Abstract
Anemia plays an important role in the development of cardiovascular structural and functional alterations. There is little data on the effect of low hemoglobin level on the left ventricle (LV) in patients without heart disease. The objective of this study was to document changes in echocardiographic variables of LV structure and function after the correction of anemia. A total of 34 iron deficiency anemia patients (34.94 ± 10.67 years old, 31 females) without traditional cardiovascular risk factors or cardiovascular disease and 34 age- and sex-matched controls were studied. Assessment included history, physical examination, and echocardiography. Of the 34 patients with anemia enrolled, 20 were followed up and underwent echocardiography after correction of anemia with oral iron supplements. There were significant differences between the anemia and control groups in the LV end-diastolic diameter (51.76 ± 3.91 vs. 47.18 ± 3.20 mm), LV end-systolic diameter (31.47 ± 3.67 vs. 27.82 ± 2.90 mm), posterior wall thickness (7.96 ± 1.31 vs. 7.34 ± 1.17 mm), LV mass index (97.03 ± 24.67 vs. 74.25 ± 15.22 g/m²), left atrial (LA) diameter (37.27 ± 5.47 vs. 33.56 ± 3.84 mm), LA volume index (28.09 ± 9.09 vs. 18.26 ± 4.48 ml/m²), peak mitral early diastolic (E) velocity (1.04 ± 0.22 vs. 0.84 ± 0.17 m/s), peak mitral late diastolic (A) velocity (0.68 ± 0.20 vs. 0.59 ± 0.11 m/s), E/A ratio (1.61 ± 0.39 vs. 1.42 ± 0.26), the ratio of mitral to mitral annular early diastolic velocity (E/E') (8.06 ± 2.11 vs. 6.70 ± 1.33), stroke volume (76.10 ± 14.82 vs. 67.92 ± 12.71) and cardiac index (3.55 ± 0.84 vs. 3.03 ± 0.64). In twenty patients who underwent follow-up echocardiography after treatment of anemia, mean hemoglobin was significantly increased (12.49 ± 1.39 g/dl vs. 5.80 ± 1.29 g/dl). The follow-up results displayed significant decreases in the LV end-diastolic diameter (51.75 ± 3.96 vs. 49.35±3.68 mm), LV end-systolic diameter (31.55 ± 2.80 vs. 29.30 ± 2.96 mm), LV mass index (100.61 ± 29.28 vs. 83.95 ± 22.76 g/m²), LA diameter (38.05 ± 6.58 vs. 35.25 ± 4.89 mm), and LA volume index (30.63 ± 9.91 vs. 22.54 ± 6.39 ml/m²) compared to baseline levels. In the Doppler echocardiographic studies, the E velocity (1.05 ± 0.24 vs. 0.84 ± 0.17 m/s), E/A ratio (1.69 ± 0.44 vs. 1.48 ± 0.50), E/E' (8.52 ± 2.30 vs. 7.27 ± 1.35), stroke volume (75.35 ± 15.35vs. 69.74 ± 12.63) and cardiac index (3.42 ± 0.90 vs. 1.80 ± 0.31) were also decreased. In conclusion, appropriate correction of low hemoglobin levels improved ventricular diastolic function and reduced LV cavity size and muscle mass even in the patients without cardiovascular risk factors or cardiovascular disease.;목적: 빈혈은 심혈관계의 구조와 기능적 변화에 중요한 역할을 하는 인자이다. 그러나 심질환이 없이 빈혈만 있는 환자에서 낮은 혈색소 수치가 심장의 좌심실의 기능과 구조에 미치는 영향에 대해서는 아직 잘 알려진 바가 없다. 본 연구는 심혈관질환에 대한 위험 인자 및 기존의 심질환이 없는 빈혈 환자를 대상으로 하여 혈색소 수치를 교정하였을 경우에 초래되는 좌심실의 구조 및 기능의 변화를 심초음파 검사를 통하여 관찰하고자 시행되었다. 연구대상 및 방법: 심혈관질환의 위험인자가 없고 심질환의 기왕력이 없는 34명의 철 결핍성 빈혈환자군과 이들과 성별 및 나이가 같은 34명의 정상 대조군이 연구에 포함되었다. 과거력, 신체검사 및 심장 초음파를 시행하였다. 처음 연구에 포함된 34명의 빈혈환자 중 20명은 빈혈 교정후 심초음파 추적 관찰을 시행하였고, 이들은 그 결과값을 빈혈 교정 전의 값과 비교하였다. 결과: 빈혈군은 대조군에 비하여 좌심실 확장기말 직경, 좌심실 수축기말 직경, 후벽 두께, 좌심실 질량 지수, 좌심방 용적 지수, E 속도, A 속도, E/A 비율, E/E' 비율, 일회 박출량 및 심박출 계수가 증가되어 있었다. 빈혈 교정 후 심초음파를 다시 시행한 20명의 환자에서 교정 후에 측정한 수치들은, 좌심실 확장기말 직경, 좌심실 수축기말 직경, 좌심실 질량 지수, 좌심방 용적 지수, E 속도, E/A 비율, E/E' 비율, 일회 박출량 및 심박출 계수가 빈혈 교정 전에 비하여 감소하였다. 결론: 낮은 혈색소 수치의 교정은 좌심실의 크기와 질량을 감소시키며, 좌심실 이완기능의 호전을 유도할 수 있다.
