dc.description.abstract비스테로이드성 항염제 (Nonsteroidal anti-inflammatory drugs, NSAIDs)는 류마티스성 질환뿐 아니라 신경질환, 종양, 알츠하이머병 등 많은 질환에 다빈도로 사용되고 있다. 전 세계적으로 임상의학에서 가장 흔히 사용되는 치료제 중의 하나인 NSAIDs는 질환에 대한 효과와 더불어 부작용이 자주 발생되는 것으로 알려져 있다. 또한 상당수 환자들은 남용하는 경향이 있어 이들 약물에 빈번한 노출로 인한 부작용 사례가 감소하지 않고 있다. 이에 본 연구에서는 NSAIDs에 의한 약물반응을 미리 예측하고 나아가 의약품의 안전성 예측 및 유효성 평가에 활용가능한 생체지표 후보물질을 발굴하고자 하였다. 최근, 대사체군(metabolome)을 분석하고 해석하는 대사체학(metabolomics)에 대한 연구가 많이 진행되고 있다. 이에 본 연구에서는 metabolomics 기술을 이용하여, NSAIDs에 의하여 변화되는 내인성 대사체를 1H NMR을 이용하여 분석함으로써 약물반응과 상관성을 규명하고, 대사체학을 이용한 대사체 생체지표 후보물질을 확보하였다. 대사체학(metabolomics)을 이용하면, 다양한 유전적, 생리적 또는 환경적 조건에서 변화되어 나타나는 대사체군(metabolome)의 구성과 농도를 분석하고 해석함으로써 생명현상의 변화 원인을 규명하고, 약물 효능 또는 부작용 예측 및 질병의 조기진단에도 활용가능할 것이다. 이는, 미지의 유전자들의 기능을 밝혀내고자 하는 기능 유전체학(functional genomic) 또는 체내 단백질이 얼마의 양으로 어떤 환경에서 발현되는가를 연구하는 단백질체학(proteomics)의 연구방향과는 구별이 되어지는 분야라 할 수 있다. 세계적으로 대사체학 연구를 수행하고 있는 연구소 및 학교 등도 점차 늘어나고 있는 추세이며, 미국, 영국, 일본 등 세계 각국에서 점차 대사체학을 이용하여 식물 및 미생물뿐 만 아니라 식품, 의약품 등의 안전성 및 독성 예측을 위한 연구로 확대∙추진되고 있다. Chapter 1 에서는 non-selective COX-inhibitor 인 indomethacin, ibuprofen 및 COX-2 selective inhibitor 인 celecoxib 약물을 각각 25 mg/kg, 800 mg/kg, 133 mg/kg 으로 투여한 후 뇨중 내인성 대사체를 분석하였다. 그 결과 내인성대사체의 변화패턴이 명확히 구분되었으며, 이들 classification 에 관여한 물질은 allantoin, taurine 및 dimethylamine 로써 ANOVA에서 통계적으로 유의적 차이(p<0.05)가 있었으며, loading plot 및 V.I.P 결과 그룹간 차이를 유발하는데 관여한 물질들 이었다. Chapter 2 에서는 indomethacin (25 mg/kg) 에 의한 위손상 유발과 동시에 위손상 억제를 위한 cimetidine (100 mg/kg) 병용투여 시 뇨중 내인성대사체 변화를 분석하였다. 또한, indomethacin 10 mg/kg 용량 투여시 개체의 위손상 정도에 따른 내인성 대사체 변화를 분석하였다. 그 결과, 2-oxoglutarate, taurine, acetate 및 hippurate 가 NSAIDs 에 의한 위손상 후보물질도 도출되었다. Chapter 3 에서는 대표적인 NSAIDs계 약물인 acetylsalicylic acid (10 또는200 mg/kg), diclofenac (0.5 또는15 mg/kg), piroxicam (1 또는 10 mg/kg), indomethacin (1 또는25 mg/kg), ibuprofen (10 또는150 mg/kg) 및 celecoxib (10 또는100 mg/kg) 를 랫트에 투여하고, 뇨중 내인성 대사체를 분석하였다. 44개의 내인성 대사체 분석결과, citrate, allantoin, 2-oxoglutarate, acetate, benzoate, glycine및 trimethylamine-N-oxide 가 NSAIDs 에 의한 위손상 예측 후보물질군으로 도출되었다. 본 연구를 통하여 랫트에서 대사체학을 이용하여 뇨중 내인성 대사체 분석 및 NMR spectrum 데이터로부터 global profiling 및 targeted profiling을 통하여 내인성 대사체 변화 패턴을 분석함으로써 NSAIDs 에 의한 위점막 손상여부를 예측할 수 있음을 확인하였다. 이는 약물개발 시 약물의 유효성 및 안전성 스크리닝 단계에서 유용하게 활용될 수 있을 것이며 의약품의 안전성․유효성 평가 기반 마련에 기초자료로 활용될 수 있을 것이다. 나아가 이러한 약물에 의한 위손상 예측방법 확립을 통하여 침습적인 방법인 내시경을 통한 위궤양검사 단계를 대사체학을 활용한 비침습적방법으로 대체할 수 있을 것이다. 이를 통하여 의약품 평가에 대사체의 적용 가능성을 입증하였으며, 확보된 대사 profile의 DB화를 통한 정보제공 및 이를 바탕으로 한 대사체학 이용 의약품 개발지원 및 평가에도 활용가능할 것이다. 나아가 제약산업에서는 신약개발 비용 기간 단축을 도모하고, 대사체학 기반기술 확보를 통한 pharmaco-metabolomics를 활용한 개인맞춤형 약물요법의 도입 및 대사체학이라는 최첨단기술을 활용한 의약품 평가방법 확보에도 기여할 것으로 기대된다.;Nonsteroidal anti-inflammatory drugs (NSAIDs) have been frequently used for the treatment of rheumatoid arthritis, tumor, nervous disease and Alzheimer's disease treatment in the world wide, however many patients are suffering from NSAIDs's side effects such as gastric damage. The purpose of this study was to identify the metabolites for prediction the NSAIDs drug response and to probe biomarkers for the drug evaluation based on the metabolomics. Recently, metabolomics is in the limelight at the point of drug development and disease diagnosis. Endogenous metabolites are changed from the inherited gene depending on physiological and environmental conditions including special stimuli. Comparing the changes of endogenous metabolites induced by certain condition can interpretate the biological response of object exposed to stimuli. Pattern recognition analysis of the 1H-nuclear magnetic resonance (NMR) spectra of urine was performed to develop surrogate biomarkers related to the gastrointestinal (GI) damage induced by NSAIDs in rats. In chapter1, urine samples were collected 5 hr after the administration of the following NSAIDs : celecoxib (133 mgkg-1, p.o.), a COX-2-selective inhibitor; and indomethacin (25 mgkg-1, p.o.) or