발달장애아동 어머니의 자녀 의사소통 문제 인식 및 지원요구에 관한 Q 방법론적 연구

Abstract

The purpose of this study was to examine and analyze the types and characteristics of the mothers of a child with developmental disabilities what they feel difficulties about child's communication problems and want they want for support needs. Q-methodology was employed in this study as a tool effective in analyzing subjective experiences. The method of study adopted was the Q-methodology whereby human subjectivity could be traced in depth from actors’ viewpoint to be objective. The Q-concourse for this study was formed through literature review, open questionnaire. The Q-concourse was classified into 17 categories, whereupon 72 Q-statements were classified out around Q-classification. Thereafter, 24 mothers, 12 autism spectrum disorders, 12 mental retardation were subject to Q-classifications, the results of which were analyzed using the QUANL PC program to divide the subjectivity to recognitions of disability into three types. The Q-methodology analysis revealed three (3) distinctive types. Type 1, ‘external communication’, consisting of 12 subjects, has 11 subjects are mother of mental retardation. They feel their children disable level mid line to mild . They have positive externally characteristics. Type 2, ‘internal analytic’, consisting of 8 subjects, has 7 subjects are mother of autism spectrum disorders. They feel their children disable level very severe. They have negative externally characteristics. Type 3, ‘media searching ’, consisting of 4 subjects, all of them are mother of autism spectrum disorders. They feel their children disable level mild to mid level. They seek to experience various media. This study may be significant in that it has identified three types of mother of developmental disable children’s unique recognitions of needs and thereby suggested some referential data useful to promoting the understanding of their conditions and welfare. The implications of this study for speech pathology practices can be featured as follows; Based on such understanding, this study tried to suggest an appropriate direction of intervention from the social welfare policy perspective for the alleviation of the needs experienced by mothers of developmental disabled children. In the field of speech-language pathology theory and practice, this study may be useful in that it has provided practical guidelines helpful to the speech-language therapist intervention in consideration of speech-language therapy conditions and disabled children’s parent’s needs, while being useful as basic data for relevant disciplines in charge of education, rehabilitation or welfare programs.;발달장애 아동의 언어 및 의사소통의 어려움은 언어학적 측면의 문제만이 아니라 사회적 결함이 동반되어 발달장애 아동의 의사소통 능력과 상대방을 이해하는 능력을 제한하게 하여 사람들과의 상호관계를 어렵게 한다. 