Abstract
연구 배경: 인간화 단일클론 항체 (humanized monoclonal antibody) 의 대표적인 약제인 rituximab은 주입 관련 부작용이 빈번하게 나타나는 대표적인 약제이다. 문헌에서는 초회 투여의 77 % 정도가 증상을 보인다고 나와있는데 국내 환자를 대상으로 현황을 파악하거나 위험 인자에 관한 연구가 많이 되어있지 않은 실정이다. 연구 목적: 본 연구는 국내 암환자를 대상으로 rituximab을 투여 받고 있는 환자에게서 주입 관련 부작용의 발생 현황을 파악하고 유발 위험 인자를 찾아 약제의 안전한 투여 계획을 수립하고 환자의 임상 관찰 시 참고자료로 활용되고자 하였다. 연구 방법: 2011 년 1 월 ~ 2013 년 3 월까지 국립암센터에서 rituximab을 항암 치료 목적으로 처음 사용을 시작하는 환자를 대상으로 약제를 투여한 시점부터 24 시간 이내에 rituximab과 연관된 주입 관련 부작용을 보인 경우에 증상이 있다고 정의하고 환자의 개별 인자들이 증상의 유무에 어떤 영향을 끼치는지 분석을 시행하였다. 환자의 기록은 전자의무기록을 후향적으로 검토하였다. 연구 결과: 총 110 명이 이번 연구의 대상이 되었으며 총 투여건수는 582 건이었다. 주입 관련 부작용의 발생 건수는 54 건 (9.3 %) 이었으며 110 명의 환자 중 50 명 (45.4 %) 에게 발생하였다. 위험 요인 분석 결과, 단변량 분석에서는 병기 (stage), rituximab의 투여 차수, 백혈구 (white blood cell, WBC), 림프구, 절대호중구수 (absolute neutrophil count, ANC), 젖산탈수소효소 (lactate dehydrogenase, LDH) 의 수치가 유의한 인자로 나타났다. 그리고 다변량 분석을 적용할 때에는 림프구수, 스테로이드의 사용 여부, 병기와 rituximab투여 차수에서 유의하게 나왔다. 결론: Rituximab의 투여 횟수가 적을수록, 특히 초회 투여 시, 그리고 림프구수가 높은 경우에 주입 관련 부작용이 많이 발현되며 스테로이드 약제의 사용이 주입 관련 부작용 발생률을 낮춰줄 수 있으며 병기가 낮을수록 부작용 발생률이 높아진다는 통계적으로 유의한 결과를 얻었다. 단변량 분석에서는 백혈구, 림프구, 절대호중구수의 수치가 높을수록 주입 관련 부작용 발생률이 높아지며 젖산탈수소효소의 수치가 낮을수록 주입 관련 부작용 발생률이 높아지는 통계적으로 유의한 결과를 얻었다. 따라서 치료 전 얻을 수 있는 환자의 혈액 검사 수치와 병기, rituximab의 투여 횟수, 스테로이드의 사용 여부에 관련된 자료를 활용하여 rituximab 주입 관련 부작용 가능성을 예측하고 그에 맞는 충분한 전처치와 환자를 주의 깊게 관찰하는 것이 필요하다고 판단된다.;Background: Rituximab is the first humanized monoclonal antibody approved for cancer therapy in 1997. Currently it is the most widely used to treat not only malignancies but also other immune related diseases. Infusion-related adverse event is the one of the representative side effects of rituximab. But there are not enough reports in the incidence and severity of the adverse events in Korean patients. Purpose: This study aimed to understand current frequency of rituximab infusion-related adverse event when the agent used to treat for non-Hodgkin's lymphoma at National Cancer Center (NCC) in Korea. It also tried to identify the risk factors related to those events. Method: A retrospective analysis using electronic medical records was conducted on cancer patients who started rituximab-included treatment at NCC during January 2011 to March 2013. Infusion related adverse events were collected when it occurred between at the beginning of rituximab infusion and within 24hrs. The relationship between infusion related adverse event and patient’s specific characteristics were analyzed to identify the associated factors. Results: 110 patients, total 582 cases were included in this study. Fifty patients (45.4 %) and 54 cases (9.3 %) were experienced rituximab infusion-related adverse events. In single variant analysis, we found that the stages of disease, numbers of rituximab administration, WBC counts, lymphocyte counts, ANC and LDH were associated factors of infusion- related adverse events. In multiple variants analysis, the stages of disease, lymphocyte counts, numbers of rituximab administration, and use of steroid were found to factors to be related to the rituximab infusion-related adverse events. Conclusion: The first time of exposure to rituximab is the most significant risk factor to predict occurrence of infusion related adverse event. In this study, we found that the first time rituximab exposure showed, higher infusion related adverse event. And higher lymphocyte counts were associated with the occurrence of adverse events. The use of steroid can reduce the chance of occurrence of adverse event. Also lower stage of cancer was a predictor of rituximab infusion-related adverse events. Thus, when patient uses rituximab for the first time, are diagnosed as the earlier stage of disease or doesn't have steroid prescription at the moment of infusion, and has higher lymphocyte counts, we should monitor carefully and consider more intensive premedication to prevent rituximab infusion-related adverse events.
