醫療保險의 組織管理와 加入者의 態度에 關한 硏究

Abstract

1970年代 福祉國家를 達成하는 것을 目標로 하는 國家에서 社會福祉는 重要한 問題로 대두되었고 그 中에서도 保健行政 특히 醫療保險은 社會福祉 實現의 아주 重票한 分野로 登場하였다. 어떤 社會나 集團의 構成員을 健康하게 維持한다는 것은 重要한 일이며 모든 國民들에게 良質의 醫療를 베풀 수 있는 醫療事業은 要請되는 일이다. 따라서 韓國에서도 1977年부터 시작되는 第4次 5個年 經濟開發計劃에는 醫療保險을 包含한 社會福祉政策에 우선권을 두기 시작하고 있다. 現在 韓國의 醫療保險은 勤勞者를 對象으로 하는 被傭者 組合, 自當者(勤勞者, 公務員, 또는 軍人을 除外한 一定한 所得을 가지는 國民) 組合, 그리고 大學敎 在學生을 對象으로 하는 大學學生 醫療保險 등이 있다. 따라서 本 硏究에서는 大學學生 醫療保險을 實施하고 있는 梨花女大를 選定하여 醫療保險 管理機構인 大學保健所의 內部組織管理와 保險加入者인 學生들의 大學保健所 및 醫療保險에 對한 態度를 調査하여 大學保健所의 效率的인 方向을 提示하고 醫療保險制度가 지니고 있는 問題點과 그 解決方案을 提示하고자 하였다. 梨花女大 醫療保險은 1969年에 시작되었으며 梨花女大에 在籍한 學生(大學院生 包含)은 登錄을 필함으로써 被保險者가 된다. 保險料는 한 학기당 700원으로 登錄金 納付時 함께 支拂하고 있어 保險料 징수율은 100%이다. 그리고 本 保險의 給與費範圍는 疾病, 負傷의 治療에 따르는 수수료, 처치료, 검사료, 약가 및 입원료이며 通院治療인 경우 5,000원, 入院治療인 경우 42,000원이 최고 상한선이며 病院 감액율은 80%이다. 이밖에 指定醫療機關으로는 梨花女大 附屬病院과 保健所內의 保險診療室이 있다. 또한 本 硏究에서는 大學保健所 職員 8名과(그中 7名이 調査에 응함), 學生 8,500여명을 對象으로 그 中 無作爲抽出된 852名에 대한를 實施하였다. 調査結果 나타난 問題點들을 票約하면 다음과 같다. 먼저 大學保健所의 組織管理에서 나타난 問題點을 살펴보면 1. 大學保健所內에 醫療保險에 대한 홍보활동을 위한 체계가 있지 않다. 保健所自體內의 적극적인 홍보활동으로 보다 많은 學生들이 醫療保險을 利用할 것이며 따라서 그 效率性을 높일 수 있다. 2. 大學保健所의 職員들에게 保健專門敎育을 위한 訓練이 實施되고 있지 않다. 職員이 組織의 效率的인 成員이 되기 위해서는 訓練은 必須的인 것이며, 급격한 社會의 變化는 保健知識에서의 變化를 가져온다. 그러므로 새로운 知識과 技能에 대한 組織의 要請으로 職員들에게 訓練의 기회가 주어져야 한다. 3. 職員들의 職務에 대한 熱誠度에 비해 그들에게 士氣를 높여주는 要因이 적다. 職員들이 그들이 지니고 있는 能力을 最高度로 發揮하기 위해서는 그 能力을 誘因할 수 있는 要因이 있어야 한다. 즉 基本的 欲求의 充足이라든가 昇進의 機會 기대를 充足시켜야 한다. 그런데 大學保健所는 그런 要因이 많이 나타나고 있지 않으며 職員들은 그에 대해 부정적인 反應을 보이고 있다. 4. 保險給與費의 額數가 充分치 못하다. 맹장 수술을 받아도 10만원이 넘는 現醫療酬價에 비추어 保險給與額은 充分한 額數가 되지 못하고 있다. 管理運營費를 감안하고 醫療酬價의 引上率과 保險給與額 支給率이 相關關係를 이룰 수 있도록 檢討되어야 할 것이다. 그 다음 學生들의 態度에서 나타난 問題點을 要約하면 다음과 같다. 무엇보다 먼저 學生들의 醫療保險 利用率과 大學保健所와 梨大 附屬病院의 利用率이 낮음을 들 수 있으며 調査에서 나타난 問題點들은 이것들과 聯關이 되어 있다. 利用率이 적은 理由를 보인 學生들은 평소에 醫療保險을 담당하고 있는 大學保健所에 대하여 좋은 인상을 갖고 있지 못하며 指定醫療機關인 梨大 附屬病院의 治療와 行政事務에 대해서도 만족스런 느낌을 갖고 있지 못하기 때문이다. 그외에 醫療保險에 대한 具體的인 知識不足을 들 수 있다. 이와 같은 두 가지 調査에서 나타난 것을 綜合하여 볼 때 醫療保險과 大學保健所에 대한 보다 效率的인 方向을 爲하여 保健所內部 組織面에서의 繼續的인 要員의 訓練, 士氣의 充足, 外部에의 홍보활동, 指定醫療機關의 治療의 向上과 行政事務의 改善등이 要求되며 學校당국의 醫療保險 및 保健所 管理에 대한 充分한 財政的 후원, 保險給與費의 인상 등이 要請된다.;Social welfare might be one of the most significant issues for the attainment of a welfare state in 1970's. In special health administration and medical insurance may play an important role for the social welfare. In a society or a group keeping the members healthy is no less important than any other and a medical care undertaking that can bestow favors on the whole people is necessarily required. The 4th five-year economic development plan of Korea to begin in 1977 puts a priority on social welfare policy involving medical insurance. Korea has presently such medical insurance systems as employee's insurance, independent enterprise insurance(for those who have considerable income excluding employees, governmental officials and solidiers) and collegians' insurance. This study aims at analysing in Ewha Womans University presently practising studentsr medical insurance. Analyzing in organitational management of University Health Cent and examining insured students' attitudes toward University Health Cent and medical insurance itself, suggesting an effective way of the center and problems deriven from the medical insurance, and then showing solutions for the