Abstract
PURPOSE: The purpose of this study was to evaluate the diagnostic value of multidetector computed tomography (MDCT) to determine the fracture stage, using magnetic resonance imaging (MRI) as a standard of reference for the diagnosis of acute vertebral fractures. MATERIALS AND METHODS: From January 2005 to February 2007, 160 vertebrae in 118 patients (range, 15-87 years; mean age, 58 years) who were suspected of having vertebral fractures and underwent both MDCT and MRI were enrolled in the study. MRI consisted of axial and sagittal T1-weighted, T2-weighted and fat-suppressed T2-weighted images. Our protocol for MDCT included both axial and sagittal images. Acute fractures were defined as fractured vertebrae with fracture lines as well as bone marrow edema on MRI. Two musculoskeletal radiologists retrospectively reviewed the MDCT findings such as fracture lines, the change of marrow density and paravertebral fluid collection to determine the stage of fractures using consensus. The sensitivity, specificity and accuracy of MDCT in determining the fracture stage were analyzed. RESULTS: Acute fractures were 131 (82%) cases and old fractures were 29 (18%) cases among the 160 vertebrae. MDCT findings of 131 acute vertebral fractures were fracture lines in 96 (73%) vertebrae on axial images and 119 (91%) vertebrae on axial plus sagittal images. Ancillary findings of 131 acute vertebral fractures were the change of marrow density in 106 (81%) vertebrae, and paravertebral fluid collection in 115 (88%) vertebrae. If any one abnormal finding was counted, the overall diagnostic values were as follows: sensitivity 97%, specificity 86%, and accuracy 92%. CONCLUSION: MDCT findings for determining the stage of vertebral fractures are the change of marrow density and paravertebral fluid collection as well as fracture lines. Therefore, MDCT is an useful alternative diagnostic tool in determining the stage of fractures.;목적: 척추 골절의 시기를 판단하는데 MRI를 급성 척추 골절의 표준 검사로 이용하여 MDCT의 진단적 가치를 평가하고자 한다. 대상과 방법: 2005년 1월부터 2007년 2월까지 척추 골절이 의심되어 MDCT와 MRI를 모두 촬영한 환자 118명 (평균 58세, 연령 범위 15-87세) 의 160개 척추 분절을 대상으로 하였다. MRI는 축상 및 시상 T1강조영상, T2 강조영상, 지방억제T2 강조영상을 얻었으며, MDCT는 축상 및 시상 영상을 얻었다. 급성 골절은 MRI에서 골절선과 골수 부종이 있는 경우로 정의하였다. 두 명의 영상의학과 의사가 합의하에 후향적으로 골절선, 골수의 밀도 변화, 척추 주변 액체 유무의MDCT소견을 평가하였다. 급성 척추 골절을 평가하는데 MDCT의 민감도, 특이도, 정확도를 알아보았다. 결과: MRI에서 급성 골절 131 (82%) 예, 만성 골절 29 (18%) 예가진단되었다. MDCT에서 급성 골절 131개 척추 분절 중에 골절선이 보인 경우는 축상 영상에서 96 (73%) 예, 축상 및 시상 영상에서 119 (91%) 예이었다. 보조적인 골절 소견은 골수의 밀도 변화 106 (81%) 예, 척추 주변 액체 직접 115 (88%) 예이었다. MDCT로 척추 골절의 급성 시기를 평가하는데 골절선, 골수의 밀도 변화, 척추 주변 액체의 직접의 소견이 적어도 하나 이상이 보이는 것을 진단 기준으로 하면 민감도, 특이도, 정확도는 각각 97%, 86%, 92%이다. 결론: 골절선과 더불어, 골수의 밀도 변화, 척추주변 액체 직접은 MDCT에서 급성 척추 골절을 평가하는 중요한 소견이다. 따라서 MDCT는 척추 골절의 시기를결정하는데 대안이 되는 유용한 검사방법이다.
