Abstract
본 연구는 현재 다수의 여성들이 병원분만을 소외감으로 경험하고 있지만, 병원분만 외의 대안이 부재한 상황에서 병원제도와는 다른 방식으로 실천되는 출산기술/지식에 대한 논의와 연구가 필요하다는 문제의식에서 출발하였다. 그 중에서도 현재 병원분만의 문제점을 극복할 수 있는 출산문화를 모색하고 있는 몇몇 여성들과 단체가 하나의 대안으로 생각하고 있는 조산사에 대한 본격적인 연구가 필요하다고 할 수 있다. 따라서 본 연구는 조산사와 산모들의 경험을 통해 조산사의 출산기술/지식의 특성은 어떠하고 그것은 어떻게 변화해 왔으며, 그들에 대한 사회적인 평가는 무엇이었는지에 대해 논의하고자 한다. 병원 외 공간에서 여성들의 출산과정을 돕는 조산사에 대한 연구는 현재의 출산문화에서 여성친화적인 출산기술/지식은 어떤 모습이며, 어떻게 가능한가를 비판적으로 논의하는 데 보탬이 될 것이다. 본 논문이 제기하고 있는 연구문제는 다음과 같다. 첫째, 출산문화에서 조산원은 어떤 위치를 차지하여 왔는가? 둘째, 조산사의 출산방식은 어떤 특성을 가지고 있는가? 그것은 어떤 점에서 병원에서 행해지는 분만방식과 구별되거나 유사하며, 그들의 출산기술/지식이 주변화되어가는 맥락은 무엇인가? 셋째, 병원을 선택하는 산모들의 변화된 요구에 대해서 조산사들은 어떻게 대응하는가? 이들의 노력은 병원제도와 어떻게 경합하고 적응하며 어떤 갈등을 겪는가? 이 과정은 조산사들의 정체감과 출산기술/지식에 어떤 영향을 미치는가? 이 논문에 사용된 연구방법은 심층면접으로, 12명의 조산사와 출산경험이 있는 4명의 여성 그리고 1명의 여성 산부인과 의사와 인터뷰하였고 연구결과는 다음과 같다. 한국에서 출산이 본격적으로 의료화되기 시작한 60년대 이전에 아이를 낳는 일은 의료적인 사건이 아니라 자연스러운 삶의 과정으로 여겨졌고 첨단의료기구나 과학적인 의료기술이 필요한 일은 아니었다. 이러한 상황에서 전통적인 산(産)할머니와는 달리 전문적으로 훈련된 간호-조산사(nurse-midwife)인 조산사들은 전문적인 의료인으로 여겨졌고, 40-50년대에 조산사를 불러 아이를 낳는다는 것은 중. 상류층 여성이거나 신식여성임을 드러내 주는 표지의 역할을 하였다. 조산사가 근대의료체계에서 교육을 받았다는 사실은 그들이 약물이나 의료도구의 개입을 완전히 배제하지는 않는다는 것을 의미한다. 그러나 그들은 전원이 여성으로 구성되어 있으며 지역공동체에 소규모 형태로 개업하고 있고, 출산의 감독자가 아니라 조력자로 교육받는다. 게다가 그들 스스로도 아이를 낳는 여성에게 필요한 도움을 주는 사람 이라고 정체화하며 이러한 사실은 그들을 제도화되고 의료자본화 되어가는 병원분만과 구별되게 만든다. 그들은 여전히 약물이나 외과적 개입을 최소화하는 손기술이나 비법들을 보유하고 있으며, 산모에 대한 지지나 격려가 출산을 가장 안전하게 만든다고 생각한다. 조산사의 이러한 출산기술/지식은 여성의 출산경험이 출산기술/지식으로 반영되고 되는 구조를 가지고 있으며, 여성과 태아를 통합적으로 생각하는 것이 가능하게 한다는 점에서 여성친화적이라 할 수 있다. 그러나 60년대에 산아제한을 목표로 하는 가족계획사업이 국가정책으로 실시되면서 출산에 대한 개념이 변화하기 시작한다. 출산은 이제 첨단의료기구와 위생성을 보장할 수 있는 병원시설만이 필수적인 의료적인 사건으로 바뀌게 되고, 아이를 낳는 여성들은 조산사의 출산기술/지식이 산모와 태아의 안전을 보장할 수 없는 낙후된 기술이라고 여기게 된다. 이러한 사회적인 변화는 한편에서 조산사들이 자신을 점점 더 강하게 근대적인 의료인으로 정체화하면서 현대적인 병원설비를 갖추기 위해 경쟁하게 하고, 병원과 마찬가지로 약물이나 초음파 기구를 일상적으로 사용하도록 만들기도 한다. 서구에서 조산사가 여성건강운동의 맥락에서 논의되고 실천되는 것과는 달리 현재 한국의 조산사는 스스로를 여성주의적인 의료실천을 하고 있는 사람으로 정체화하지는 않으며, (몇몇 소수를 제외하면) 그들이 의식적으로 여성친화적인 출산방식을 만들어내기 위해 고심하고 있는 것 또한 아니다. 그러나 한편에서는 이들 스스로가 출산을 경험하는 의료소비자이면서 출산과정에 조력자로 참여하는 의료제공자라는 사실은 여성의 출산경험이 의료기술/지식으로 전환될 수 있는 중요한 가능성을 가지고 있다. 이들의 출산기술/지식이 어떤 하나의 특성으로 고정되어 있는 것은 아니다. 그들의 출산방식이 현재의 병원분만이 가지는 부정적인 측면을 지양해 낼 수 있는 가능성을 가지고 있다면, 그것을 현실화해 내는 힘은 출산하는 여성들이 스스로의 몸에 대한 통제권과 자율성에 대해 비판적으로 사고하며 그것을 수용할 수 있는 출산방식은 어떠해야 하는가를 진지하게 고민하는 것이다. ; This study explores what midwifery technology have affected women s experiences of childbirth and how the social and cultural status of midwifery services has been changed in korea. Raised questions are as follows: First, what has been the position of a midwifery clinic in the culture of childbirth? Second, what is the characteristic of midwive s birth ways distinguishing with it in the hospital? Why has been marginalized their birth technology in some social contexts? Third, how have been coping with the situation that women choose a hospital instead of a midwifery? In addition, I researched how midwives compete, embrace, and conflict with an institutionalized hospital and how this has affected their identities and midwifery. This study mainly adopted an in-depth interview research technique, so I interviewed with 12 midwives, 