Abstract
본 연구는 4년제 간호대학생을 대상으로 임상실습 만족도, 임상실습관련 자기효능감을 파악하고 이들 간의 상관관계를 규명하여 효과적인 임상실습 교육에 필요한 기초 자료를 제공하고자 시행되었다. 연구대상은 전국의 4년제 간호학과 중 8개를 편의 추출한 후, 2012년 4주 이상 임상실습을 수행한 4학년 368명을 대상으로 하였고, 자료수집은 2012년 5월 15일부터 5월 30일까지 실시하였다. 연구의 도구로 일반적 특성, 임상실습관련 특성, 임상실습 만족도 및 임상실습관련 자기효능감으로 구성된 구조화된 설문지를 이용하였다. 임상실습 만족도는 조결자와 강현숙(1984)이 개발하고 이순희, 김숙영과 김정아(2004)가 수정보완한 임상실습 만족도 도구로 측정하였고, 임상실습관련 자기효능감은 Harvey와 McMurray(1994)가 간호대학생을 대상으로 개발한 Nursing Clinical Self-Efficacy Scale(NCSE)를 안은경(2000)이 수정보완한 자기효능감 척도로 측정하였다. 수집된 자료는 SPSS 20.0 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, ANOVA, Scheffé 사후검정, Pearson’s correlation으로 분석하였다. 본 연구의 결과를 요약하면 다음과 같다. 1. 연구대상자의 임상실습 만족도는 5점 만점에 평균 3.230.44이었고, 임상실습 만족도가 가장 높은 문항은 ‘새로운 경험으로서의 임상실습’ (3.910.71)이었으며, 임상실습 만족도가 가장 낮은 문항은 ‘단순하고 기능적인 일만 반복’ (1.990.79)이었다. 2. 연구대상자의 임상실습 만족도는 성별(t=-2.867, p=.006), 전공에 대한 만족도(F=5.578, p<.001), 실습 동료와의 관계(F=5.454, p<.001), 실습 중 수행 비율(F=4.358, p=.005), 임상실습 지도자(F=3.369, p=.035), 임상실습 지도자로부터 지도받는 시간(F=8.687, p<.001), 병동별 실습학생의 수(F=6.258, p=.002)에 따라 유의한 차이를 나타냈다. 3. 연구대상자의 임상실습관련 자기효능감은 문항별 100점 만점에 평균 74.9317.20이었고, 혈당측정 (96.638.43) 문항은 자기효능감이 높았고, 관장하거나 좌약 투여 (56.3524.59), 정맥 내 주사 실시 및 유지 (56.7823.05), 유치도뇨관 설치 및 유지 (57.7023.48) 등 비교적 복잡한 간호행위에 대해 낮게 나타났다. 4. 연구대상자의 임상실습관련 자기효능감은 간호학과 입학동기(t=2.031, p=.043), 전공에 대한 만족도(F=4.767, p<.001), 실습 동료와의 관계(F=5.938, p<.001), 실습 시 어려웠던 관계(F=2.389, p=.038), 실습 중 수행 비율(F=2.987, p=.031)에 따라 유의한 차이를 나타냈다. 5. 연구대상자의 임상실습 만족도와 임상실습관련 자기효능감은 유의한 양적 상관관계(r=.159, p=.002)를 나타냈다. 본 연구 결과에 근거하여 임상실습 만족도와 임상실습관련 자기효능감을 높이기 위해서는 전공에 대한 만족도를 높이고, 실습 동료와의 우호적인 관계를 형성하며, 실습 중 관찰보다는 수행의 비율을 높여야 한다. 또한 실습 동료 뿐 아니라 실습기관의 간호사, 타 의료인과 갈등을 최소화 할 수 있는 대인관계술과 의사소통술에 대한 교육도 이루어져야 한다.;This study was a descriptive study to investigate the satisfaction of clinical practice and self-efficacy of clinical practice targeting the University nursing students. The sample consisted the students currently attending universities of 8 nursing schools, selected through the convenience sampling, and 368 seniors who took the clinical practice courses for 4 weeks or more. The data was collected from May 15 to May 30, 2012. A structured questionnaire composed of the following items were used: general characteristics, satisfaction and self-efficacy of clinical practice. Satisfaction of clinical practice tool developed by Cho and Gang(1984), revised and supplemented by Kim and Kim(2004) was used. When it comes to clinical self-efficacy, the Nursing Clinical Self-Efficacy Scale(NCSE) developed by Harvey & McMurray(1994) revised and supplemented by Ahn(2000) was used. The collected data was analyzed by using the SPSS 20.0 program. The study results were summarized as follows 1. The students' satisfaction of clinical practice was 3.230.44 in average on a scale of 5. The students' satisfaction was the highest for the item ‘Clinical practice as new experience’ 3.910.71, and the lowest for the item ‘Doing simple functional labour continuously’ 1.990.79. 2. The students' satisfaction of clinical practice showed significant differences according to the gender(t=-2.857, p=.006), satisfaction of the major(F=5.578, p<.001), friendly relations with the co-clinical trainees(F=5.454, p<.001), instruction time spent by the clinical instructor(F=8.687, p<.001), ratio of practice performance(F=4.358, p=.005), clinical instructor(F=3.369, p=.035), and the number of students for practice per a ward(F=6.258, p=.002). 3. The mean of students' clinical self-efficacy was 74.9317.20 on a scale of 100. Invasive and complex nursing practices represented the lowest scores. For enema or suppository insertion 56.3524.59, for intravenous injection and its maintenance 56.7823.05, and for inserting urinary catheter and its maintenance 57.7023.48. 4. There were significant differences on the students' clinical self-efficacy according to the satisfaction of major(F=4.767, p<.001), friendly relations with the co-clinical trainees(F=5.938, p<.001), difficulties in personal relations during the clinical practice(F=2.389, p=.038) and motivation of choosing major(t=2.031, p=.043). 5. Satisfaction of clinical practice was positively correlated with clinical self-efficacy(r=.159, p=.002). According the result, to increase the satisfaction of clinical practices and self-efficacy it is needed to boost up the students satisfaction with major, more friendly relations should be built with co-trainees and more performances should be experienced than observation during the clinical practice. Nursing schools should provide the nursing vision and it’s active fields through positive information to increase the students’ satisfaction with major and practical opportunity such as a workshop should be provided to build favorable relationship. It is necessary to train the nurse students how to develop desirable attitudes, and communication skills to minimize any conflicts with nurses and other health care providers .
