dc.description.abstractIt was during the latter half of the 1950s that psychiarty bacame settled in Korea. At that time, it began to develop its own field independently and actively, no longer accepting western psychiatry as the standard. Since the latter half of the 1960s, the necessity of psychiatric social work has been recognized and discussed among the psychiatric specialists as well as social workers. However, the psychiatric social worker's role in the psychiatric field still remains at a low level. This research, therefore, deals with cases in which psychiatric patients unadjustable to society were readjusted to society with the assistance of a team of psychiatric specialists and scoial workers. In other words, through teamwork, psychiatric social workers could find and analyze the cause of adaptation failure in the patient's home life, social life and interpersonal relationships. Thus, they could undertake a part in solving the problems that the patient met with. Futher, they could provide data that would help to prevent a relapse of the disease or prevent the condition from becoming chronic, and also assist the patient to develop his potential ability and help him to take a new attitude toward life and society. The writer introduces here the method of group therapy as an approach to treating psychiatric patients. It is presented as an attempt to enter psychiatric patients into the therapeutic process for the social rehabilitation and to lead them to adapt harmoniously to society. As a result of the application of group therapy method as a means for social adaptation, the following advantages were found. First, among the group therapy members who formed inter-relationships, psychical functions, various emotional interchanges and identification were actively developed and phenomena of emotional catharsis were observed. Second, during the group therapeutic process, the group members could identify their unconscious impulses or negative emotions which they had not recognized before. Moreover, they generalized all such psychological problems, so that they could be relieved from their internal anxiety and conflict. Third, observing the psychological reaction of other members, they could understand the cause of their own disease from a new point of view, and have a corrective emotional experience with a deeper insight into their own disease. Finally, they could appreciate the concept of one's social role, social norm or order through the process of group therapy. Thus, they could promote friendship, responsibility, obedience and a sense of union in themselves. Al related above, group therapy has the great advantage of increasing one's capacity for social participation more than individual psychotherapy can. Also, the former takes less time for the therapist and requires less expenses from the patient than the latter, in order to establish therapeutic relationships. The subject of this study is about eight patients who had problems of social maladaptation. Among eight patients who were treated with group psychotherapy, five cases were found to adapt themselves well to the environment functionally as members of society, though they still felt a little difficulty in social life. Two cases aquired insight into the disease and understanding of the problem solving method, but their social adaptation was left much to be improved. The last case had need to change and showed attempts to do her best, but the reaction was far from satisfactory. To sum up, in the field of psychiatry, psychiatric social workers could contribute much to reduce the treatment time and play an important role to prevent the patient from relapse or developing chronic conditions, which are very much prevalent a among psychiatric diseases. They could do this by working in teams with psychiatrists and intervening in the therapeutic process of developing the social adaptation of psychiatric patients. Group psychotherapy, which psychiatric social workers could employ as an approach for the purpose of treatment, have a significant effect on treatment. Morever, in cases where each member had individual problems of his own, it brought more effective results by using combined treatment with individual psychotherapy.;근대 정신의학이 1950년대 후반에 들어 우리나라에 정착하면서 부터 종래의 서양 정신의학의 피동적인 수용에서 벗어나 자주적이고 능동적인 정신의학으로 발견되어 오늘에 이른 것은 주지의 사실이다. 정신의학의 발전과 더불어 1960년대 후반에 접어들어서는 우리나라에서도 정신의료사회사업에 대한 관심이 환기되었으며 특히 정신의학 영역은 물론 사회사업 분야에서 중점적으로 논의되어 이의 필요성에 대한 인식도 점차 증대되어 왔다. 그러나 이 분야에 직접 참여해야할 정신 의료사회사업가에 대한 인식은 물론 구체적인 역할에 대해서도 만족할만한 연구가 없었으며 정신의료사회사업가 자신의 역할도 현재로선 미미한 단계를 벗어나지 못하고 있는 실정이다. 따라서 본 논문은 사회적응상에 문제점을 가진 정신질환자들을 대상으로 신경정신과 전문의와 팀웍을 이루어 그들을 사회에 재 적응시킨 실제 사례를 통해 정신의료사회사업가의 역할이 문제해결의 중요한 분야를 담당하고 있었음을 확인하는 한편 환자의 개별적 제조건과 대인관계에서 야기되었던 적응상의 실패요인을 찾아내고 분석함으로써 정신질환의 재발이나 만성화의 요인을 억제하는 자료를 제공하고 뿐만아니라 환자의 잠재능력을 개발토록하여 새로운 생활자세를 갖추도록 유도해줄 수 있다는 결론을 얻었다.본 연구자는 정신질환자를 위한 치료에 개입하게되는 접근방법으로 집단치료 방법을 도입하였고 이를 통하여 정신질환자의 사회복귀를 위한 치료과정에 개입하여 이들이 원만한 사회적응을 하도록 시도하였다. 사회적응을 위한 치료도구로서 집단치료방법을 사용한 결과 다음과 같은 이점을 발견하였다. 1. 집단회원들은 상호관계를 맺음으로서 다양한 감정교류 및 동일시를 비롯한 정신기능이 활발하게 전개되어 감정의 정화현상이 잘 일어났다. 2. 집단치료과정중 회원들이 의식하지 못 했던 무의식적으로 일어난 충동이나 부정적연 감정을 이해할 뿐만 아니라 이러한 심리적인 제반 문제를 일반화시킴으로해서 내재되어 있던 붙안이나 갈등을 완화시킬 수 있었다. 3. 타 회원들의 심리적 반응을 관찰하므로 자신의 병적인 요인도 새로운 각도에서 파악하게되고 자신의 병에 대한 통찰력이 깊어짐에 따라 교정된 감정세계를 경험하게 되었다. 4. 집단치료과정을 통하여 인간의 사회적인 역할의 개념이나 사회규범 및 질서를 이해하게 되므로서 동료의식, 복종심, 및 책임감, 더 나아 가서 공동체의식을 부각시킬 수 있었다. 이상과 같이 집단치료는 개인치료보다 사회참여능력이 증가되는 큰 이점을 갖고 었었으며, 또 다른 일면에서 치료자의 시간절약과 치료대상자의 비용절약도 치료관계형성 조건상으로 보아 헛점이라고 할 수 있겠다. 본 연구자가 대상으로 하였던 사회 부적응의 문제를 가진 환자 8사례 중 집단치료를 통해서 5례는 스스로는 사회생활에 약간의 지장을 아직 느낀다고 하였으나, 사회인으로서의 기능적인 역할을 갖고 큰 문제없이 잘 적응하고 있었으며, 2례는 병에 대한 통찰력과 문제해결방법을 알고 있기는 하나 사회적응과정상 아직도 미숙한 상태였고, 1례는 변화하려는 욕구를 갖고 노력을 하려는 의욕을 보이기는 하였으나 그 반응은 미약한 편이었다. 결론적으로 정신과 영역에서 정신의료사회사업가는 신경정신과 전문의와 팀웍을 이루어 정신질환자의 사회적응을 위한 치료과정에 개입함으로써 치료기간을 단축시킬 수 있었고, 정신질환에서는 흔한 재발이나 만성화를 막을 수 있는 중요한 역할을 할 수 있었다. 그 치료목적을 위해 정신의료사회사업가가 사용할 수 있는 접근방법으로는 집단치료방법으로 치료효과를 볼 수 있었으나, 각 회원 특유의 개인적인 문제가 있을 경우는 개별치료방법도 병용해서 치료에 더 큰 효과를 가져올 수 있었다.-
