Abstract
현대사회에서 비만증의 조절은 점차 중요한 의학적 과제로 대두되게 되었다. 비만증의 조절을 위해 금식요법을 실시함에 체중이 감소되는 것 외에 내분비학적 혹은 대사적인 변화가 초래된다. 내분비학적인 변화로는 시상하부-뇌하수체-난소축의 기능변화, 무월경이나 초경의 지연, 기능성 자궁출혈, 황체기의 결합등 생식기능의 장애가 임상적 문제가 된다. 이러한 금식요법을 할 때 동반되는 내분비학적인 변화의 기전을 규명하기 위해 폐경기 비만증환자 10명(평균 연령 53.2세 ; 평균 35.8% 과체중)을 대상으로 10일간의 단기간 금식요법을 실시하여 혈청 및 뇨중 생식선자극호르몬과, 혈장 catecholamine, 혈청 melatonin, 뇨중 cortisol 등의 자극호르몬을 측정하고, 외부에서 Luteinizing hormone releasing hormone(LHRH)을 주사한 후, 혈청 생식선자극호르몬의 농도변화를 관찰하여 다음과 같은 결과를 얻었다. 1. 금식기간중 뇨의 Luteinizing hormone(FSH)의 배설이 증가되었으나 혈청 LH와 FSH의 농도는 변화되지 않았다. 2. 외부에서 LHRH를 주사하였을때, LH와 FSH의 농도는 변화되지 않았다. 3. 금식기간에 자극호르몬중 norepinephrine과 melatonin이 증가되었다. 따라서 단기간의 금식요법을 할 때, 생식계의 혈중 호르몬의 변화는 일어나지 않고, 뇨중 LH와 FSH의 배설이 증가되는 것은 저분자량단백질의 신장내 배설기전의 변화 때문이며, 신경적인 자극이 유도되나 LH와 FSH의 분비양상에는 영향을 주지 않았다. ; Fasting in obesity results in weight loss as well as endocrinologic and metabolic changes. In young women, delayed menarche, dysfunctional uterine bleeding, inadequacy of luteal phase, and amenorrhea occur. The goal of this study was to elucidate the mechanism of endocrinologic changes accompanied by fasting. Ten obese postmenopausal women were studied for 10 days of fasting. Blood and urinary levels of pituitary gonadotropins(LH and FSH), their responses to LHRH stimulations, and hormones alleged to be stress markers such as plasma epinephrine, norepinephrine and dopamine and urinary excretion of cortisol were measured. The results were as follows ; Daily serum LH and FSH concentrations did not change significantly during fasting. In contrast, the urinary excretion of gonadotropins increased significantly during fasting (p<0.01). Significant changes in LH or FSH responses to LHRH were not present during fasting. Plasma norepinephrine concentrations and serum melatonin concentration during fasting increased significantly (p<0.01), while plasma epinephrine, dopamine and urinary excretion of cortisol did not change significantly during fasting. In conclusion, the stress of short term fasting activates only certain components of the neuroendocrine system without any appreciable changes in the function of the gonadotropin secreting system. Fasting induced gonadotropinuria may be due to altered renal handling of hormone molecules.
