Abstract
뇌졸중은 일반적으로 뇌혈관에 순환장애가 일어나 갑자기 의식장애와 함께 신체의 반신에 마비를 일으키는 급격한 뇌혈관질환을 말한다. 뇌졸중은 전 세계적으로 3대 사인의 하나로, 발병 후 1개월 이내에 약 25%가 사망하며 나머지 75%의 환자는 뇌 손상의 결과로 여러 가지 후유증을 남기게 된다. 즉 자가간호수행이 불가능하여 의존적이 되고, 우울, 좌절 등 심한 신체적, 정서적 고통을 경험하게 되며 이는 본인 뿐 아니라 가족에게도 영향을 주어 개인과 가족 모두의 안녕, 곧 삶의 질을 떨어지게 하는 결과를 초래하게 된다. 따라서 오늘날 뇌졸중 환자의 증가와 이에 따른 문제는 개인이나 가족의 차원을 넘어 지역사회의 심각한 문제로 지적되고 있다. 이에 본 연구는 뇌졸중 장애인의 신체, 심리, 사회 등 하위 영역을 포함하는 삶의 질의 향상에 초점을 맞춘 사회사업적 개입의 프로그램이 뇌졸중 장애인의 삶의 질에 영향을 미치는 지를 프로그램 미 참가자와 비교하여 알아보고, 뇌졸중 장애인의 사회사업적 개입의 활성화 방안을 모색해 보고자 하였다. 연구방법은 연구대상으로 C장애인복지관의 건강교실 프로그램 참가자를 연구집단으로 하였고, 비교집단은 동일지역 내에 노령인구 밀집지역에서 뇌졸중으로 파악되는 대상자를 편의 표집하였다. 프로그램 개입은 4월 2일부터 5월 4일 까지 주3회씩 총15회로 진행되었으며, 자료수집은 프로그램 종료시기에 연구집단과 비교집단을 각각 1:1 면접조사로 실시하였다. 측정도구는 방활란이 개발한 도구를 뇌졸중 장애인에 맞게 수정, 보완한 도구를 사용하였다. 수집된 자료는 백분율, 교차분석, t-test, 회귀분석 등의 통계방법으로 분석하였으며 자료분석을 통한 연구결과는 다음과 같다. 건강교실 프로그램에 참가한 연구집단은 삶의 질이 5점 척도 중 3.28점으로 조사되었으며, 프로그램에 참가하지 않은 비교집단은 2.78점으로 연구집단의 삶의 질이 높게 나타났으며 이는 통계적으로 유의한 차이가 있는 것으로 나타났다. 또한 삶의 질의 신체,심리,사회,경제 등의 6개 하위영역에서도 건강교실 프로그램에 참가한 집단은 참가하지 않은 집단에 비해 삶의 질이 높게 나타났으며 통계적으로 유의한 차이가 있는 것으로 나타났다. 결론적으로, 뇌졸중 장애인의 삶의 질의 각 영역에 초점을 맞춘 사회사업적인 개입으로서의 건강교실 프로그램은 사회사업적인 개입을 하지 않은 뇌졸중 장애인에 비해 삶의 질의 각 영역 모두에서 삶의 질이 높음이 확인 된 바, 건강교실 프로그램은 뇌졸중 장애인이 안고 있는 개인의 신체,심리적인 문제와 제한된 사회활동, 가족의 안녕 등 다양한 문제에 대해 사회사업적 개입이 거의 이루어지지 못하고 있었던 임상실무에서 구체적으로 적용하여 활용하는 하나의 방법이 될 수 있을 것으로 본다. ; A stroke relates to cerebral hemorrhage causing the paralysis of half the body accompanied by a conscious disorder due to the circulatory disorder of cerebral blood vessels. A stroke is one of the three causes of death in the world, and about 25 percent of the patients die within a month and the rest of them suffer from various sequelas. That is, home nursing is not available, and the patients are getting more dependent and experiencing serious physical and mental pain including depression and frustration. This is responsible for a decrease in the quality of life not only on the part of the patients but also on the part of their family members. An increase in the number of stroke-suffering patients and related problems come to the fore as serious problems of the communities where the patients and their family reside. In this context, this study is designed to shed light on the influences of social-work interference programs on the quality of the patients focusing on such subordinate areas as the patients bodies, psychology and their society and to grope for measures to activate social-work intervention for stroke-related disabled persons. The research group consisted of participants in the health promotion programs of C Welfare Center for Disabled Persons and the contrast group was composed of stroke patients in the same area as the research group. The program intervention lasted for a total of 15 sessions (three times a week) in the period from April 2, 2001 to May 4, 2001. And the researcher interviewed the subjects of both the groups on a one-to-one basis when the programs came to an end. The measuring tool was an adaptation of Bang Whal-ran s instrument, and the researcher depended on percentage, cross-tabulation analysis, t-test and regression analysis for the analysis of collected data. The findings based on the analysis can be summarized as follows. The research group stood for 3.28 scores out to five-point scale in terms of quality control, while the contrast group accounted for 2.78. The former turned out to be higher in terms of their life quality, which means that there is a statistically significant difference. And in terms of six subordinate areas such as physical aspect, psychology, society and economy etc, the former is higher than the latter, showing a statistically significant difference. In conclusion, the former is better than the latter in term of all the areas of life quality due to the social work intervention of health promotion programs. The programs can serve as a facilitator for clinical practice targeting stroke-related disabled persons.