suggested problems. Medical insurance in Ewha Womans University began in 1969 and all the registered students containing graduate students are the insured. The insurance rates are \700 each semester. The rates are to be paid together with semester registration fee. Thus insurance rates are paid by 100%. This medical insurance covers medical charges for diseases and ailments, medical treatment, medical test, drugs and charges fir hospital accommodation. In case of commutation treatment, less than \5,000 is indemnified; in case of hospitalization, less than \42,000. In both cases hospital reduction rates are 80%. Beside these, there are such designated medical institu. tions as Ewha Womans University Hospital and Campus Insurance Clinic in the University Health Center. An examination was conducted through 7 University Health Center staff and 765 students in Ewha Womans University. The resultant findings are as follows : A. For the University Health Center 1. The center has no system to give appropriate informat ons on medical insurance. More positive activities to supply students with detailed informations may be helpful to the students, then the effectiveness would be expanded. 2. The staff has hardly received training. A more effective training may not be left ignored for a member of the staff to effective and to meet the requirements of ever-changing societies because a variety of knowledge on medicine is necessitated. Thus it is recommendable to give opportunities to get training to obtain the relative new knowledge and'informations. 3. In comparison with their entheusiasm, there is no measures to boost the morale of,,the staff. In order to show their ability fully, some measures should be made; for example the fulfilment of their basic needs or giving full opportunities for promotion. But in this center such factors are hardly seen. To this the staff shows a negative attitude. 4. Full insurance compensation should be indemnified. In light if medical charges of \100,000 for a appendectomy, the insurance indemnity is scanty. It is desired that the equilibrium between the increasing medical charges and insurance indemnity should be maintained. B. For students' response Almost all the students have low rate of utilizing medical insurance, University Health Center, Ewha Womans University Hospital. The suggested problems in this study are closely related with these. The reasons of the low rate might be deriven from that students have no good impression on University Health Center taking charge of medical insurance, and have no satisfaction in medical care or administration in Ewha Womans University Hospital. In addition to it, they have little knowledge of medical insurance. From the standpoints of these series of findings, it is recommendable that continuous training for the staff of the center, right timing, information-supplying activities, improvement if medical care and administration in the designated medical institute should be forwarded. And university's fully supporting medical insurance and the administration of the center, the increment of insurance indemnity are desirable. The university medical insurance should not be confined to student, but should be a model case for the whole people's medical care.
