노인환자의 영역욕구에 관한 연구

Abstract

The action for his own territory provides him with the mechanism which fulfills the basic needs. The hospitalized situation not only becomes a factory bringing about the vulnerability to the old patient, but also get them to perceive a threat of safety when they are intruded personal space and territories. The purpose of this study was provided theoretical understanding of the old patient's perceptions of the territorial intrusion in the hospital, and provided to the satisfication of the need. The subjects were 21 old patients in the general unit of one university hospital in Seoul. Data were collected from September 13 to 27, 1990 by non-structured interviews. Interviews were done by the researcher and all were tape-recorded. The data analysis was done by Van Kaam's phenomenological method. Validity assured by confirmation of the internal consistency of the statesments and cateogry by nursing colleagues in educational and clinicians in the general unit. From the emic data, 75 descriptive statesments were organized into 10 subcluster, 11 cluster and 3 levels. The results of study was summarized as follows : 1. The situation of the territorial intrustions : (47 raw data, 10 subcluster, 5 cluster, 3 level) · physical level-noise, crowding, visiting · psychological level-disregard · body level-exposure 2. Reaction to the territorial intrusion : (28 raw data, 6 cluster) · rage, unplesantness, discomfort, anxiety, withdrawl, defense The results contribute relevant factors to the territorial intrusion and the theoretical understanding grounded in subjective experience of the old patient as a basis for nursing action and thus will enhance their well-being and to provide the security;현대간호의 목표는 전인간호로써, 간호수행은 간호사 자신이 지닌 간호지식을 바탕으로 환자욕구에 대한 이해를 하므로써 이루어진다. 그중 영역에 대한 인간의 욕구는 보편적인 것으로, 영역소유를 위한 행동은 사생활, 안전, 자율성, 정체감에 대한 욕구를 충족시켜주는 기전을 제공하고 또한 환경속에서 안정된 상호작용을 확립하도록 해줌으로써 안전, 안정감의 욕구를 충족시켜준다. 특히, 노인은 모든 기본적인 욕구를 더욱 강하게 느끼는 시기로, 이러한 욕구가 충족되지 않았을 때 많은 문제에 당면하게 된다. 더우기 낮선 사람과의 상호작용이 필요하게되고. 신뢰할 수 있는 지지체제를 잃게되는 생소한 병원으로 옮겨지게 되는 것은 노인환자에게 취약성을 가져다주는 한 요인이 될 뿐만 아니라, 노인환자가 전체상황을 통제하기 어렵기 때문에 개인공간과 영역의 침해는 안전에 대한 위협으로 지각하게 된다. 이에 본 연구자는 전인간호의 견지에서, 노인환자의 기본욕구를 충족시켜주고 개인의 안녕을 증진시키기 위해, 노인환자가 경험하는 영역침해 현상을 파악하여 그 발생을 줄이기 위한 간호수행이 이루어지도록 본 연구를 시도하였다. 연구대상은 서울시내 A대학병원에 입원한 65세 이상의 노인환자 21명을 대상으로 하였다. 자료수집 기간은 1990년 9월13일부터 9월27일까지였으며, 비구조적 면담을 통해 자료를 수집하였다. 수집된 자료는 Van Kaam의 현상학적 분석방법을 이용하였으며, 분석된 내용의 신뢰도와 타당도는 석사과정생 2인과 임상 수간호사 3인에 의해 결정되었다. 본 연구결과를 통하여 얻은 결과는 총 75개의 진술로 10개의 하위속성과 11개 속성이 모여져 3개 차원으로 분류되었다. 1 . 노인환자에게서 나타난 영역침해상황은 물리적 차원에서는 소란, 밀집, 방문으로, 신체적 차원은 노출, 그리고 심리적 차원에서는 무시로 나타났다. 2 . 노인환자가 영역침해시 나타내는 반응은 분노, 불쾌, 불편, 불안, 위축, 방어로 나타났다. 이상과 같이 영역침해상황은 물리적, 신체적, 심리적 차원에서 다양하게 나타났고, 이로 인한 반응은 부정적 정서상태 및 관찰가능한 행위적 표현들로 나타났다. 이러한 현상들은 병원환경에서 쉽게 일어날 수 있는 일들이고, 의료진이 노인환자의 영역을 존중하지 않는 태도로 치료적 행위가 이루어지고 있음을 알 수 있었다. 이는 의료진이 치료중재를 제공할때 대상자의 안전을 보장하고 치료적 목적을 달성하기 위해 고려해야 할 요소로, 특히 대상자와 가장 많은 접촉을 갖게되는 간호사가 이러한 자료를 근거로하여 간호를 수행할 때 노인환자의 영역에 대한 욕구를 충족시켜 주도록 도와줌으로써 그들의 안녕에 크게 공헌할 수 있으리라 생각되며, 이에 노인환자의 개별적 간호접근을 위한 다각적인 연구가 있어야 할 것이다.
