforming growth factor- β1 (TGF-β1) 및 fibronectin의 농도를 효소결합면역흡수분석법으로 정량화하였다. SV 40으로 transfection된 인체각막상피세포 모델에서 만성신부전 환자와 정상인의 자가혈청의 증식 효과를 비교하기 위하여 MTT 분석법을 시행하였다. 또한 자가혈청점안액 노출 시 각막상피세포의 유주 정도를 알아보기 위하여 생체외 창상회복 분석법을 시행하였으며, 형태학적 변화를 투과전자현미경으로 관찰하였다. 연구결과: 만성신부전 환자의 자가혈정점안액 내 EGF, PDGF-AB 및 TGF-β1의 농도는 정상인에 비하여 유의하게 낮았다. MTT상 환자 혈청을 처리한 각막상피세포의 증식 정도가 적었으나 통계적으로 유의하지 않았다. 유주 분석 결과 환자군에 비해 정상인의 혈청을 처리한 경우 유주 세포가 유의하게 많았다. 자가혈청점안액을 첨가하여 배양한 경우 음성대조군에 비해 두 군 모두에서 의미 있게 증식 및 유주가 증가하였다. 희석한 혈청을 사용한 경우 희석하지 않은 혈청을 사용한 경우보다 증식 및 유주 정도가 높았다. 만성신부전 환자의 자가혈청으로 배양한 경우 세포질 내 미세소기관의 변성은 없었으나 일부 세포들에서 미세융모가 감소함이 관찰되었다. 결론: 만성신부전 환자의 혈청 내 상피영양인자의 농도와 생물학적인 기능은 정상인과 달랐다. 만성신부전 환자의 자가혈청 점안액도 각막상피 치유과정에 도움이 되었으며, 특히 50% 농도에서 효과적이었다.
