dc.description.abstractChildren with cerebral palsy suffer primarily from central motor disorder, developmental delay in cognitive, language skills and sensory-perception accompanied by emotional disorder. In order to lead an independent life, the children has to be exposed to active physical rehabilitation and therapy in different motor areas including gait movement. The purpose of this study was to examine how Rhythmic Auditory Stimulation, which is neurological music therapy approach can be used to enhance children s gait movement. Five children with cerebral palsy participated and received 23 sessions of music therapy for seven weeks. The music therapy session incorporated the RAS technique facilitating changes in velocity, cadence, stride length, gait cycle and symmetry ratio. Stride analyzer was used to collect data and the obtained data were analyzed using Student s t-test and Paired Comparison t-test. Results showed that the mean of the gait parameters increased as the sessions progressed. The data showed statistical significance for velocity(p=0.0097), cadence(p=0.0028), and gait cycle(p=0.0736) when the 23rd session was compared to the baseline data. Data collected from the post evaluation session were rather reduced since the last music therapy session, however, the obtained data were still higher than the baseline. Although results of this study have difficulty to be generalized since the number of case involved is rather small, the results certainly shows that the Rhythmic Auditory Stimulation triggers different responses in cerebral cortex, basal ganglia and the cerebellum which further brings improvement in gait movement. Therefore, structured use of rhythm in music therapy can be used to facilitate improvement in gait training and other motor function. This study is meaningful in that it suggest the possibility of music as an approach for the rehabilitation of patients who has a central motor disorder like children with cerebral palsy. ; 뇌성마비 아동은 뇌의 손상과 결함으로 인하여 중추성 운동장애를 가지고 있으며 이외에도 지능장애, 언어장애, 경련, 감각장애, 지각장애, 그리고 정서장애와 같은 중복적인 장애를 수반하고 있다. 그러나 이들의 독립적인 생활과 자립을 위해서는 무엇보다 운동영역의 재활과 치료가 절대적으로 우선되어야 한다. 즉, 이들의 삶의 질에 직접적인 영향을 미치는 중추성 운동기능의 장애중 하나인 보행장애를 개선하고 그 능력을 증진시킬 수 있는 다각적인 재활치료적 접근과 훈련 프로그램의 개발이 요구되어진다. 따라서 본 연구에서는 뇌성마비 아동을 대상으로 하여 신경학적 음악치료의 한 방법인 Rhythmic Auditory Stimulation(RAS)을 실시하고, 이들의 보행능력 증진에 미치는 RAS의 영향을 알아보고자 하였다. 연구를 통해 뇌성마비 아동을 위한 재활치료의 새로운 접근방법으로서의 음악치료의 가능성을 제시하고 그 유용성에 대해서 살펴보고자 한다. 본 연구에서는 뇌성마비아동의 보행능력증진을 그 목표로 하여, 7주간의 연구기간 중 치료전 평가와 치료후 평가를 제외한 5주 동안 신경학적 음악치료 방법 중의 하나인 RAS를 적용한 음악치료 프로그램을 구성하여 실시하였다. 음악치료는 총 23회 실시되었으며, 각 세션은 30분간 개별치료의 형태로 실시되었다. 각 세션은 인사노래(2-3분)-준비운동(4-5분)-RAS 보행훈련(20분)-헤어짐의 노래(2-3분)의 순서로 구성되었으며 음악치료의 효과를 높이기 위한 자가훈련(self training)이 병행되었다. 치료 전과 음악치료기간 중 매주 1회, 그리고 음악치료 종결후 1회의 측정을 통해 보행능력의 변화를 평가하고 분석하였다. 평가의 영역은 보행속도, 보행수, 보행폭, 보행주기, 그리고 보행균형율이며 정확한 측정을 위해 보행측정기구인 Stride Analyzer를 사용하였고, Student s t-test와 Paired Comparison t-test를 통해서 자료에 대한 통계처리를 하였다. 연구의 결과 음악치료 기간 중 모든 대상아동의 보행인자의 평균값이 점진적으로 증진되었으며, 보행속도와 보행수, 그리고 보행주기는 통계적으로도 유의한 증진을 보였다. 전체대상아동의 평균 보행속도는 치료전 33.81 m/min에서 5주 후에는 57.75 m/min으로 증진되었고, 평균 보행수는 치료전 78.64 steps/min에서 5주 후 99.74 steps/min으로 증가되었다. 평균 보행폭은0.83 m에서 1.17 m로, 그리고 평균 보행균형율은 0.66에서 0.85으로 각각 증가되었다. 또한 보행주기는 치료전 1.63에서 5주 후에는 1.23으로 감소되었다. 추후평가에서는 5주간의 음악치료기간에 증진되었던 보행인자들의 기능이 모든 대상아동에게서 조금씩 떨어지는 것이 관찰되었으나, 치료 전 보다는 증진된 것이었다. 이는 지속적인 음악치료의 필요성을 제시하며, 음악치료가 뇌성마비 아동의 보행능력 증진에 영향을 미친다는 사실을 반증한다. 치료 전과 추후평가시의 차이를 Paired Comparison t-test를 사용하여 분석한 결과, 5가지의 보행인자 중 보행속도와 보행수(p<.05), 그리고 보행주기(p<.1)는 각각 유의하게 증가 또는 감소되었으나, 보행폭과 보행균형율의 증가는 통계적으로 유의하지 않았다. 본 연구는 대상아동의 표본수가 적고 단기간의 연구기간동안 이루어졌다는 제한점을 가진다. 그러나 5주간의 음악치료는 뇌성마비 아동의 보행능력의 증진에 긍정적인 영향을 주었으며, 이를 통해 RAS가 뇌성마비아동의 운동중추인 대뇌피질과, 기저핵, 소뇌를 자극함으로서 이들의 보행능력의 증진에 영향을 준다는 사실을 입증하였다. 따라서 본 연구는 RAS를 적용한 음악치료가 뇌성마비아동을 비롯한 중추성 운동기능 장애를 가진 환자의 재활을 위한 하나의 접근법으로 사용될 수 있다는 가능성을 제시하였다는 점에서 의의를 가진다.-
