스펙트럼영역 빛간섭단층촬영으로 측정한 녹내장 환자의 맥락막 두께 분석

Abstract

Purpose : To evaluate the choroidal thickness in primary open–angle glaucoma (POAG) and normal tension glaucoma (NTG) with spectral-domain optical coherence tomography (SD-OCT) and to explore the relationship between the choroidal thickness and glaucoma. Methods : A retrospective analysis was performed on patients who had been diagnosed with NTG or POAG at Ewha Womans University, Mokdong hospital. SD-OCT scans were obtained to estimate retinal nerve fiber layer(RNFL) thickness, macular thickness, subfoveal and peripapillary choroidal thickness in a group of NTG, POAG and normal control. Results : 53 NTG, 21 POAG and 42 normal subjects were enrolled in the study. The RNFL thickness and macular thickness were significantly thinner in the NTG and POAG groups as compared to normal subjects. In contrast, there were no significant differences in subfoveal and peripapillary choroidal thickness among the three groups. There was no significant correlation between the peripapillary choroidal thickness and retinal nerve fiber layer thickness. Conclusion : To conclude, choroidal thickness does not seem to differ between glaucoma and normal subjects, and there was no significant correlation between the choroidal thickness and glaucomatous optic neuropathy. ;혈류 조절 이상은 녹내장 시신경병증의 병인 중의 하나이며, 시신경 전사상판의 혈액공급을 맥락막이 담당하므로 맥락막과 녹내장 시신경병증의 발생이 연관이 있을 것으로 생각된다. 본 연구에서는 정상안압녹내장과 원발개방각녹내장 환자의 맥락막 두께를 측정하여 맥락막 두께와 녹내장 시신경병증의 관계를 알아보고자 하였다. 정상안압녹내장, 원발개방각녹내장, 대조군 세 집단에서 스펙트럼영역 빛간섭단층촬영으로 측정된 망막신경섬유층 두께, 황반 두께, 황반 하 및 유두주위 맥락막 두께를 비교하였다. 또한 녹내장 환자에서 망막신경섬유층 두께와 유두주위 맥락막 두께의 연관성을 분석하였다. 정상안압녹내장 53안, 원발개방각녹내장 21안, 대조군 42안이 포함되었으며, 망막신경섬유층 두께, 황반 두께는 녹내장안에서 정상안에 비해 얇게 나타났다. 황반 하 및 유두주위 맥락막 두께는 녹내장안과 정상안 사이에 유의한 차이를 보이지 않았다. 녹내장안에서 망막신경섬유층 두께와 유두주위 맥락막 두께 사이에 유의한 연관성은 없는 것으로 나타났다. 본 연구 결과, 맥락막 두께는 녹내장안과 정상안 사이에 유의한 차이를 보이지 않았으며, 맥락막 두께와 녹내장 시신경병증 발생의 관련성은 없는 것으로 나타났다. 그러므로 맥락막 두께를 녹내장의 병인 규명 및 진단에 활용하기 위해서는 향후 추가적인 연구가 필요할 것으로 생각된다.
