최근에 작용시간이 짧은 정맥마취제의 소개로 전정맥마취(total intravenous anesthesia)에 대하여 많은 관심이 유발되고 있다. 그러나 대부분의 정맥마취제가 심혈관계를 억제하기 때문에 통상적으로 정맥마취제를 사용할 때 심혈관계의 안정성에 대한 평가 요소로 평균동맥압, 심박출량에 대하여 평가가 주로 이루어지나 정맥마취제가 정맥용량에 미치는 영향에 대한 평가는 매우 드물다. 심혈관계에 미치는 영향에 대한 평가에서 정맥계는 혈액의 70%를 포함하는 혈류의 도관이고, 순환 혈액의 저장소로서 순환을 통제하며, 정맥용량(venous capacitance)과 정맥저항(venous resistance)은 심박출량을 조절하는데 관여하기 때문에 정맥계는 정맥마취제 또는 혈관작용성 약물에 대한 심혈관계 반응에 있어서 매우 중요하다. 그러나 정맥계는 심혈관 반사계에 의하여 조절되기 때문에 분리된 정맥 조직표본은 정맥긴장도에 대한 정보를 얻기 힘들고 기술적인 어려움 때문에 정맥계에 대한 약리학적 연구가 적은 편이다. 따라서 저자는 본 연구에서 백서를 대상으로 마취유도시 심혈관계 안정성을 나타내는 마취제인 ketamine이 정맥용량에 미치는 영향을 평균 순환계 충만압(mean circulatory filling pressure)을 측정하여서 평가하고, ketamine을 투여한 상태에서 교감신경절차단제인 hexamethonium chloride를 투여하고, hexamethonium chloride를 전처치한 상태에서 ketamine을 투여하여 ketamine이 정맥용량에 미치는 영향과 교감신경활동도 사이의 관계를 관찰하였다. 본 연구의 결과 정상혈량상태에서 교감신경 활동도가 유지될 때 ketamine은 정맥용량을 유지시키나, 교감신경활동도가 차단된 상태에서 ketamine 자체는 정맥용량을 증가시키기 때문에 ketamine이 정맥용량을 유지시키는 기전은 교감신경활동도의 유지에 의하여 나타남을 알수 있다. ; As ketamine does not suppress sympathetic nervous activity ketamine may be drug of choice for hemodynamic stability in the intravenous anethesia induction for mild hypovolemic patient. The purspose of this study was to examine the effect of ketamine on venous capacitance in the normovolemic rats and to test hypothesis: the venous capacitance change produced by ketamine is a result of activation of intact sympathetic nervous activity in the normovolemic rats. The 60 Sprague-Dawley rats previously anethetized with pentobarbital were divided into three groups. All the rats are fully recovered from previous pentobarbital anethesia for surgical preparation. The awake group(A, N=20) underwent intraperitoneal administration of ketamine(intraperitoneal 125 ㎎/㎏) after full recovery from pentobarbital anethesia. The ketamine pretreatment group(K, N=20) underwent intravenous administration of 10 ㎎/㎏ hexamethonium chloride in the intraperitoneal ketamine 125 ㎎/㎏ preanesthetized state. The hexamethonium pretreatment group(H, N=20) underwent intraperitoneal administration of 125 ㎎/㎏ ketamine in the inravenous hexamethonium chloride 10 ㎎/㎏ pretreatment state. Venous capacitance was assessed by measuring the mean circulatory filling pressure(MCFP). MCFP was measured during brief periods of circulatory arrest produced by inflating an indwelling balloon in the right atrium. Mean aterial pressure(MAP), heart rate(HR), central venous pressure(CVP) and MCFP was measured before and after ketamine injection in A group. Same parametus was measured before and after ketamine injection in B group and before and after hexamethonium injection in C group. The result are as follows: 1) Ketamine did not change MAP, HR, MCFP. 2) Hexamethonium decreased MAP, HR and MCFP significantly in the ketamine pretreatment group(P<0.05). 3) Ketamine decrease MAP, HR, MCFP significantly in the hexamethonium pretreatment group(P<0.05). These results demonstrated that ketamine has venodilating property in the state of sympathetic nervous activity block but venoconstricting property in the state of intact sympathetic acticity. The results suggest that the maintained venous capacitance produced by ketamine via activation of intact sympathetic nervous activity may contribute to a maintained mean arterial pressure in the awake normovolemic rats.
