어머니가 정동장애인 자녀의 행동문제와 부모의 양육태도

Abstract

부모의 정신병리는 부모의 양육태도를 결정하는 주요 요인으로 자녀의 발달에 좋지 않은 영향을 미치게 되고, 그들의 자녀가 정신병리 및 행동문제를 가질 위험을 증가시킨다. 본 연구에서는 주요 우울증 및 양극성 정동장애 어머니 자녀의 행동문제를 정상대조군 자녀와 비교하고, 정상대조군과 주요 우울증군, 양극성 정동장애군의 부모 양육태도의 차이를 알아보고 이와 행동문제와의 상관관계를 알아보고자 하였다. 정동장애군은 DSM-IV 진단 기준상 주요 우울증 혹은 양극성 정동장애로 통원치료를 받고 있는 여자 환자 중 배우자가 정신병리가 없고, 자녀가 12-18세이며, 배부된 설문지를 완성한 환자를 대상으로 하였고, 정상대조군으로는 정신과적 문제가 없고 같은 연령의 자녀를 둔 어머니를 대상으로 하였다. 세 군의 자녀 모두 정신지체나 기타 다른 정신과적 질환에 이환되지 않았고 자신의 연령에 해당되는 학교에 재학 중이었다. 모든 대상의 일반 인구학적 특성, 정신병리, 부모양육태도를 조사하였다. 자녀의 행동문제는 한국판 아동·청소년 행동평가척도(Child Behavior Checklist : 이하 K-CBCL)와 청소년용 자기보고식 행동평가척도(Youth self Report : 이하 YSR)로 평가하였고, 부모양육태도는 부모자녀 결합형태 조사(Parental Bonding Instrument : 이하 PBI)로 평가하였다. 연구의 결과는 다음과 같다. 1. K-CBCL 결과 행동문제척도 중 신체증상, 불안/우울, 내재화문제에서 세 군간의 유의한 차이를 보였으며(p<0.05), 정상대조군 자녀, 주요 우울증군 자녀, 양극성 정동장애군 자녀 순으로 높은 점수를 보였다. 2. YSR 결과 통계적으로 유의하지는 않지만 주요 우울증군 자녀가 정상대조군 자녀에 비해서 행동문제가 많은 경향을 보였다. 정동장애 내에서는 주요 우울증군 자녀가 양극성 정동장애군 자녀보다 행동문제가 유의하게 많았다(P<.05). 3. 통계적으로 유의하지는 않지만 주요 우울증군, 양극성 정동장애군에서 정상 대조군에 비해 어머니의 돌봄 척도가 낮고, 어머니와 아버지의 과보호 척도가 높은 경향을 보였다. 4. 정동장애군의 행동문제 중 불안/우울, 신체증상, 미성숙, 주의집중문제는 아버지의 과보호 척도와 통계적으로 유의한 상관관계를 보였다.(p<0.01) 이상의 결과를 종합하면, 정동장애 어머니의 자녀가 정상대조군 자녀에 비해 행동문제가 더 많았고, 이들의 행동문제는 아버지의 과보호 척도와 상관관계가 있었다.;This study is investigated to compare psychopathology and their perceived parental rearing pattern of the children of mother with major depression and bipolar disorder and those of normal control group. The correlation was also assessed between behavior problem and perceived parenting style. The subjects were 30 children(age range 12～18) of mothers who met the criteria of DSM-IV for major depressive disorder or bipolar I disorder and 30 children of normal control group. All completed self-report questionnaires containing Child·adolescent Behavior Problem checklist(K-CBCL), Youth Self Report(YSR) and Parental Bonding Instrument(PBI). The results were as follows : 1. The mean score of subscales of somatic complaints, anxious/depressed, internalizing problems in K-CBCL was significantly higher in the control subjects compared with that of the children of mothers with affective disorder(p<0.05). 2. In YSR, there was a tendency of increased behavior problems among the children of mothers with major depression compared with the control subjects. Compared with children of mothers with bipolar disorder, children of mothers with major depression had more behavior problems significantly (p<0.05). 3. Mothers with affective disorder tended to display less caring and fathers and mothers with affective disorder tended to display more overprotective parenting style than the control subjects. 4. There was positive correlation between paternal overprotection and behavior problems in the children of mothers with affective disorder. In summary, the author identified that the children of mothers with affective disorder have more problems in social adjustment than normal control subjects. Parental rearing style is thought to have more negative influence on the children of mothers with affective disorder than normal control. It is guessed that less caring and more overprotective rearing style could have certain influence on behavior problems of their children.
