Abstract
비만은 심혈관 질환의 위험 인자로서 관상동맥질환, 고혈압, 당뇨병과 흔히 관련이 있다. 비만에서 좌심실 수축 기능에 대한 연구는 많이 되어 있지만 비만 소아에서 좌심실 이완 기능에 대한 연구는 많지 않다. 비만 환자에서 고식적인(conventional) 도플러 기법은 해상도가 떨어지기 때문에 심초음파로 승모판 혈류를 측정하는 데 제한점이 많다. 비만 소아에서 좌심실 이완 기능을 평가하기 위하여 tissue Doppler imaging(TDI)과 고식적인 심초음파를 이용하였고 정상 소아와 비교 분석하였다. 10-12세 사이의 중등도 이상의 비만 소아 23명과 정상 소아 22명을 대상으로 체중, 신장을 이용하여 체질량지수, 비만도를 산출하였고, 생체전기저항법을 이용하여 체지방량, 체지방율, 복부지방율을 측정하였다. TDI와 고식적인 심초음파를 이용하여 좌심실 이완 기능을 나타내는 승모판 지수[peak early diastolic(E) 속도, peak late diastolic(A) 속도, E/A 속도비, isovolumic relaxation time(IVRT), deceleration time(DT)]를 측정하였다. 고식적인 심초음파로 측정한 결과에서 E 혈류 속도, A 혈류 속도, E/A혈류 속도비는 정상 소아와 비만 소아간에 유의한 차이가 없었으나 IVRT는 비만 소아에서 100.2 ± 28.0 msec, 정상 소아에서 86.9 ± 8.9 msec로 비만 소아에서 유의하게 연장되어 있었고, DT도 비만 소아에서 171.8 ± 17.3 msec, 정상 소아에서 100.7 ± 21.0 msec로 비만 소아에서 의미있게 연장되었다. 한편, TDI로 측정한 값에서 비만 소아가 정상 소아에 비해 E 심근 속도, E/A 심근 속도비가 유의하게 낮았고, A 심근 속도는 기저부에서만 비만 소아에서 유의하게 높았다. E 심근 속도는 체지방율(r=-0.391, P<0.05), 비만도(r=-0.253, P<0.05)와 유의한 음의 상관관계를 보였고, A 심근 속도는 체질량지수(r=-0.464, P<0.05), 비만도(r=-0.253, P<0.05)와 의미 있는 음의 상관관계를 보였다. E/A 심근 속도비는 복부지방율(r=-0.309, P<0.05)과 유의한 음의 상관관계를 보였다. 중등도 이상의 비만 소아에서 심실 이완 기능이 감소되었고, TDI가 고식적인 심초음파보다 더 예민한 검사 방법임을 알 수 있었다. 중등도 이상의 비만 소아에서 심초음파를 이용하여 심실 이완 기능을 주기적으로 측정함으로써 좌심실 이완 기능 이상을 조기에 발견할 수 있을 것으로 생각된다. ;Obesity is a risk factor for cardiovascular morbidity and is frequently associated with coronary artery disease, hypertension and diabetes mellitus. Although left ventiricular systolic function in obesity has been widely evaluated, the relation between obesity and diastolic function is not well understood. Conventional Doppler technique is limited by the absence of an adequate apical window to assess transmitral flow in obese patients. Transmitral indices were evaluated by tissue Doppler imaging(TDI) and conventional Doppler echocardiography to assess the influence of obesity on left ventricular diastolic function. In 23 obese and 22 normal children aged 10 to 12 years, height, weight, body mass index(BMI) and obesity index(OI) were measured. Fat mass, body fat percent and abdominal fat percent were estimated by bioelectrical impedance. Transmitral indices[peak early diastolic (E) velocity, peak late diastolic (A) velocity, and E/A ratio, isovolumic relaxation time (IVRT), deceleration time (DT)] were estimated by TDI and conventional echocardiography to evaluate left ventricular diastolic function. E myocardial velocity, myocardial E/A ratio were significantly lower in obese children than normal children. IVRT was prolonged in obese children (100.2 ± 28.0 msec) as compared with normal children (86.9 ± 8.9 msec ; P<0.05). DT was significantly prolonged in obese children (171.8 ± 17.3 msec) than normal children (100.7 ± 21.0 msec ; P<0.05). E myocardial velocity correlated negatively with body fat percent (r=-0.391, P<0.05) and with OI (r=-0.253, P<0.05). A myocardial velocity correlated negatively with BMI (r=-0.464, P<0.05) and with OI (r=-0.253, P<0.05). Myocardial E/A ratio correlated negatively with abdominal fat percent (r=-0.309, P<0.05). In conclusion, a decrease in early diastolic filling was noted in moderate degree of obesity and TDI is more superior diagnostic method than conventional echocardiography to evaluate left ventricular diastolic function. Diastolic dysfunction is an early indicator to detect cardiac involvement in obesity.
