Abstract
1920년대 초기에 이르러 가족이라는 개념이 지역사회건강간호의 사업단위로서 간호분야에 도입되었다. 가족은 개인의 성장과 최적의 건강을 돕는 환경을 제공하는 과업을 맡고 있다. 오늘날 의과학 기술의 발달로 만성적인 건강문제를 지닌 아동들이 생존하여 그들의 가족과 함께 살아가는 수가 증가하고 있다. 이러한 아동은 가족이 정상적으로 기능하는 것을 방해한다. 그러므로 가족이 기능하는 유형에 대한 지식과 가족기능수행에서의 변화를 설명할 수 있는 변수를 확인하는 것은 가족을 위한 간호중재를 계획하는데 유용할 것이다. 이에 만성적인 건강문제를 지닌 아동이 있는 가족과 건강하고 정상인 아동이 있는 가족이 기능하는 것을 비교하여 가족 중심의 간호를 하기 위한 기초 자료를 제공하고자 본 연구를 시도하였다. 자료수집기간은 1983년 9월 1일부터 9월 30일까지였으며, 연구대상은 서울 시내에 위치한 5개 종합병원과 1개 재활원에 만성질환으로 입원해 있는 6세 이하 아동의 부 혹은 모 54명과 서울 시내에 거주하며 만성질환을 앓았던 과거병력이 없는 건강하고 정상인 6세 이하 아동의 부 혹은 모 56명이었다. 연구도구로는 Feetham이 개발한 가족기능수행조사표(Feetham Family Functioning Survey)를 사용하여 자료를 수집하였다. 수집된 자료는 SPSS에 의한 전산통계처리를 하였다. 연구결과는 다음과 같다. 1. 총가족기능수행의 현재수준에서는 환아부모군이 정상아부모군보다 유의적으로 낮았으며 (P=0.020), 중요성(P=0.000)과 불일치점수(P=0.000)에서는 환아부모군이 정상아부모군보다 유의적으로 높게 나타났다. 2. 영역별 가족기능수행간에는 환아부모군과 정상아부모군에서 모두(가족내 구성원간의 관계)와 (가족과 친구나 친척간의 관계) 사이에 밀접한 상관관계가 있었다. 3. 가족이 기능하는 현재수준, 기대수준, 중요성, 불일치점수 간에는 환아부모군에서 현재수준과 기대수준(P<0.01),현재수준과 중요성(P<0.05), 기대수준과 중요성(P<0.01), 기대수준과 불일치점수(P<0.05), 중요성과 불일치점수(P<0.05)사이에 각각 양의 상관관계가, 현재수준과 불일치점수 (P<0.01)사이에 음의 상관관계가 있었으며, 정상아 부모군에서 현재수준과 기대수준 (P<0.01), 현재수준과 중요성 (P<0.01 ), 기대수준과 중요성 (P<0.01), 중요성과 불일치점수(P<0.05)간에 유의적인 양의 상관관계가 있었다.;The concept of family was introduced in Nursing as a unit of community Health Nursing in the early part of the 1920s. The family has the task of providing an environment conductive to growth and optimal health for individuals. Because of improved medical and scientific techniques, increasing number of children with chronic health problems are surviving and living with their families. The presence of a child with chronic health problems brings about changes in family relationships. Research indicates that change occur both within the family and between the family and other social systems. Knowledge of the patterns of family functioning and the identification of variables accounting for the greatest variance in family functioning may be useful to the nurse in planning the nursing interventions for the family. Therefore these is a need for systematic assessment of family functioning. The purpose of this study was to compare families with nomal healthy children and familiies with chronic health problems in relation to family functioning. In this comparative study the data were collected by a self-administered questionnaire. The Feetham Family Functioning Survey was used to obtain data concerning the parents perceived family functioning in the areas of the relationship between family and board social units, the relationship between family and subsystems, and the relationship between family and individual members. Subjects consisted of two non-random convenience sample. One sample consisted of 54 mothers or fathers of children with chronic health problems, who were less then 6 years old. The other sample consisted of 56 mothers or fathers of healthy normal children who were less than 6 years old. Data were analyzed using t-test and Pearson correlation Coefficient. The result of analysis were as follows: 1. Families with children with chronic health problems has significantly lower score of the amount of activity on the family function survey than families with healthy normal children. (P=0.020) Family with healthy normal children had significantly higher importance score (P=0.006) and greater discrepant score (P=0.000) on the family function survey than families with children with chronic health problems. 2. There was a close correlation between "the relationship between family and individual" and "the relationship family and subsystem" in both families. 3. There was a significant correlation among the amount of activity, the desired amount of activity, the importance score, the discrepant score in families with children with chronic health problems. There was a significant correlation between the amount of activity and the desired amount of activity, between the amount of activity and the importance score, between the desired amount of activity and the importance score and between the importance score and the descrepant score in families with healthy normal children.
