dc.description.abstractSince 1960’s, the family structure of Korea has been transformed vastly such as nuclear family, the minimize of the family unit, and the increase of elders who living apart from their children. It caused by rapid industrialization and urbanization. In addition, the increase of working females, the spread of individualism, and changes of children’s responsibility for their parents have directly lead to decrease the people who caring for old people. So, the growth of the older population becomes one of most important social problem. This paper deals with the problem that caring for the old people, more specifically, it focuses on the family structure that have aged family member who suffering from the stroke. The analysis of the family structure is largely in two ways. First, the current family structure and the relationship between the family members. Second, the effect of the stroke on the family structure. The result are as follows. Compare with the studies that the family structure of the older without their health condition, it shows that co-residence rate with married children was not particularly higher for families and the aged patients and the rate for other forms of residence were similar. This was different from the prior studies which showed that co-residence rate with children increased when the health of the aged parents deteriorated. In cases where the old parents co-resides with married children, the children were not always the eldest son, like the traditional way, but younger sons and daughters could be seen living with the parents, with some parents choosing to freely move from one family to another. When aged stroke patients were asked about their most desired form of co-residence and whether they were satisfied with their current living structure, 85.5% of them answered they were living in a family structure of their choice and 14.5% said they were not. With regard to satisfaction rate with their life, for those that were living apart from their children, they were most happy in situations where the children maintained a healthy relationship with the parents by living near by and visit frequently to look after the parents. In situations where the aged stroke patient lived with children, the level of emotional satisfaction was lower than that of physical comfort and had a better relationship with the spouse than the children. Spouses ranked the highest for the person who responsible for the caring patient and then the daughter and the daughter-in-law showed similar figures. The level of patient satisfaction was higher when the carer was the spouse or the daughter, compared with paid nurse or the daughter-in-law. However, the correlation has shown that ‘who is the carer’, and also, the patient satisfaction was not particularly significant. With regard to whether there was a change in the family structure because of the patient, 28.2% went through some change, whereas 71.8% did not. Where there was change, the change occurred over 1~4 times, with 1 time begin the most prevalent. The most common form of change, which made up 73% of the answers, consisted of the sick parent, who used to live apart, moving in with a married child. Most parents lived with the eldest son’s family, next, the daughter’s family, and then the form of moving from one family to another was prevalent. Then came the younger son’s