퇴원한 정신분열증 환자의 심리사회적 재활에 관한 사례연구

Abstract

정신분열증 (SCHIZOPHRENIA)은 인간이 가장 활발하게 사회에 적응하고 기능해 나가야 할 것이 기대되는 시기인 10대에서 30대 사이에 가장 많이 발병하는 대표적 정신장애이다. 이 장애를 지닌 환자들은 장애 자체의 문제 외에 사회로의 재적응 과정에서 큰 어려움을 겪고 있으며, 재발·재입원하는 경향이 높다. 환자들이 겪는 많은 문제점 중에서도 근본적인 문제는 심리사회적 측면에 관한 것으로 최근, 정신의료사회복지사들은 환자들의 치료 및 사회 기능회복을 위하여 예방과 재활영역에 관심을 두기 시작하였다. 그러나 퇴원 후 정신분열증 환자들의 심리사회적 재활을 위해 개입한 연구결과는 극히 부족하며, 특히 임상의 기초가 되는 사례는 절대 부족한 실정이다. 본 논문은 이러한 필요에 따라, 퇴원한 정신분열증 환자의 심리사회적 재활에 대해 연구자가 사회복지사로서 직접 개입한 임상사례연구이다. 본 연구의 대상은 K의료원에 입원했다가 퇴원한 21세의 남환자로 정신분열증 망상형(SCHIZOPHRENIA-PARANOIDTYPE)으로 진단되었던 경우이다. 연구자는 1989년 5월 - 1990년 1월 사이에 주 1회 이상 환자와 가족에게 개입하였고 환자의 담당의료진과 매주 TEAM-CONFERENCE를 가졌으며, 사후지도 (FOLLOW-UP)는 1990년 4월까지 이루어졌다. 대표적 케이스 워어크 이론인 F.Hollis의 심리사회적 접근론(PSYCHOSOCIAL APPROACH)을 재활도모의 이론적 틀로 사용한 본 연구의 결과는 다음과 같다. 1. 주로 사용된 F Hollis의 기법은 지지적 방법 (SUPPORTIVE METHOD), 지시적 방법(DIRECT INFLUENCE),환기법(VENTILATION) 혹은 정화법 (CATHARSIS), 클라이언트와 그의 상황·형태에 대한 반성적 고찰 (REFLECT10NAL DISCUSS10NS ABOUT CLIENT AND HIS SITUAT10NGESTALT), 환경조정(ENVIRONMENTAL WORK)등이었다. 각각의 기법의 활용을 보면, 지지적 기법은 개입의 처음부터 끝까지 두루 활용되었으며 관계형성과 집단, 신뢰감 진작에 큰 도움이 되었다. 지시적 방법은 변화시키고자 하는 문제에 직접적으로 개입할 때에 도움이 되었으나 초기단계 개입에서는 조심스럽게 시도되어지다가 점차로 비중을 늘려가며 사용되었다. 정화법은 환자와 환자부모 모두에게 쌓여있는 욕구를 발산하도록 하여 불건강한 상태를 지속시킬 여지가 있는 부분을 예방대처하는 데에 활용되었다. 중심이 되었던 것은 클라이언트와 그의 상황·형태에 대한 반성적 고찰이었는데 전반적 문제해결과 환자의 기능회복, 부모의 교육 등을 위하여 두루 활용되었다. 이것은 통찰력(INSIGHT) 개발에 큰 도움이 되었으며, F.HOLLIS의 중심기법이기도 하다. 마지막으로, 환경조정은, 치료와 재활을 분리해서 보지 않으며, 전반적인 회복을 위한 재활에 치료를 포함시키는 데 의의를 둔 본 논문에서 긍정적인 의미로 많이 사용되었다. 환경조정은 직접적인 정보제공·자원과의 연결 이외에 자원활용능력을 개선시킴으로써 환자의 기능회복에 직접적 영향을 (이후원문누락);Schizophrenia is a representative mental disturbance which happens at the age from 10 to 30 when people are expected to adapt themselves to society. And the patients who have got this disturbance have a great difficulty in overcoming the disturbance itself and in readapting themselves to society, and they have a tendency to relapse and rehospitalization. Of all the problems they suffer, the most important one is on the psycho-social view. In these years, psychiatric social workers have come to take an interest in prevention and rehabilitation to cure the patients and to recover their social function. But the case studies which are supposed to restore the psycho-social function are insufficient. Following this requirement, this thesis is a clinical case study in which the author has intervened as a social worker. The object of this study is a male-patient who was in K-clinic and was diagnosed as a Schizophrenia-paranoid type. Based on the theory of psycho- social approach of F. Hollis, the author intervened from May, 1989 to January, 1990 and the follow-up was made until April, 1990. During the intervention time, the author intervened the patient and all the members of his family at least once a week and held a weekly conference with doctors in charge of him. The consequences are as follows: 1. The main method used in this thesis are supportive method, direct influence, ventilation - or catharsis, and reflectional discussion about client and his situation, environmental work. 2. In this case, psycho-social approach is effective to the patient's improving personal relation, readapting family life and being able to planning and executing social life. And the author can find his restoration of daily habit and improvement of faculty of using social resources. As the result of the study, it was discovered that psychosocial approach of F. Hollis directly assisted in the rehabilitation of domestic schizophrenic patients. With various studies dealt in this thesis as the basis, and when the clinical studies developing professional knowledge and techniques of social workers continue, then effective and systematic social work services will be established that will aid the rehabilitation of numerous schizophrenic patients.
