Abstract
과거, 치명적인 질병으로 알려졌던 암은 현대의학 기술의 발달로 암 환자의 수명은 상당기간 연장할 수 있게 되었고 따라서 근래에는 암이라고 진단받은 후 많은 환자들이 죽음과 직면한 상태에서 투병하고 있는것을 많이 볼 수 있다. 그러나 치료 과정중 동통이나 약물요법에 의한 부작용과 독작용이 심하여 암 환자는 신체적, 사회 심리적인 격심한 변화를 겪게된다. 특히 암 환자의 정서적 상태는 암 자체에 못지않게 중요시 되며 암의 임상적 경과, 예후 및 환자의 생존기간에 심각한 영향을 미친다. 본 연구의 목적은 약물 요법을 받는 암 환자 자신의 가치성에 대한 개인적 평가인 자아 존중감과 죽음을 앞둔 불안과의 관계를 규명하여 남은 여생의 적응을 돕기 위한 과학적 간호 중재의 기초 자료를 수집하는데 있다. 연구대상 환자는 서울시내 2개 대학 병원의 외래 치료실과 입원을 통해 약물요법을 받는 암 환자중 113명을 뽑아 대상으로 하여, 1984년 9월 19일부터 10월 8일까지 20일간에 걸쳐 자료를 수집하였다. 연구도구는 Rosenberg의 자아 존중감의 척도(Self-Esteem Scale)와 Spielberger의 STAI(State-Trait Anxiety Inventory)를 사용하였다. 위의 도구를 이용하여 총 68문항으로 구조화된 설문지를 사용하여 조사하였으며 수집된 자료를 ANOVA, Pearson Correlation Coefficient 방법으로 분석한 결론은 다음과 같다. 1 .연구대상 환자의 일반적 특성과 자아 존중감, 기질불안 및 상태불안과의 관계를 분석한 결과, a. 일반적 특성과 자아 존중감과의 관계에서는 성별로 남자 환자가 여자 환자에 비해 높은 존중감을 나타내었고,(P=0.0375) 연령별로는 31세~40세에서 자아 존중감이 가장 높았고 16세~30세에서 가장 낮았다. (P=0.0148) 이밖에도 조사대상 환자중 공무원, 회사원, 교사등 봉급 생활자가 자유업 종사자 및 무직자에 비해 높은 자아 존중감을 나타내었으며, (P=0.0053) 1~2회로 가장 적은 횟수의 약물요법을 받은 환자에 있어 자아 존중감이 높았고,(P=0.0387) 약물요법을 최근에 시작한 환자에서 가장 높은 자아 존중감을 나타냈다.(P=0.0351) b. 일반적 특성과 기질불안과의 관계에서는, 여자 환자가 남자 환자에 비해 기질불안이 높았고,(P=0.0001) 또한 16세~30세된 환자에서 기질불안을 가장 높게 나타났고 61세~75세에서 가장 낮은 기질불안을 나타내었다.(P=0.0287) 가족수별로는, 2인 이하의 가족이 있는 경우 기질불안이 높았다.(P=0.0491) c. 일반적 특성과 상태불안과의 관계에서는, 여자 환자가 남자 환자에 비해 상태불안이 높게 나타났다.(P=0.0001) 이러한 연구결과를 근거로 분석된 본 연구의 가설검정의 결과 제 1 가설 ; 조사 대상 환자의 자아 존중감이 높을수록, 기질불안은 낮았다. (r=0.481, p=0.0001) 제 2 가설 ; 조사대상 환자의 자아 존중감이 높을수록, 상태불안은 낮았다. (r=0.385, p=0.0001) 제 3 가설 ; 조사 대상 환자의 기질불안이 높을수록, 상태불안도 높았다. (r=0.675, p=0.0001) 따라서 제 1, 2, 3가설은 모두 지지 되었다.;In the past, the lifespan of a cancer patients was very short. However, as a result of the rapid development of modern medical technology, we can now prolong considerably, a concer patients life. We can now treat cancer patients with the hope that one day, we may find a cure for this fatal disease Despite the progress made in cancer research, the cancer patient must confront painful side effects including physical and psychological changes during the treatment process of chemotherapy. Chemotherapy has toxic effect which creates many types of physical, emotional and psychological problems for the cancer patient. This study examined the degree of and the relationships among Self-Esteem, State-Anxiety, and Trait-Anxiety of cancer patients receiving chemotherapy. The purpose of this study is to obtain data which may be useful in the improvement of nursing care practice concerning chemotherapy cancer patients. The survey was conducted among 113 cancer patients who were receiving chemotherapy at an out-patient department, treatment room and an admission room in two hospitals attached to universities from September 19, 1984 to October 8, 1984. The instruments used in this survey were Rosenberg's Self-Esteem scale and spielberger's State-Trait Anxiety Inventory scale which together totalled 68 items. Analysis of data was conducted using ANOVA and pearson correlation coefficient. Summary of Results. 1. Analysis of factors influencing Self-Esteem a. Gender; male patients were found to have a higher level of self-esteem than women patients. b. Age; the level of self-esteem in younger patients was lower than that of older patients. c. Occupation; the patients who were receiving a salary were found to have a higher level self-esteem than the patients who were not receiving a salary. d. Frequency of chemotherapy; cancer patients who were subjected to many chemotherapy were found to have lower level of self-esteem than those patients who were subjected to a small number of chemotherapy. 2 . Analysis of factors influencing trait Anxiety. a. Gender; Woman patients were found to have a higher level of trait Anxiety than male patients. b. Age; the level of trait anxiety was lowest for cancer patients aged 16-31. c. Family size; the level of trait anxiety was lowest for cancer patient families with consisted of 1 or 2 members. 3. Analysis of factors influencing state anxiety. a. Gender; women patients were found to have a higher level of state anxiety than male patients. 4. The first hypothesis; the higher the level self-esteem, the lower the level of trait anxiety, was accepted. (r=0.481, p=0.0001). 5. The second hypothesis; the higher the level of self-esteem the lower the level of state anxiety, was accepted. (r=0.385, p=0.0001). 6. The third hypothesis; the higher the level of the trait anxiety, the higher the level of state anxiety, was accepted (r=0.675, p=0.0001).