ibuprofen (800 mgkg-1, p.o.), nonselective COX inhibitors. The urine was analyzed using 600M 1H-NMR for spectral binning and targeted profiling. The level of gastric damage in each rat was also determined. Indomethacin and ibuprofen caused severe gastric damage, but no lesions were observed with celecoxib. The 1H-NMR urine spectra were divided into spectral bins (0.04 ppm) for global profiling, and 36 endogenous metabolites were assigned for targeted profiling. Multivariate data analyses were carried out to recognize the spectral pattern of the endogenous metabolites related to NSAIDs using partial least square-discrimination analysis (PLS-DA). There were different clusterings of 1H-NMR spectra according to the gastric damage scores in global profiling. In targeted profiling, a few endogenous metabolites of allantoine, taurine and dimethylamine were selected as putative biomarkers for the gastric damage induced by NSAIDs. In chapter 2, urine samples were analyzed after oral administration of indomethacin (25 mgkg-1) or co-administration with cimetidine (100 mgkg-1). Indomethacin alone caused severe gastric damage, but indomethacin administered with cimetidine did not. Simultaneously, the patterns of changes in their endogenous metabolites were different. Multivariate data analyses were carried out to recognize the spectral pattern of endogenous metabolites related to indomethacin using PLS-DA. In targeted profiling, 2-oxoglutarate, acetate, taurine and hippurate were selected as putative biomarkers for the gastric damage induced by indomethacin. These metabolites changed depending on the degree of GI damage, although the same dose of indomethacin (10 mgkg-1) was administered to rats. In chapter 3, several NSAIDs were administered to rats : acetylsalicylic acid (10, or 200 mgkg-1, p.o.), diclofenac (0.5, or 15 mgkg-1, p.o.), piroxicam (1, or 10 mgkg-1, p.o.), indomethacin (1, or 25 mgkg-1, p.o.), ibuprofen (10, or 150 mgkg-1, p.o.), nonselective COX inhibitors; and celecoxib (10, or 100 mgkg-1, p.o.), a COX-2-selective inhibitor. The nonselective COX inhibitors caused severe gastric damage, but no lesions were observed in the celecoxib treated rats like vehicle and 44 endogenous metabolites were assigned for targeted profiling. A few endogenous metabolites of citrate, allantoin, 2-oxoglutarate, acetate, benzoate, glycine, glucose and trimethylamine-N-oxide were monitored in the targeted profiling as putative biomarkers for the gastric damage induced by NSAIDs. The results of global and targeted profilings suggest that the gastric damage induced by NSAIDs can be screened in the preclinical stage of drug development using a current metabolomics study. A further study will refine the predictive potential of these metabolites in the development of new NSAIDs. Additional investigations should also focus on the metabolites that are corrected by the disease treatment with NSAIDs, but not by administration of the agent itself. Statistical identification of the biological processes associated with NSAIDs will enhance the robustness of these findings. Nevertheless, metabolic profiling allowed the identification of drug-induced endogenous metabolites. Moreover, monitoring of drug-induced disturbances in the homeostasis of the basal metabolites by metabolomics is more accessible and sensitive, and less biased than the classical methods used to characterize drug safety and efficacy in drug development. The major advantage of this study are its simplicity and non-invasive nature because it is based on urine analysis using metabolomics. Overall, these putative biomarkers could be used to predict the risk of adverse effects provoked by NSAIDs. In addition, metabolomics with a surrogate biomarker can serve as a more rapid and easier preliminary screening tool to determine GI damage induced by new drug candidates.-