이러한 의사소통의 문제는 상호관계의 문제이기 때문에 발달장애 아동 뿐 아니라 장애 아동의 어머니들에게 심리적인 스트레스를 발생시킨다. 또한 이런 심리적 스트레스로 인해 아동의 양육은 물론 일상적인 생활에까지 영향을 주는 경우까지 있다. 발달장애 아동의 지원에 대해서는 아동뿐만 아니라 가족구성원의 요구사항에 대해 심도 있게 파악하고 대책을 마련해야 한다는 주장이 커지고 있는 실정임에도 불구하고 아직까지 이런 현상들에 대해서는 제한적인 연구만이 이루어지고 있다. 본 연구의 목적은 의사소통장애를 가진 발달장애 아동의 어머니들의 주관적인 의사소통 문제와 요구의 유형과 특성에 관한 주관적 구조를 분석하고, 이를 통해 의사소통장애 아동의 어머니에 대한 이해를 넓히고, 어려움과 요구를 위한 실제적인 자료로써 이론적인 이해의 틀을 제공하고 효율적인 대처 방안을 제공하고자 한다. 현재까지 발달장애 아동의 의사소통 문제와 어머니의 요구에 대한 개별적이고 다양한 견해를 구체적으로 드러내기에는 양적인 조사만으로는 어려움이 있어, 주관적 요인을 주요 변인으로 하는 대안적 연구방법인 Q-방법론을 사용하고자 하였다. Q-방법론을 통한 구체적인 연구과정은 다음과 같다. 발달장애 아동의 어머니 24명(자폐 범주성 장애 아동의 어머니 12명, 지적장애 아동의 어머니 12명)을 선정하여 Q 분류를 실시하였다. Q 모집단은 설문조사와 문헌조사를 통해 의사소통 어려움과 요구에 관련된 진술문을 추출하였다. 다음으로 표집 된 진술문 중에서 내용의 중복과 표현의 명확성을 고려하여 수정한 Q-모집단을 의미와 주제별로 11개의 범주로 분류한 다음 각 범주에서 대표적이며 의미의 변별성이 높다고 생각되는 진술문 72개를 Q-표본으로 완성하였다. 총 72개의 진술문을 9 x 5 cm의 카드에 인쇄하고 코팅하여 연구대상자로 하여금 강제 정상분포가 되도록 각자의 의견의 중요도를 13점 척도 상에 Q-분류하도록 하였다. 마지막으로 Q-분류된 결과를 QUANL PC 프로그램으로 Q-요인분석을 하였다. 본 연구의 결과, 발달장애아동 어머니의 자녀 의사소통 문제 인식 및 지원요구는 세 가지 유형으로 나타났다. 세 가지의 유형은 전체 변량의 57.77%를 설명하고 있으며 각 유형별 설명력은 제 1유형이 33.19%, 제 2유형이 16.09%, 제 3유형이 8.49%로 나타났다. 제 1유형이 33.19%의 설명력을 가지므로 자녀와의 의사소통 문제와 요구의 구조를 가장 많이 설명하는 유형으로 볼 수 있다. 연구대상자 24명은 제 1유형이 12명, 제 2유형이 8명, 제 3유형이 4명으로 분할되어 나타났다. 각 유형은 다음과 같다. (1) 외적 소통형 제 1유형은 외적 소통형으로 명명하였다. 이 유형에 속한 어머니들은 전반적으로 긍정적인 경향성을 띠고 있으며 불만 섞인 요구를 표현하지 않고 아이의 장애에 대한 수용이 넓은 것이 특징으로 나타났다. 대상자 24명 중 제 1유형으로 분류된 대상자는 12명이다. 12명의 어머니 중 11명이 지적 장애 아동의 어머니로 분류되었다. (2) 내적 분석형 제 2유형은 내적 분석형으로 명명하였다. 이 유형에 속한 어머니들의 표면에 들어나는 성향으로는 모든 것에 부정적인 경향성을 띠고 있으며 사회에 대한 불만과 요구사항이 많고 극복의지 면에서도 약한 유형이다. 그러나 내면을 살펴보면 냉철하고 분석적인 꼼꼼함이 비춰짐을 알 수 있다. 내적 분석형에는 총 8명의 아동 중 7명이 자폐 범주성 장애를 보였다. 이 유형의 어머니들은 아이의 장애 때문에 생활이 어렵고 능력의 한계를 철저하게 인식하고 사회에 의존하고 보상받으려는 마음이 강하지만 겉으로 드러내지는 않는다. (3) 매체 탐험형 제 3유형은 매체 탐험형으로 명명하였다. 이 유형의 어머니들은 스마트폰 App, 로봇, 태블릿 PC 등의 다양한 매체를 원하는 호기심이 강한 유형으로 나타났다. 대상자 24명 중 제 3유형으로 분류된 대상자는 4명이다. 4명의 어머니 모두 자폐 범주성 장애 아동의 어머니로 분류되었다. 본 연구의 의의는 발달장애 아동 어머니들이 가지고 있는 자녀의 의사소통 문제를 알아보았고, 요구의 형태를 세 가지 외적 소통형, 내적 분석형, 매체 탐구형으로 유형화 하였다는 것이다. 또한 이러한 세 가지의 유형은 고정된 것이 아니라 이들이 처한 환경이나 상황과의 상호작용을 통해 지속적으로 변화될 수 있으므로 이를 토대로 어머니를 이해하며 바람직한 방향으로 변화시키고 그들의 증진을 위한 참조의 틀을 제시했다는 점이다.
