비만 아동의 습관 평가에 유용한 설문지 개발

Abstract

소아비만의 치료는 자세한 문진으로 신체 활동이나 식습관의 정도를 파악하여 행동을 변화시키는 것이 치료의 중심이 된다. 본 비만 클리닉에서는 부모용과 소아용의 기존의 설문지를 사용하여 과거력과 가족력, 생활 습관 및 식습관을 파악하여 왔는데, 좀 더 간단하고 유용한 설문지를 작성하기 위하여 본 연구를 시행하였다. 본원 비만 클리닉에 방문한 소아 94명에서 부모용과 소아용 설문지를 통하여 대상 소아의 습관을 살펴보았고, 부모와 소아의 대답의 일치도를 살펴보았다. 대상 소아는 대부분 신체 활동이 적고 좌식 생활을 주로 하였으며, 신체 활동을 싫어하는 경우가 많기는 하였으나 비만으로 인해 규칙적인 운동을 하는 경우가 50%에서 있었다. 아침식사를 하는 경우가 대부분이어서 식사 횟수는 3회가 많으며 식욕이 좋고 식사속도는 빠른 경우가 많았다. 즉석식품, 기름기가 많은 고기류, 기름에 조리한 음식의 섭취는 주 1∼2회가 많았고, 외식은 월 1∼2회가 많았다. 야식은 먹지 않는 경우가 많았다. 식습관으로 배가 부를 때까지 먹기, TV나 책을 보면서 먹기, 편식, 폭식의 빈도가 많았다. 중복된 문항에서의 답변의 일치도 비교에서 중등도 이상의 일치도를 보였다. 본 저자는 위의 결과와 문헌 고찰을 토대로 비만 환자를 외래에서 처음 진료할 때 생활 습관과 식습관을 효율적으로 파악하고자, 기존의 비만 설문지에서 부모의 설문과 소아의 설문에서 일치하는 부분을 빼고 보다 새롭게 작성하였다.

;Recently, the prevalence of childhood obesity has increased dramatically. Obese children develop serious medical and psychosocial complications, and are at greatly increased risk of adult morbidity and mortality. Therefore, the evaluation and treatment of obesity and obesity-related complication is essential. The treatment of childhood obesity focuses on using behavioral modification techniques to modify physical activity and dietary habit. The questionnaire is a practical method for evaluating physical activity and dietary habit in obese children. Both children and parents who had visited our obesity clinic completed the questionnaires about children's past and family history, physical activity and dietary habits. Our object was to revise the questionnaire for the purpose of providing practical guidance to pediatric clinicians who evaluate and treat obese children. We analysed the paired questions of both parents and childhood, and the results were found to be in moderate to high agreement. It is acceptable to complete question by either parents or children alone. We revised questionnaires (Table 8,9) by literature review and putting together the paired questions.
