dc.description.abstractFor pregnant women with gestational diabetes mellitus (GDM), adequate nutrition intake is important to maintain maternal and fetal health. The aim of this study was to examine dietary pattern, nutritional intake, and diet quality of Korean pregnant women with GDM. Between October 2008 and May 2012, 166 pregnant women diagnosed with GDM completed a questionnaire to collect data on general background characteristics. Dietary intake was assessed using a 3-day food record. Blood pressure, fasting plasma glucose, and hemoglobin A1c (HbA1c) concentrations were measured and oral glucose tolerance test (OGTT) was performed. Two major dietary patterns (‘carbohydrate and vegetable’ and ‘western’ patterns) were identified through factor analysis. The ‘carbohydrate and vegetable pattern’ was characterized by greater intakes of fruits, rice and cereals, fermented vegetables, vegetables, and meat. The ‘western pattern’ was characterized by high consumption of poultry and eggs, fast food, deep-fried food, processed meat and seafood, snacks and desserts, coffee and other beverages, and seaweed. Subjects with higher carbohydrate and vegetable pattern scores reported less physical activity (P < 0.05) and have higher diastolic blood pressure levels (P = 0.05). The level of 100 g OGTT at 1 hr was significant higher in western pattern than that of carbohydrate and vegetable pattern (P = 0.02). Higher energy intake was associated with higher scores for both dietary patterns (P < 0.0001). After adjusting for age and energy intake, higher carbohydrate and vegetable pattern scores were associated with higher sodium intakes (P = 0.02), but lower intakes of fat (P = 0.002) and other micronutrients. On the other hand, higher western pattern scores were associated with higher fat intake (P = 0.0001), but lower intakes of sodium (P = 0.01) and other micronutrients. Higher scores for both dietary patterns were associated with lower scores in the moderation category of the dietary quality index-international (DQI-I) (P < 0.0001). HbA1c and fasting plasma glucose levels were significantly lower among participants with high than among those with low DQI-I (P < 0.05). In conclusion, the study findings suggest that many Korean women with GDM do not consume nutritionally adequate or balanced diets, regardless of dietary pattern. Consumption of a high quality and balanced diet is recommended to maintain maternal and fetal health in women with GDM.; 임신 시에는 적절한 식사섭취가 임신성 당뇨를 가진 산모는 물론 태아에게까지 영향을 주는 위험인자로 제기되고 있다. 따라서 본 연구는 임산부의 식습관을 알아보고 분석한 결과로 임신성 당뇨병과 식사패턴, 식사의 질과 영양섭취간의 관련성에 대한 연구를 수행하였다. 본 연구에 대상자는 2009년 1월 1일부터 2010년 12월 31일까지 임신성 당뇨병으로 판정 받은 임산부에게 실시되어 일반사항, 신체계측, 생화학검사, 식습관 및 식이섭취상태조사는 설문지를 통해 조사하였으며 식이섭취조사를 위해 임신 중 식이섭취 상태를 이용하여 3일 식이기록을 작성하였다. 생화학 검사는 혈액검사를 통해 당화혈색소, 선별검사를 통해 선별하였고, 확진검사에서는 경구 당부하검사 결과(OGTT)를 조사하였으며, 수축기, 이완기 혈압을 측정하였다. 요인분석을 실시한 결과 2가지 패턴 (‘Carbohydrate and vegetable’와 ‘western’)이 도출되었다. 이 패턴은 야채, 과일, 잡곡류, 김치류가 섭취가 높았고, ‘western dietary’ 패턴은 가금류, 계란류, 패스프푸드, 튀김류, 가공육, 스낵 및 빵류, 커피 및 탄산음료 등의 섭취가 높았다.‘Carbohydrate and vegetable’ 패턴점수가 높은 임산성 당뇨 산모들이 낮은 산모에 비해 신체적 활동이 낮고, 이완기 혈압의 위험도가 증가하였다. 100g OGTT 1시간의 경우, ‘western pattern’이 증가할수록 OGTT가 유의하게 증가하였다. 영양소 섭취에 대해서는 열량은 ‘Carbohydrate and vegetable’와 ‘western pattern’ 두 패턴 모두 평균적으로 높게 섭취하고 있는 것으로 나타났고, 나트륨 또한 두 패턴 모두 높았으나, 지방과 다른 영양소에서는 ‘Carbohydrate and vegetable’비해 ‘western pattern’ 이 평균적으로 더 섭취하였다. 각 패턴 별 식사의 질 점수에 따른 임신성 당뇨의 위험성을 조사하여본 바, 절제성 점수가 낮을 때, 두 패턴 모두 점수가 유의적으로 증가하였다. 다양성 항목의 총 다양성 점수는 ‘Carbohydrate and vegetable’은 ‘western pattern’보다 점수가 높았다. 또한, 적절성 항목의 과일 점수는 ‘Carbohydrate and vegetable’은 ‘western pattern’보다 점수가 높았다. 총 DQI 점수와 2개의 식사패턴과의 관련성은 유의하지 않았으나, ‘Carbohydrate and vegetable’은 ‘western pattern’보다 점수가 약간 높은 것으로 나타났다. 혈액수치와 DQI점수를 분석 해 본 결과, 당화혈색소, 공복혈당 수치가 낮을수록 DQI점수가 유의적으로 높아짐을 나타내었다. 결론적으로, 본 연구에서는 우리나라 임신성 당뇨 임산부의 식사패턴과 상관없이 부적절한 식사 섭취와 균형되지 못한 식사를 하고 있었으며, 따라서, 임신성 당뇨 산모와 태아의 건강 유지를 위해 질이 높은 식사와 균형적인 식습관을 유지하는 것이 권장되어야 할 것으로 사료된다.-