4 women who delivered children, and a female obstetrician. The research findings are as follows. Before 1960s when the medical birth has pervaded in Korea, it had been not a medical event but a sort of normal bodily process. In 1940-50s, a woman who was helped by a midwife was considered as a upper-middle class or a modernized person. Midwives have trained in a modern medical education system, and this means they do not exclude to use drugstuffs or medical instruments completely. However, it can be pointed that midwives are consists of all women and their job itself is female-gendered. They mostly work in a local community, and they understand themselves not a professional supervisor in birth but a just helper. Therefore, it has effected making discrimination between a medical-capitalized hospital birth and a midwifery birth. The different thing with male-gendered medical birth is that midwives have been apt to not use drugs or surgical intervention. Instead, they have hand-skills and empirical therapies. They also think that the most important thing in birth is the safety of a mother and a baby, so they try to encourage and care for women s psychological stability. In this point, midwifery technology can be called a pro-woman way in birth because, above all, their midwifery has based on their own experience, thus, they do not objectify women. Rather, they tend to suppose a mother and a baby integrated one. By the way, while a family planning for a birth control has begun in 1960s, the concept of a childbirth started to change. It is understood women s safety only can be guaranteed by a modern hospital where is full of a high technology-oriented medicine and medical staffs. In contrast, midwive s birth technology has become to be expelled to the margin. Experiencing this social change, they have tried to identify themselves modern medical professional and they have equipped more modern appliances to compete with hospitals. For example, like as hospitals, they began to use ultrasound equipment whether necessary or not. Sometimes, they have in trouble with doctors to secure the newest medical instruments in a course of applying a qualification to the government. Using these modern instruments causes conflict not only with doctors but also between midwives themselves. The history of a midwifery in Korea has different context with western s which has reviewed on the female health movement in a feministic perspective. At present, Korean midwives do not regard themselves as pro-woman medical personnels, and they, except a few persons, are not especially conscious about a pro-woman birth method. Notwithstanding, they already have been doing to convert their own experience as consumers of medical services in birth into their medical performance. The fact that they excluded from the central knowledge called obstetrics, in other sense, tells us that they can accumulate their experience the birth technology. Although their competence with mega-capitalized medical institution seems to be much hard to extend, it has to be paid attention as the possibility of a feministic ways in birth continuously.
