외장연결나사형 임프란트 Neoplant?와 3i-Osseotite?의 누년적 보존율 및 변연골의 변화

Abstract

결손치의 수복에 많이 사용되고 있는 외장연결 나사형 임플란트인공치(External-hex screw-type implants) 중에서 외형이 유사하고 고정체의 표면 처리를 달리한 국산 임플란트(Neoplant?)와 미국산 3i-Osseotite?의 누년적 보존율과 임플란트 주변 변연골의 변화량을 후향적으로 조사하고 비교하였다. 임플란트고정체(implant fixture)가 모래분사-산부식(sand-blasted acid etching : SLA) 표면 처리된 Neoplant?(Neobiotec Co., Korea)를 식립한 79명의 환자 232개의 임플란트를 2002년 1월부터 2005년 5월까지 41개월간 추적관찰하고, 이중산부식(dual acid etching : DAE) 표면 처리된 3i-Osseotite? (3i Co., USA)를 대상으로 하여 2000년 1월부터 2004년 7월까지 82명의 188개 임플란트를 55개월 동안 임상적 및 방사선학적으로 추적 관찰하여 각 임플란트의 누년적 보존율을 조사하고, 변연골의 변화량을 술후 6개월, 12개월에 계측하고 비교 평가하였다. Neoplant?는 총232개의 임플란트 중 2개가 2명의 남자환자에서 제거되었는데, 식립 부위는 상.하악 소구치부였으며 모두 골이식 등의 부가적 술식이 병행된 곳에 심어졌었다. 3i-Osseotite?는 총188개의 임플란트 중 8개가 2명의 여자환자와 3명의 남자환자에서 제거되었는데 상악에서 5개, 하악에서 3 개였으며 직경과 길이이가 다양하였으나 7개가 상악동 골이식 등 부가적 술식을 시행한 곳에서 실패하였다. 통계학적으로 누년적 보존율에서는 환자의 성별, 연령별 및 길이와 직경에 따라서 유의한 차이가 없었다. Neoplant?는 41개월(평균26±10) 동안 99.1%, 3i-Osseotite?는 55개월 (평균 47.7±16.9) 동안 95.7%의 누적 보존율을 보였다. 임플란트주위 변연골은 식립 6개월 후 Neoplant?는 0.2 ± 0.9mm, 3i-Osseotite?는 0.0 ± 0.8mm를 흡수를 보였고, 12개월 후에 Neoplant?는 0.4 ± 1.1mm, 3i-Osseotite?는 0.4 ± 0.9mm 감소하였으나, 두 임플란트 모두 만족할만한 임상 성적을 거두었다.;This is a retrospective study on the cumulative survival rate of Neoplant? (Neobiotec, Seoul, Korea) and 3i-Osseotite? (Implant Innovation, West Palm Beach, FL, USA) and to evaluate the changes in peri-implant bone level. Two hundred thirty two Neoplant? implants were placed in 79 patients between 2002 and 2005, and one hundred eighty eight 3i-Osseotite? implants were placed in 88 patients between 2000 and 2004. Medical records and radiographs were reviewed and analyzed to get overall survival rate by gender, age, fixture length & diameter, and position. Of the two hundred thirty two Neoplant? implants, 2 implants were removed and 41 months cumulative survival rate was 99.1%. Of the one hundred eighty eight 3i-Osseotite? implants, 8 implants were removed and 55 months cumulative survival rate was 95.7%. There are no significant differences in survival rate by genders, ages, length of implants and locations. But the implant diameter related with the implant survival rate on 3i-Osseotite?. Bone level surrounding implants were changed -0.2±0.9mm on Neoplant? and 0.0±0.8mm on 3i-Osseotite? implants after 6 months, and -0.4±1.1 on Neoplant?, -0.4±0.9mm on 3i-Osseotite? after 12 months.
