알콜중독자의 아내학대에 관한 연구

Abstract

알콜중독은 의료적, 경제적, 사회·심리적 자애를 초래하는 만성적이고 진행적인 질병이다. 또한 그 가족기능의 심각한 갈등과 손상은 가져다 주고 가정이 핵심을 이루는 부부관계를 파괴, 왜곡시키는 가족병으로 정의되어진다. 알콜중독으로 인한 입원율의 증가가 두드러지는 한국에서 알콜중독과 그 가족병적 현상에 대한 인식과 연구는 초보적이고 단편적인 수준에 머물러 있는 실정이다. 특히 1970년대 이후 제기되어 온 아내학대와 알콜중독의 연합에 대한 연구는 전무한 형편이다. 이에 본 연구는 가족병으로서의 알콜중독 특성에 대한 이해도를 증진시키고, 아내학대 정도에 대한 수량화, 체계화 작업을 통하여 사회사업 재활 프로그램 개발의 기초자료를 제공하는데 그 목적이 있다. 이러한 연구목적을 위해 본 연구는 알콜중독과 가족, 알콜과 폭력의 연관성, 알콜중독과 아내학대 등에 대한 외국의 선행 문헌 연구와 한국내 실태조사를 병행하여 진행하였다. 연구의 대상으로는 서울시내 3개병원에 알콜중독으로 진단되어 입원 치료중인 결혼한 남자환자와 그 아내들로 하고 구조진단적인 면접 후 설문조사를 실시하였다. 분석방법으로는 SPSS를 통한 단순빈도, 백분율, 비모수통계에 의한 방법과 상관관계 분석의 방법으로 처리되었다. 주요 조사결과를 분석하면 다음과 같다. 첫째, 대상자들의 입원연령이 40대 초반으로 늦은 형태를 보이고 있으며 대다수의 아내들이 실질적인 집안의 생계 유지자 역할을 하고 있는 것으로 나타났다. 그리고 부부 공히 알콜중독의 가족력이 높았으며 입원환자의 재발 경력 또한 높았다. 이는 알콜중독의 특수한 부부역할을 명확히 나타내 주는 것이며 가족적인 알콜중독의 가능성을 보여주는 것이라 할 수 있다., 둘째, 알콜중독 가정내 아내학대 정도를 살펴 본 결과 지난 1년내에 남편으로부터 신체적인 학대를 당한 아내는 대상자의 92.9%라는 놀라운 비율을 나타내었다. 특히 이들 가운데 "일반적 폭력"을 경험한 것이 92.8%, "심한 폭력"을 경험한 비율이 83.3%, "칼 망치 등 흉기로 위협하거나 또는 실제로 사용한 매우 심한 폭력"이 40.5%라는 수치로 나타났음을 주목할 수 있다. 세째, 알콜중독자의 대다수가 음주후 가정에서 폭력을 행사한다고 응답했으며 이러한 아내학대 정도는 남편의 음주량이 증가할수록 점차 심해지는 것으로 나타났다. 학대시 아내들은 적극적으로 자신을 방어하는 것으로 나타났다. 위의 결과들을 고려해 볼 때 입원한 알콜중독자의 아내학대 정도와 가족적인 손상도는 매우 높았으며 이들 가족을 위한 서어비스 개발이 시급하다는 것을 알 수 있었다. 현재 한국에서 알콜중독과 아내학대를 남성적인 힘의 한 표현으로서 관대하게 수용하는 경향을 보이고 있으나 사회의 기초체계인 가정의 파괴가 매우 두드러진 양상을 보인다는 점에서, 알콜중독자의 치료와 재활에 절대적인 역할을 담당하는 부부체계의 자료와 교육을 위한 사회사업 프로그램이 매우 필요한 실정이다.;Alcoholism has been called the "family disease". Because of its damaging impact upon the entire family unit. There are serious conflicts and role disfunctions in the families of alcoholics. Studies and experiments about the interactions between alcoholic couples indicate that it's very tough, rigid and untolerable, especially more conflicts. They engage in the marrital relationship in more negative and hostile attitudes. It's known that the relationships of alcoholic couples are generally more, when be compared with normal couples, irrational in the action of trouble shoot. With low abilities to solve problems, they got negative reputation from crime statistics. Social Work studies of U.S.A. pointed to the close relations between alcohol consumption and aggressive vehavior. Studies suggests that aggressive, violent and illegal behaviors are more likely to occur when individuals have consumed alcohol and that heavy drinking may be a means to arrogate power and control over the others with the excuses of alcohol. Over the past years, professionals in the fields of social work, psychiatry, psychology and sociology had directed that more increased attention be paied in the problems of wife abuse. The purpose of this study is to give reality of the relationshp between alcoholism and wife abuse. This study assessed the clinical features of alcoholics and wife abuse. By measuring wife abuse in alcoholic family, it will be shown the main and expected correlation of alcohol and wife abuse. This study incorpporated with some instruments to measure the degree of alcoholism and violence. Concerning alcoholism, MAST(Michigan Alcoholism Screening Test) scale was used. For the wife abuse, CTS(Conflict Tactions Scale) scale was used. The result of this study is summerized as follows ; 1. MAST score of the samples(alcoholic husband), 9 had mild alcohol dependence(MAST scale 7-19 points), 10 moderate alcoholics(MAST 20-29), and 23 severe alcoholics(MAST 30-53) 2. Results indicate that 92.9% in wives of alcoholics experienced violence during the last year. So 92.9% of samples experienced general violence(through something, push, grabbed, shoved, etc), and 83.3% of samples had severe violence(slap, kick, bit, hit, etc), and 40.5% of samples experienced too severe violence(threatened with knife or hammer, axe or used it) 3. The comparision of MAST score of husband and CTS score of wife abuse showed that the two score come together to positive direction. The result of this study can strongly support the clinical observation and previous studies that wife abuse is more often present with alcoholism. When considering the data reported by this sample, one may becomes amazed by the high levels of violence. From the theoritical view point, men who are concerned about demonstrating their mascurinity may try to accomplish this symbolically by drunkeness. Results will be dominance over women, and the exertion of physical force on the other. At the individual level, it probably includes acting on a belief in the disinhibiting power of alcohol. At the societal level, the underlying process probably includes norms which excuse "drunken comportment". The major conclusion of this study may be stated quite simply : wife abuse in alcoholic marrage was far more prevalent, frequent, and severe in Korea.
