피부 색소침착병변에 대한 색소레이저(PLTL-1)의 치료효과

Abstract

피부 색소침착병변 치료에 피부 박피술, 탈색소제제 및 다양한 종류의 레이저가 이용되어 비교적 양호한 효과를 보인다. 이중 Flashlamp-pumped pulsed dye laser(PLTL-1)는 selective photothermolysis(SPT)원리를 이용한 색소레이저로 액체레이저인 PLDL과 고체레이저인 TATULAZR가 기계 한대에 조립되어있다. PLDL(pigmented lesion dye laser)은 510nm의 파장과 400nsec의 파 간격 및 5mm 조사직경을 가지고 표피 색소침착병변 치료에 이용되며, flashlamp-excited, Q-switched alexandrite laser(TATULAZR)는 755nm의 파장, 100nsec의 파 간격 및 3mm 조사직경으로 진피 색소침착병변 치료에 이용한다. 저자는 PLTL-1레이저로 피부 색소침착병변을 치료하였으며 그 결과는 다음과 같다. 1. 반문상 모반, 주근깨, 노인성 혹자 및 Peutz-Jegher syndrome은 우수 및 양호한 반응을 보였다. 2. 색소성 모반과 Becker씨 모반은 보통반응을 보였다. 3. 기미와 밀크 커피색반점은 불량반응을 보였다. 4. 오타모반, ABNOM 및 문신은 양호내지 보통반응을 보였다. 5. 부작용으로는 일시적인 과색소침착. 저색소침착 및 재발이 있었다.;Many conventional modalities such as dermabrasion, depigmenting creams as well as several types of lasers have been used for the treatment of cutaneous pigmented lesions without countable effects. Flashlamp-pumped pulsed dye laser (PLTL-1) is one of the selective photothermolysis laser. This laser system incorporates two separate lasers, for use in separate treatments: the pigmented lesion dye laser(PLDL), having an emission wave length of 510nm, a pulse duration 400 nanoseconds and 5mm spot sire, a flashlamp-excited pulsed dye laser is used to treat superficial cutaneous lesions, and a flashlamp-excited, O-switched alexandrite laser(TATULAZR), having an emission wavelength of 755nm, a pulse duration 100 nanoseconds and 3mm spot size is designed for the treatment of blue-black tattoos. Purpose of this study is to evaluate efficacy of PLTL-1 laser for treatment of cutaneous pigmented lesions. The results were as follow: 1. Excellent and good responses were observed in nevus spilus, freckles, senile lentigines and Peutz-Jeher's syndrome. 2. Fair responses were observed in melanocytic nevus. Becker's nevus. 3. Poor response were observed in melaslma and cafe-au-lait spot. 4. Good to fair responses were observed in nevus of Ota, ABNOM, and tattoos. 5. There were mild complications such as transient hyperpigmentation, transient hypopigmentation and recurrence.
