Abstract
근골격계 질환인 턱관절 장애는 스트레스에 영향을 받는다. 일반인보다 턱관절 장애의 지식도가 높은 치과위생사도 턱관절 장애 위험에 노출될 수 있고 관심과 시간 부족으로 턱관절 증상을 간과하거나 치료를 지연하는 경향이 있다. 턱관절 장애가 만성화되고 스트레스와 겹쳐지면 직무 활동에도 영향을 미치고 삶의 질이 저하된다. 이에 치과위생사 턱관절 장애의 자각증상을 조사하여 건강관리 수준을 점검하고, 턱관절장애 자각증상이 직무 스트레스와 관련이 있는지 알아보고자 본 연구를 시행하였다. 본 연구는 서울특별시 내에 소재하는 의과대학부속병원 치과에 재직하고 있는 치과위생사 143명, 치과대학부속 치과병원에 재직하는 치과위생사 86명 등 총 229명을 대상으로 하여 2013년 5월부터 6월까지 조사하였고, 수집된 자료는 SPSS(Version. 21.0)프로그램을 사용하여 통계 분석하고 다음과 같은 결과를 얻었다. 1) 대학병원에 재직하는 치과위생사의 40.6%(93명)가 관절잡음을, 31.4%(71명)가 턱관절에 동통을, 21.8%(50명)가 개구제한이 있다고 자각하고, 전체 치과위생사 중 53.3%(122명)가 최근 6개월 이내에 턱관절 자각증상을 경험하고 있었다. 2) 턱관절에 자각증상이 있는 치과위생사는 “업무 스트레스로 인한 피로함”에 유의한 차이를 나타냈다. 3) 턱관절 장애 자각증상은 치과위생사의 연령에 따라서는 유의한 차이를 보이지 않았으나, 임상 총 경력이 짧은 경우 개구제한에서 유의한 차이를 보였다. 4) 치대병원재직 치과위생사의 38.4%(33/86명)가 턱관절의 동통을 자각하였고, 의대병원재직 치과위생사의 27.3%(39/143명)보다 더 많았으나(p<0.05), 관절잡음과 개구제한에는 유의한 차이를 보이지 않았다. 5) 턱관절 장애 자각증상을 가진 치과위생사는 “업무 스트레스로 인한 피로함”과 유의한 상관성을 갖고, 턱관절의 동통은 허리와 골반의 동통 및 긴장성 두통과 관련 있으며, 관절잡음은 업무자세의 불편함과 상관성이 있었다.;This study was to investigate the relation between the job-stress and temporo-mandibular joint disorders of dental hygienists working at the university hopsitals. The study surveyed a total of 229 dental hygienists working at university hospitals in Seoul, Korea. The questionnaire consisted of 5 demographic features, 11 for symptoms of the TMJ disorder and 5 for job stress. The data were collected and analyzed statistically by frequency, Chi-square test and Mann-Whitney U test using the SPSS (Ver. 21.0) program. The results are as follows; 1) A total of 122 (53.3%) dental hygienists had TMJ disorder symptoms within recent 6 months; joint noise in 40.6%(93/229), pain in 31.0%(71/229), and limited mouth opening in 21.8%(50/229). 2) The dental hygienists who had TMJ disorder symptoms showed significant relation with “fatigue from job-stress". 3) There are no significant differences in TMJ pain and joint noise by the age, but significant in limited mouth opening for the short career hygienists. 4) TMJ pain were more frequent in dental hygienists who are working at dental infirmary as 38.4%(33/86) than those of medical hospital as 27.3%(39/143) (p<0.05), but there were no significant differences in joint noise and limited mouth opening between the groups. 5) The dental hygienists who had TMJ pain showed significantly related with "fatigue from job-stress", "back and pelvis pains", "tension headache", but there were no significant relation between "joint noise" and "uncomfortable posture".
