Abstract
본 e환자분류체계에 의한 병원 간호인력의 적정 수요추정u 연구는 1983년 6월 1일부터 동 21일까지 E병원의 내과와 외과병동에 입원한 환자 연인원 1,055명과 간호요원 연인원 131명을 대상으로 환자분류연구, 직접간호활동연구, 그리고 간호활동연구방법을 통하여 (1)병원환자간호의 구성요소 및 이에 소요되는 평균 간호시간을 조사하고, (2)우리나라 병원환경에서 환자분류체계 개발의 가능성을 검토하며, (3)간호업무량을 추정하는 모형을 개발하고자 하였다. 이를 위하여 다섯 가지 연구 가설을 검정하였고 가설검정 결과에 근거하여 주어진 병동에서의 환자구성에 대한 직접간호업무량을 추정하기 위한 직접간호활동지표를 고안하였다. 환자분류연구를 위하여 대상환자를 개인위생, 자세, 식이, 정맥주사, 관찰 등으로 간호요구량에 따라 I군(경환자), II군(중등도 환자), III군(중환자)으로 분류하였으며, 직접가호활동연구를 하기 위하여 자료수집원이 조사기간 중 매일 24시간을 매 10분마다 환자를 순회, 관찰하면서 환자와 간호요원과의 의사소통, 투약 등을 포함하는 11가지 직접간호활동을 관찰하여 직접간호시각, 직접간호를 받은 환자, 직접간호를 한 간호요원의 수준을 조사하였다. 간호활동연구는 자료수집원이 조사기간 중 매일 24시간을 매 10분마다 간호요원을 관찰하면서 의무기록지 기록 및 확인, 투약준비 등을 포함하는 23가지 간호활동을 관찰하여 간호활동시각과 간호요원수준을 조사하였다. 24시간 동안 받은 직접간호시간은 환자군에 따라 통계학적으로 유의한 차이가 있었으나 동일환자군 내에서 환자의 연령군별, 성별, 병동별, 그리고 진료과목별 간에는 유의한 차이가 없어서 연구 가설 다섯 가지는 모두 기대한 바 대로 받아들여졌다. 24시간 동안 받은 직접간호시간 중에서 전문간호요원과 비전문간호요원 간의 직접간호시간의 비율은 I 환자군에서는 1:0.67, II환자군에서는 1:0.5, 그리고 III환자군에서는 1:0.33으로서 중환자군으로 갈수록 전문간호요원에 의한 직접간호시간의 비율이 증가되었다. 직접간호활동지표(I)는 I=0.3(R1+A1)N1+0.8(R2+A2)N2+1.7(R3+A3)N3이었으며 여기에서, Ni = i군으로 분류된 환자의 수 Ri = i환자군에게 간호한 직접간호시간 중 전문간호요원의 직접간호시간 백분율 Ai = i환자군에게 간호한 직접간호시간중 비전문간호요원의 직접간호시간 백분율 i는 I, II, III의 환자분류군이다. 한편 간호요원의 총 간호활동시간은 61,610분으로서 이 중 직접간호활동시간은 20.9%, 간접간호활동시간은 26.6%, 병동관리관련활동시간은 25.5%, 그리고 개인활동은 26.9%이었다. 총 간호업무량은 직접간호활동지표에 의하여 추정된 직접간호업무량과, 직접간호를 제외한 다른 간호업무량(WL)은 WL=I+49시간이었으며 여기에서 49시간은 1개 병동에서 1일간 직접간호활동을 제외한 총 간호활동에 요구되는 시간이다. 본 연구에서 독립변수로 사용한 직접간호활동지표, 간호요원수, 그리고 환자수는 생산적 간호활동을 종속변수로 사용한 회귀모형에서 94%를 설명하였다. ; This research was conducted (1) to determine the elements of nursing care activities and to measure the approximate time to perform them, (2) to assess patient classification system in order to demonstrate that the classification on acute nursing care in Korea as applicable, and (3) to develop the model which can estimate nursing work load through patient classification study, direct nursing care activity study, and nursing care activity study. The data for patient classification study and direct nursing care activity study were obtained from the 1st of June, 1983 to the 21of June, 1983. A total of 1,055 hospitalized patients (on a daily basis) in te medical and surgical wards at Ewha University Hospital were observed. The result showed that demand for direct nursing care varied with the patient categories which were classified according to the following five items: personal hypiene, posture, diet, intravenous injection, and observation. A trimodel classification scheme was developed which suggested three categories of patients where the researcher labeled minimal care (category I), moderate care (category II), or intensive care (category III). Statistical analysis showed that the average nursing care time for each category of patients were statistically significantly different. Therefore, in can be concluded that classifying patients into the three categories defined was valid. Category I patients received 20.3 minutes of direct nursing care during 24 hour period, category II patients 49.9 minutes, and category III patients 95.2 minutes respectively. The analysis of variance showed that this classification was applicable to the younger or older patients, male or female patients, and medical or surgical patients. Nursing care time by registered nurses was increased according to the severity of patient’s nursing care need categories. The direct nursing care requirement for a given patient mix on an acute care ward can be predicted by direct care index. The direct care index is as follows: I=0.3(R1+A1)N1+0.8(R2+A2)N2+1.7(R3+A3)N3 where Ni = number of patients in certain categories Ri = the ratio of direct care time by professional nursing personnel. Ai = the ratio of direct care time by nonprofessional nursing personnel. i = categories of patient (I, II, III) applicable to above Ni, Ri, and Ai Nursing care activity were observed from the 8th of June, 1983 to the 14th of June, 1983 totaling 131 nursing personnel on an 8-hour per day basis. They worked in the same medical or surgical wards at same hospital. The result showed that the total nursing activity hour was 61,610 minutes, direct nursing care activity was 20.9% (12,900 minutes), indirect nursing care activity 26.6% (16,410 minutes), ward management related activity 25.5% (15,720 minutes), and personal time 26.9% (16,580 minutes) in order respectively. The total work load of an acute care ward can be determined by: WL = I + 49 hours where the additional factor of 49 hours represents the average nursing time required for all other indirect nursing care activities, ward management related activities, and personal time. The regression equation model showed that selected variables (scheduled personnel, daily census, and direct nursing care index) explained a total of 94% for productive nursing care activities.
