Abstract
연구배경: 요독물질의 하나인 indoxyl sulfate 자체가 신기능을 악화시킬 수 있다고 알려져 있다. 만성콩팥병 환자에서 혈중 indoxyl sulfate의 농도가 증가해 있을 뿐 아니라, 저단백식이나 AST-120 같은 indoxyl sulfate의 농도를 저하시키는 치료는 투석시기를 미루는 효과를 얻을 수 있다. 상피-중간엽 세포이행과 세포사멸은 신장질환의 발생이나 진행에 중요한 역할을 담당하고 있는데, indoxyl sulfate 가 이 두 가지 기전에 미치는 영향은 잘 알려져 있지 않다. 실험재료 및 방법: NRK-52E 세포 (백서 신장 근위 상피세포주)와 HK-2 세포 (인간 신장 근위 상피세포주)를 이용하여 Indoxyl sulfate의 세포증식에 대한 영향을 확인하기 위하여 세포수를 세어 농도 (6.25 - 125 μg/ml)에 따른 변화를 비교하였고, 세포독성을 확인하기 위해 젖산 탈수소효소 (lactate dehydrogenase, LDH) 활성도를 측정하였다. indoxyl sulfate를 처치하여 세포의 형태학적 변화를 광학현미경으로 분석하였다. Indoxyl sulfate (6.25 - 125 μg/ml)에 의한 E-cadherin과 α-smooth muscle action (이하 α-SMA)의 단백 발현과 MAPK 활성화를 관찰하였다. MAPKinase 억제제인 PD98056와 SB203580와 indoxyl sulfate의 세포내 유입을 억제하는 organic anion transporter 차단제인 probenecid의 전처치가 상피-중간엽 세포이행과 세포사멸에 미치는 영향을 관찰하였다. 세포사멸의 양적 측정은 annexin V 염색을 통해 유세포 분석기를 이용하여 측정하였다. 결과: Indoxyl sulfate는 신장 상피세포의 증식을 6.25 μg/ml의 농도부터 농도 의존적으로 유의하게 억제시켰고, 125 μg/ml의 농도부터 LDH 분비를 증가시켰다. Indoxyl sulfate에 노출된 NRK-52E 및 HK-2 세포는 대조군과 비교하여 세포의 형태가 입방형에서 원추형으로 바뀌면서 세포-세포 결합이 떨어지는 양상을 보였다. Indoxyl sulfate를 자극 48시간부터 NRK-52E 세포에서 α-SMA 단백발현은 증가하고, E-cadherin 단백 발현은 감소하였다. Indoxyl sulfate에 의한 E-cadherin 및 α-SMA 발현의 변화는 indoxyl sulfate가 세포내부로 유입되는 것을 차단시키는 probenecid에 의해 유의하게 억제되었다. NRK-52E 세포에서 indoxyl sulfate 자극은 5분 후부터 ERK1/2 및 p38의 인산화를 유도하였다. Indoxyl sulfate는 25 μg/ml의 농도부터 세포사멸을 유의하게 유발하였다. 하지만 ERK1/2 및 p38의 억제제 전처치는 indoxyl sulfate에 의한 E-cadherin 및 α-SMA 발현에 유의한 변화를 초래하지 않았으나 indoxyl sulfate 에 의한 세포사멸은 유의하게 억제하였다. 결론: 신장 상피세포의 EMT 와 세포사멸은 요독물질인 indoxyl sulfate에 의한 신장 섬유화의 기전 중의 하나라고 생각되며 MAPKinase 활성화는 indoxyl sulfate 에 의한 세포사멸의 중요한 역할을 담당하나, 상피-중간엽 세포이행의 주요기전은 아닐 것으로 사료된다.;Background: Indoxyl sulfate, one of the uremic toxins, is regarded as an important substance in the progression of chronic kidney disease (CKD). The level of indoxyl sulfate is elevated in CKD, and indoxyl sulfate lowering therapy is known to improve renal function and delay the initiation of dialysis. Epithelial-to-mesenchymal transition (EMT) and apoptosis of renal tubular cells are known to play a critical role in the development and aggravation of CKD. However, the effects of indoxyl sulfate on EMT and apoptosis have not been determined. Methods: The effects of indoxyl sulfate on cell proliferation and cytotoxicity were assessed by direct cell counting and LDH assay respectively, in NRK-52E cells. EMT was evaluated by morphologic transformation of NRK-52E and HK-2 cells under inverted microscopy as well as a quantitative analysis of the expression of the epithelial marker, E-cadherin and the mesenchymal marker, α-smooth muscle actin (α-SMA) protein, by western blot analysis. Cell apoptosis was assessed by flow cytometric analysis after annexin-V and PI double staining. ERK1/2 and p38 MAPKinase activation in indoxyl sulfate-stimulated cells were examined by western blotting. The effects of pre-treatment with PD98059 and SB203580 (MAPKinase inhibitors) and probenecid (an organic anion transporter inhibitor) on indoxyl sulfate-induced EMT or apoptosis were also determined. Results: Indoxyl sulfate significantly inhibited cell proliferation in a dose-dependent manner from a concentration of 6.25 μg/ml. Indoxyl sulfate induced a morphological transformation from cuboidal epithelial cells to spindle shaped scattered fibroblast-like cells in HK-2 cells and NRK-52E cells. Indoxyl sulfate down-regulated E-cadherin and up-regulated α-SMA expression at 48 hours, which were (both) blocked by a pre-treatment of the organic anion transporter, probenecid. Indoxyl sulfate also induced apoptotic cell death in NRK-52E cells from a concentration of 25 μg/ml. Pretreatment of ERK1/2 or p38 MAPKinase inhibitors, PD98059 or SB203580 resulted in no protective effect on indoxyl sulfate-induced EMT, whereas ameliorated indoxyl sulfate-induced apoptosis of NRK-52E cells. Conclusion: The result of the present study suggested the phenotypic transformation and apoptosis as a novel mechanism of indoxyl sulfate-induced renal damage. Although indoxyl sulfate activated ERK1/2 and p38 MAPKinase in renal tubular cells, it was not responsible for the indoxyl sulfate-induced EMT, but was related to apoptosis of renal tubular cells.
