조산원과 가정에서 분만한 여성의 순산(順産) 경험

Abstract

출산은 여성의 일생에 가장 중요한 순간으로 여성은 출산을 통해 여성과 어머니로서 새로운 전환점을 맞이한다. 본 연구는 여성의 순산경험의 의미를 탐구하고자 질적 연구방법 중 van Kaam의 현상학적 방법으로 시도하였다. 연구를 위해 2011년 1월부터 7월까지 조산원과 가정에서 분만한 여성 12명을 심층 면담하였다. 자료 분석을 통해 403개의 의미 있는 진술을 172개의 요소, 48개의 하부범주, 19개의 범주로 유목화 하였고, 19개의 범주에서 8개의 주제를 도출하였다. 8개의 주제는 “병원분만과 조산원 분만 결정에 갈등함”, “순산에 대한 확신과 조산사 신뢰로 자연분만을 선택함”, “조산사와 가족이 한마음이 되어 힘이 남”, “자연스러운 분만진행을 몸으로 느낌”, “힘든 진통과정을 편안하게 출산함”, “가슴 벅찬 출산감격”, “순산으로 만족스러움”, “가족 간의 애정이 깊어짐”으로 나타났다. 8개의 주제를 종합하여 여성의 순산경험의 의미를 다음과 같이 진술하였다. “여성은 병원분만과 조산원 분만 사이에서 갈등하였으나, 잘 낳을 수 있을 것이라는 자기 자신에 대한 확신과 조산사에 대한 신뢰로 조산원에서 자연 분만할 것을 선택하였다. 여성은 출산과정에서 조산사와 가족 및 태아가 함께하여 힘이 났으며, 아기가 나오는 것을 몸으로 느끼며 힘든 진통과정을 조산사의 세심한 돌봄과 배려를 통해 편안하게 출산하였다. 순산으로 만족스럽고 가슴 벅찬 출산감격을 경험했으며, 아기와의 행복한 만남을 통해 가족 간의 애정이 깊어졌다” 본 연구를 통하여 병원보다 자연스러운 환경에서 조산사의 도움으로 스스로 출산하는 여성의 순산경험을 탐구함으로써 출산여성이 무엇을 중요시하고 무엇을 원하는 가를 이해할 수 있었다. 가족이나 친지들의 반대에도 불구하고 자신과 태아의 출산능력에 대한 확신과 조산사에 대한 신뢰와 믿음으로 순산한 후 느끼게 되는 만족감과 행복감은 스스로에게 자신감과 힘을 북돋우어 주는 계기가 되었다. 또한 분만과정 뿐만 아니라 임신- 분만- 산욕, 신생아와의 관계 등 출산 전 과정과 가족과의 상호작용까지도 관찰하고 경험을 나눈 것도 의미가 있다. 본 연구는 대부분의 여성들이 선택의 여지없이 병원에서 수동적으로 출산하는 것에 비해 조산사에 의해 분만한 여성의 경험을 통해 여성들이 병원 외에 출산장소를 선택하는데 중요한 정보가 될 것이다. 또한 조산사에게는 여성의 입장을 고려하는 조산원의 시설과 조산업무의 전문성을 향상시킬 수 있는 자료가 될 것으로 기대한다.;A baby gives a woman the most important moment and turning point as a woman and a mother. This study used van Kaam's phenomenological method as the qualitative data analysis to inquiry for the meaning of woman's experience of spontaneous delivery. For this study, in-depth interviews were carried out from January to July of 2011, with twelve women who had delivery at midwifery clinics and homes. Through the data analysis, 403 significant statements, 172 elements, 48 subcategories, and 19 categories were extracted, and from the 19 categories, 8 themes were drawn. The eight themes were: “Conflict on whether a hospital or a midwifery clinic”, “Choosing natural delivery on the assurance of ability in spontaneous delivery of herself and on the trust in the midwives.” “Being encouraged by a midwife and family members with one accord”, “Experience of the spontaneous delivery process on body”, “Comfortable delivery in spite of painful process”, “Deeply impressed by the overwhelming joy of birth”, “Satisfaction of spontaneous delivery”, and "Deeper love among the family members". When the eight themes are synthesized together, meaning of women's experience of Spontaneous Delivery can be stated as: “A woman had a conflict on whether a hospital or midwifery clinic, but she chose natural delivery because she was assured that she would have spontaneous delivery and also because she trusted the midwives. She was encouraged during the labour for she was with her fetus and also for midwife and her family worked together in one accord. She had easy and comfortable delivery by the delicate and considerate care of the midwife even though it was painful labour and she also felt the baby coming out of her. She had very impressing and satisfying experience by the spontaneous delivery, and she came to have deeper love with her family by the happy meeting with the baby” This study helped understanding what women who had delivery want and what is most important to them by researching those who deliver by themselves with the help of midwives at natural environment rather than in hospital. The satisfaction and happiness after the spontaneous delivery gave her confidence and encouragement when she chose it on the assurance of delivery ability of herself and her fetus, and on the trust in the midwives in spite of the opposition of her family or relatives. It was also very meaningful to have researched and looked into not only the delivery process but also the interaction with the family and the whole process of delivery such as pregnancy, delivery, puerperium, and relationship with the new born baby. The present study provides very important information and data for women on choosing the delivery place other than just hospitals by the experience of delivery with the midwives while most women choose the hospital helplessly without option. And also this study is expected to provides some information and data for the midwives to improve their facilities concerning women and level of expertise.
