d-Tubocurarine 의 succinylcholine 으로 인한 안압 상승억제 효과

Abstract

The effect of prior administration of d-tubocurarine on the increased intraocular pressure associated with the use of succinylcholine was studied in 30 randomely selected human subjects, who did not have cardiopulmonary and ocular diseases preoperatively. Intraocular pressures were measured with Schiotz tonometer. The major findings of this study were as follows: 1. When succinylcholine was given along, a rise from 15.5mmHg to 18.0mmHg(16.l%) were seen. (p<0.05) 2. When give d-tubocurarine, 3㎎, three minutes prior to the administration of succinylcholine, 15 healthy human subjects had no significant increase in intraocular pressure. (p>0.05) 3. Intraocular pressure were significantly increased in both groups after endotracheal intubation, but d-tubocurarine precurarization group was minimal than succinylcholine alone group. (p<0.05) The intraocular pressure was inhibited in the d-tubocurarine precurarization group. This simple, convenient method prevents the increased intraocular pressure associated with the use of succinylcholine.;전신마취 유도시에 기관내삽관을 위하여 탈분극성 근육이완제인 succinylcholine의 투여는 불가피한 현실이다. 탈분극성 근육이완제인 succinylcholine은 내안근 섬유속연축 및 외안근 섬유속연축등의 원인에 의하여 일시적인 안압의 상승을 초래한다. 본 시험은 전신상태가 양호하고 안질환이 없는 환자 30명을 무작위 선택하여 비탈분극성 근육이완제인 d-tubocurarine 3㎎으로 전처치한후 탈분극성 근육이완제인 succinylcholine에 의한 안압상승을 예방할 목적으로 시험에 착수하였다. 안압은 Schiotz Tonometer를 사용하여 측정하였다. 이 연구에서 얻은 결과는 succinylicholine단독투여군에서는 안압이 대조치에 비해 16.1% 상승하였으며, succinylcholine투여 3분전 3㎎의 d-tubocurarine을 투여한 15명의 환자에서는 대조치와 큰차이가 없었다. 또한 기관내삽관후 안압은 두개군에서 모두 상당히 상승되었으나 d-tubocurarine으로 전처치한 군에서는 succinylcholine 단독투여군에 비해 상승률이 낮았다. 이상의 연구 결과는 안구질환이나 안압상승으로 인해 위험을 초래할 환자의 전신마취 실시에 있어서 비탈분극성 근육이완제인 d-tubocurarine 3㎎을 전처치 함으로써 succinylcholine투여에 의한 안압상승을 예방할 수 있음을 알 수 있었다.