family. Aged stroke patients who had to experience some change in family structure such as moving in with children were mostly over 70 years of age, female, whose family’s financial status was below average, financially dependent, and those who were alone without a spouse. On the other hand, those who maintained a separate household despite the sickness had a spouse who was able to take care of the patient and were financially independent, not meaning that they were financially well-off. Such couples insisted on living by themselves. Some patients lived alone even when they required full assistance in carrying on their daily lives. These people were extremely poor, mostly recipients of government aid, and that they had trouble on family relationships. They did not choose to live alone, but had to because of their poor circumstances. The above data, in comparison with other studies on the general sense of responsibility toward caring for the old, sense of filial duty based on Confucian teachings, and traditional values, show the process of how the aged, sick parent come to co-reside with a child’s family and the resulting changes in the family structure. By showing the diversity and the particularity of family structure with a sick and aged family member, I hope that the data provided here could contribute to the development of more social programs for the aged suffering from chronic ailments. ; 1960년 이후 급격한 산업화, 도시화로 인하여 우리 나라의 가족구조에는 핵가족화, 가족인원의 소규모화, 자녀별거 노인가구의 증가현상이 두드러지게 되었다. 또한 여성의 사회진출 증가, 개인주의 가치관의 확산, 부양의식의 변화는 노인부양인력의 절대적 감소로 직결되어 오늘날의 노인 고령인구 증가현상은 한국사회에 노인부양의 문제를 야기하고 있다. 본 연구는 노인부양의 문제 그 중에서도 특히 노인의 건강상태가 좋지 않을 때 가족의 부양이 어떻게 이루어지는지 뇌졸중 노인환자의 가족구조를 통하여 살펴보았다. 노인환자의 가족구조는 크게 두 가지로 나누어 살펴보았다. 첫째, 뇌졸중 노인환자의 현재 가족구조 및 가족관계를 살펴보고 둘째, 노인의 뇌졸중 발병이 가족구조의 변화에 어떠한 영향을 미치는지 알아보았다. 연구결과는 다음과 같다. 뇌졸중 노인환자의 현재 가족구성 형태를 보면 노인의 건강상태에 관계없이 모든 노인들을 상대로 조사한 기존의 연구와 비교하여 기혼자녀와의 동거율이 높지 않았고 가족구성 유형별로 비슷한 경향을 나타내었다. 즉, 노인의 건강이 약화되면 자녀와 동거하게 될 가능성이 높아진다는 선행연구에 부합하지 않았다. 기혼자녀와의 동거는 노인부양이 전통적으로 장남에게 전적으로 전담되었던 것과는 달리 기타 아들가족이나 딸가족, 그리고 아들, 딸 구별 없이 모든 자녀들이 교대로 부양하는 순환형태 등 다양한 모습으로 이루어지고 있었다. 동거희망가족의 형태와 일치여부에 대해 살펴보면, 뇌졸중 노인환자의 85.5%는 현재 자신이 원하는 동거가족의 형태로 살고 있었고, 일치하지 않는 경우는 14.5%였다. 생활만족도는 자녀와 별거하고 있는 노인환자의 경우, 특히 자녀가 가까운 거리에 살면서 자주 찾아와 환자의 생활을 보살피는 긍정적인 관계가 형성될때 더욱 만족스럽게 나타났다. 자녀와 동거하고 있는 환자의 경우, 정서적 부양에 대한 만족도는 신체적 부양에 대한 만족도 보다 낮았고 자녀와의 관계보다 배우자와의 관계에 더 만족하였다. 주간호자의 유형을 살펴보면, 배우자가 자녀와의 동거여부에 관계없이 가장 많았다. 자녀동거 가구에서는 배우자 다음으로 며느리와 딸이 비슷한 비율을 차지하였고, 자녀 별거가구에서는 배우자 다음으로 간병인이나 가정도우미 등이 가장 많았다. 뇌졸중 발병으로 인한 가족구조의 변화를 살펴보면 발병 후 동거가족에 변화가 있었던 가족은 28.2%이었고, 변화가 없었던 가족은 71.8%이었다. 동거가족의 변화를 경험한 경우 변화횟수는 1회부터 4회까지 다양하였으나 1회가 일반적이었고, 변화형태는 자녀와 별거상태에서 기혼자녀와 동거하는 형태로 변화되는 경우가 73.0%로 가장 많았다. 기혼자녀와의 동거하게 되는 경우 장남가족과의 동거가 가장 많았고, 다음으로 딸가족, 순환형태, 기타 아들가족 순이었다. 뇌졸중 발병 후 자녀와의 동거와 같은 가족구조의 변화를 경험하게 되는 경우는 70대 이상의, 여자환자 일수록, 경제적 수준이 중 이하인 경우, 경제적으로 의존할수록, 배우자 없이 혼자사는 노인일수록 많이 발생하였다. 반면에 뇌졸중 발병에도 불구하고 가족구조의 변화 없이 자녀와의 별거를 유지하고 있는 경우는, 특히 노부부 가구일 때 배우자가 환자를 돌 볼 수 있고, 환자의 경제적 수준에 관계없이 경제적 자립이 가능할 때 자녀와의 동거보다는 별거를 적극적으로 선택하였다. 그러나, 발병으로 일상생활 수행능력이 의존적이지만 자녀와의 동거변화 없이 노인 혼자 살고 있는 경우는 생활보호대상자와 같은 저소득층의 노인들 가운데 자녀와의 관계가 원만하지 않거나 관계가 좋다고 하여도 워낙 형편이 어려워 혼자 살 수 밖에 없었다. 이와 같은 연구결과를 통해 노인부양 의식이나 효 의식, 전통적 가치관에 대한 연구와 비교하여 실제 노인이 발병하였을 때 가족과의 동거가 어떻게 이루어지는지 또 가족구조의 변화가 어떠한 양상으로 이루어지는지 알 수 있었다. 그리고, 뇌졸중 노인환자의 가족구조를 살펴봄으로써 만성질환 노인의 가족구성 형태별 특성에 따른 다양한 복지정책을 개발하는데 기초자료를 제공할 수 있으리라 기대 한다.-
