Abstract
배경: 샤르코-마리-투스 (CMT)병은 유전말초신경병의 대표적인 질환이다. 최근 전체 엑솜 염기서열분석 (whole-exome sequencing)은 유전병의 원인 유전자를 발굴하는 새롭고 강력한 진단방법으로 제시되고 있다. 오늘날까지 Berardinelli-Seip congenital lipodystrophy 2(BSCL2) 유전자의 이종접합변이는 두 가지 종류(N88S, S90L)가 보고되고 있다. Seipin의 N-당화과정(N-glycosylation) 에서의 N88S 및 S90L 이형 돌연 변이는 신경계 퇴행성 질환을 유발하는 것으로 알려져 있다. BSCL2 유전자의 돌연변이는 (N88S, S90L) 실버증후군/경직하반신마비, 제5형 유전말초운동신경병, 제 2형 샤르코 마리 투스를 유발하는 것으로 알려져 왔다. 방법: 유전말초신경병을 지닌 129명을 대상으로 전체 엑솜 염기서열분석을 시행하였다. BSCL2유전자 변이를 지닌 총 20명 환자 (남자13명, 여자 7명)를 대상으로 신경학적 진찰 및 신경전도검사를 시행하였다. 이와 함께 유전자 변이를 지닌3명의 환자를 대상으로 장딴지신경 병리조직 검사를 시행하였다. 결과: 본 연구의 결과 BSCL2 유전자에서 N88S 및 S90W 이형돌연변이를 규명하였다. N88S는 2가족 (FC-51, FC-504) S90W는 1가족(FC-305) 이 발견되었다. 이중S90W돌연변이는 이전에 보고되지 않은 새로운 돌연변이로 확인되었다. 이는 정상인 300명에서 관찰되지 않았으며 가족내에서도 일치하는 양상을 보였다. S90W 돌연변이를 지닌 한 가족은 82%에서 경직보행(spastic gait)을 보였지만 이와 대조적으로 N88S 돌연변이가 발견된 가족은 발쳐짐 보행(steppage gait)이 많이 관찰되었다. S90W 환자에서는 특징적으로 근위축 및 위약은 우측의 엄지두덩근(thenar muscle)에서 시작되었으며 비대칭적으로 우측에서 심한 소견이 관찰되었다. N88S와 S90W 돌연변이 모두에서 같은 가족 내에서도 다양한 임상증상을 보였다. 감각증상이 있는 환자와 감각증상이 없는 환자 모두에서 장딴지신경 조직검사 상 축삭신경병증 소견이 관찰되었다. 축삭의 손상의 정도는 N88S 돌연변이 환자에서 S90W 돌연변이 환자에 비하여 심하게 관찰되었고 임상증상 또한 N88S 돌연변이 가족에서 S90W 가족에 비해 심하게 관찰되었다. 결론: 본 연구에서 저자들은 전체 엑솜 염기분석을 통하여 한국에서 최초로 BSCL2 유전자의 N88S 및 S90W 돌연변이를 발견하였다. 특히 S90W 돌연변이는 저자들이 알기로는 이제까지 보고된 바가 없었으며 본 연구는 S90W 돌연변이를 지닌 환자들의 임상양상 및 병리조직 연구를 최초로 시행하였다는데 의의가 있겠다.;Background : Charcot-Marie-Tooth (CMT) disease comprises a large number of genetically distinct forms of inherited peripheral neuropathies. Whole-exome sequencing has recently been introduced as a novel and an alternative approach. To date, only two different heterogeneous mutations in Berardinelli-Seip congenital lipodystrophy 2 (BSCL2) have been reported (N88S and S90L). The heterogenous mutations N88S and S90L, in the N-glycosylation site of seipin, are associated with the neurodegenerative diseases. Toxic functional mutation (N88S and S90L) in the BSCL2 gene have been found in Silver syndrome/spastic paraplegia, distal hereditary motor neuropathy type V (dHMN-V) and Charcot-Marie-Tooth disease type 2 (CMT2). Methods : We performed the whole-exome sequencing in 120 Korean families with CMT2/dHMN-V and Silver syndrome. We identified two causative heterozygous mutations in BSCL2 gene (S90W and N88S) in three unrelated families. In these families, we performed neurological exams and nerve conduction studies (NCS). In addition, in performing a sural nerve biopsy, we compared pathology and phenotype correlation in BSCL2 mutation. Result : We described 20 patients from three unrelated hereditary peripheral neuropathy families. Through whole exome sequencing, we detected a N88S mutation and a novel S90W mutation in the BSCL2 gene, after filtering 300 Korean control exomes. In S90W mutation patients, muscle weakness and atrophy started and predominated in the right thenar muscles. Comparing to this, the two groups with N88S mutation had showed the most steppage gait. In both mutations, intrafamilial phenotype variability existed. Sural nerve biopsies done in those patients with or without sensory impairment and sural sensory nerve conduction changes revealed similar findings suggestive of axonal neuropathy with subtle differences in degree of changes. Axonal loss was more severe in the N88S mutation. Further, clinically neuropathic symptoms were more severe in the N88S mutation family than that of the S90W mutation family. These pathological findings were suggestive of more severe axonal injury in N88S mutation and these pathological findings were correlated with clinical symptoms. Conclusion : In this study, we identified N88S and a novel S90W BSCL2 mutation by whole exome sequencing. This report expands the variability of clinical spectrums associated with the BSCL2 gene and describes the first family with S90W mutation. And this study is the first study that reports extensive and detailed histopathological features of sensory nerve injury in BSCL2 mutation.
