Connecting the Dots Or Missed Opportunities? Decentralization and Access to Antenatal care in Uganda

Abstract

This thesis uses the Principal Agent Approach with the Decision Space Map to determine the efficacy of decentralization on access to antenatal care in Uganda. Since 1997, Uganda has implemented decentralization to deepen democracy and improve delivery of public services. A number of authors are not conclusive on the effectiveness of decentralization to achieve the objectives of health care systems, thereby advocating for re-centralization of key functions. To explain this subject, the thesis analyzes Uganda local government finance statistics, health sector performance reports and national demographic and health survey reports since 1997 to date. The thesis found that with scarce local revenue, undeveloped human resource and heavy reliance on donor funding, local governments are limited to make independent decisions towards public services such as antenatal care. Health policy makers can expand opportunities to partner with the civil society and improve people's economic output so as to reduce donor dependency and diversify sources of local revenue.;본 논문은 다차원적 유용성 분석 방법(Decision Space Map)과 프린시펄 에이전트 방식(Principal Agent Approach)을 사용하여 우간다의 출산 전 관리의 접근성에 있어서 지방분권체제의 유효성을 판단하고자 한다. 우간다는1997년도 이후로 민주주의를 보다 깊이 육성하고 공공 사업의 개선을 도모하고자 지방분권제도를 도입해왔다. 하지만 의료서비스 체계의 목적을 달성함에 있어 지방분권제도의 효율성에 대해서는, 다수의 학자들이 확정적인 결론을 내리지 못하고 있으며, 이로 인하여 그들은 정부의 핵심적 기능을 엮는 재-중앙집권화 방식을 옹호하고 있는 실정이다. 본 논문에서는 이 주제에 관해 설명하고자 1997년 이후 현재까지의 우간다 지방 정부의 재정 통계자료와 의료부문 실적 보고서 및 국민 건강 진단 조사 보고서를 분석하고 있다. 결과적으로 본 논문은 부족한 국가 세입과 인력개발의 미흡, 그리고 기부 자금에 대한 심각한 의존 상태로 인하여 지방정부가 출산 전 관리와 같은 공공 사업분야에 있어서 독립적인 의사결정에 한계가 있음을 밝히는 바이다. 그러므로 의료 정책 수립자들은 기부 자금 의존도를 낮추고 국가 수입원을 다각화하기 위하여, 시민사회와 협력할 수 있는 기회를 확장하고 국민 경제 생산량을 증가시킬 수 있도록 노력해야 한다.
