Abstract
시험관아기시술(in vitro fertilization and embryo transfer) 및 생식자 난관내 이식술(gamete intrafallopian transfer)등의 보조생식술 시술시 마취는 필수 불가결하다. 경질초음파기기(transvaginal ultrasonography)를 사용한 난자의 채취와 생식자 난관내 이식술시 생식자의 난관내 이식을 용이하게 하고 환자의 불안감, 통증해소 목적으로 마취를 하는데 이때 난자는 난포액 또는 난관액 내에서 마취제에 노출되게 된다. 이러한 보조생식술 시술시 사용할 마취제 또는 마약제제를 선택할 때 수정(受精, fertilization)과 초기 배아과정에 나쁜 영향을 줄 수 있는 약제를 피해야 되나, 어떤 약제가 안전한가에 대한 연구는 아직 미비한 실정이다. 따라서 본 연구에서는 생쥐난자의 체외수정을 모형으로 하여 휘발성 마취제와 마약제제가 난자의 난할 및 초기 배아 발생에 미치는 영향을 관찰하고자 하였다. C57BL/6와 CBA계 생쥐를 교배시켜 얻은 제1대 잡종(C57BL/6 X CBA)의 암컷과 ICR계 숫컷 생쥐를 이용하여 본 연구를 시행하였다. 암컷 생쥐에서 과배란을 유도하여 획득한 난자를 휘발성 마취제인 enflurane 0.5mM, 1.5mM과 isoflurane 0.26mM, 0.78mM 및 halothane 0.24mM, 0.72mM 그리고 마약제제인 meperidine 1.0μM, 3.6μM과 fentanyl 6.0nM, 30.0nM에 각각 노출시켰고, 약제에 노출되지 않았던 생쥐난자를 각각 휘발성 마취제와 마약제제의 대조군으로 사용하였다. 이런 약제들에 노출된 생쥐난자의 수정(受精, insemination) 후 제1일에서의 퇴화 및 분절된 난자율, 난할율, 제4일에서의 상실배이상의 발생율, 제 3-8 세포기에서의 발생중지율 및 제6일에서의 포배 발생율과 그 부화율을 각각 약제에 노출되지 않았던 대조군과 비교 분석하여 다음과 같은 결과를 얻었다. 1. 휘발성 마취제들에 노출되었던 생쥐난자의 수정후 제1일에서의 난할율은 대조군과 차이가 없었으나, 퇴화 및 분절된 난자율은 휘발성 마취제중 isoflurane 0.78mM군에서만 대조군보다 유의하게 높았다. 2. 휘발성 마취제중 isoflurane 0.78mM군, halothane 0.24mM군 및 0.72mM군에서 수정후 제4일에 상실배 이상의 발생율이 대조군보다 유의하게 낮았으나, 제3-8 세포기에서의 발생 중지율은 대조군보다 유의하게 높았다. 3. 생쥐난자의 수정후 제6일에서의 포배 발생율은 isoflurane 0.78mM군, halothane 0.72mM군에서 대조군보다 유의하게 낮았으나 enflurane군, isoflurane 0.26mM군 및 halothane 0.24mM군에서는 유의한 차이가 없었다. 4. 휘발성 마취제들중 halothane 0.72mM군에서만 수정후 제6일에서 부화 포배 발생율이 대조군보다 유의하게 낮았으나, 일단 포배까지 발생한 경우 이런 포배의 부화율은 enflurane 0.5mM군과 isoflurane 0.26mM군에서 대조군보다 유의하게 높았다. 5. Meperidine 또는 fentanyl에 노출되었던 생쥐난자의 수정후 제1일에서의 난할율, 퇴화 및 분절된 난자율과 제4일에서의 상실배 이상의 발생율 및 제 3-8 세포기에서의 발생중지율은 각각 대조군에 비해 유의한 차이가 없었다. 6. Meperidine군과 fentanyl군에서 수정후 제6일에서의 포배 발생율은 대조군과 유의한 차이가 없었다. Meperidine 3.6μM군과 fentanyl 30.0nM군에서 포배의 부화율이 다소 높은 경향을 나타냈으나, 대조군에 비해 유의한 차이는 없었다. 이상의 결과로 사람의 치료 용량 범위내의 meperidine과 fentanyl은 생쥐난자의 초기 발생과정에 영향을 주지않으며 휘발성 마취제들 또한 생쥐난자의 난할에 영향을 주지 않으나 고농도의 isoflurane과 halothane은 제 3-8 세포기에서의 발생중지를 증가시키며 상실배 이상의 발생율을 저하시킴으로써 생쥐난자의 체외수정에 유해한 영향을 끼친다고 생각된다. ; Anesthesia is prerequisite to assisted reproductive technologies such as in vitro fertilization and embryo transfer(IVF-ET) and gamete intrafallopian transfer(GIFT). When choosing the anesthetic agents to facilitate laparoscopic transfer of oocytes and sperms during GIFT procedures and transvaginal ultrasonographic harvesting of oocytes, it is essential to rule out adverse effects of these drugs on fertilization and subsequent development. Unfortunately very little information is available to ascertain if any of the various anesthetics used in these procedures are toxic to the oocyte and developing preimplantation embryo. The purpose of these studies was undertaken to investigate the effects of volatile anesthetics and narcotics on the fertilization and early development of embryos using in vitro fertilization model of mouse oocytes. In the first experiment, mouse oocytes obtained from F1 hybrid(C57BL6 X CBA) were randomly exposed to three volatile anesthetics, enflurane(0.5mM; 1.5mM), isoflurane(0.26mM; 0.78mM) and halothane(0.24mM; 0.72mM), and in the second experiment they were exposed to two narcotics, meperidine(1.0μM; 3.6μM) and fentanyl(6.0nM; 30.0nM) respectively. In both experiments mouse oocytes unexposed to any drugs were served as controls. In vitro developmental patterns were observed on the first, fourth and sixth day after insemination of above oocytes. The results were as follows. 1. Any volatile anesthetics studied had no effect on the cleavage rate of mouse oocytes on the first day after insemination, but the incidence of degenerative and fragmented oocytes in 0.78mM isoflurane group increased significantly compared with control. 2. The rates of mouse embryos developed over morula stages on the fourth day after insemination were significantly lower in 0.78mM isoflurane and halothane group than in control. The rates of embryos arrested at 3-8 cell stage in these groups were significantly greater than that of control. 3. The percentages of mouse oocytes developed to blastocysts on the sixth day after insemination decreased significantly in 0.78mM isoflurane and 0.72mM halothane group compared with control. 4. The hatching rate of blastocysts developed from mouse oocytes preexposed to 0.72mM halothane was significantly lower than that of control, but the proportions of hatching and hatched blastocysts among total blastocysts were significantly greater in 0.5mM enflurane and 0.26mM isoflurane group than in control. 5. Mouse oocytes preexposed to narcotics had a rate of degeneration and cleavage comparable to that in control on the first day after insemination. Also no significant differences in the rates of embryos arrested at 3-8 cell stage on the fourth day after insemination were noted. 6. There were no significant differences in blastocysts development and their hatching on the sixth day after insemination between the groups preexposed to either fentanyl or meperidine and control. In conclusion these results indicate that narcotics, fentanyl and meperidine does not show adverse effects on in vitro fertilization of mouse oocytes in concentration calculated to approximate those to which human ova would be exposed during clinical anesthesia and that volatile anesthetics such s isoflurane and halothane is detrimental to in vitro fertilization of mouse oocytes in high concentrations.
