초음파를 이용한 한국성인의 비장계측에 관한 연구

Abstract

Ultrasound has been found to be accurate reliable and noninvasive method in the measurement of spleen. Also ultrasound offers the rapid and simple method of estimating volume of spleen in vivo against the computed tomography. The study was undertaken to obtain standard values of size in three dimensions and normal range of splenic volume by the use of splenic volumetric index (SV1) in normal korean adults. The author analyzed 100 cases of abdominal ultrasonography of normal korean adults. The results were as follows: 1. The average size of spleen in adult male was 6.85±1.31cm in breadth. 4.93±1.27cm in thickness,6.33±1.46cm in height : in adult females, 6.61±1.23cm ,5.17±1.26cm,6.33±1.42cm, respectively : total average, 6.73±1.27cm, 5 05±1.27cm, 6.33±1.39cm, respectively. 2. The average splenic volumetric index in adult male was 8.20±3.95 : in adult females, 8.41±4.08: total average, 8 3114.00 : upper limit, 20.65 : lower limit, 2.24. 3. There were no statistical differences of SV1 and size between sex and age.;비장의 크기를 측정하는 것은 간질환과 혈액학적 질환을 포함한 다양한 질병의 진단과 치료판정에 매우 유용하다. 비장의 크기를 측정하는 방법에는 방사선학적으로 단순 복부 X-선 찰영, 방사선 동위원소 주사법, 혈관 조영술, 초음파 촬영 그리고 전산화 단층 촬영술 및 자기공명 영상술등이 있다. 이들중 초음파 촬영술은 다른 방법에 비하여 비교적 정확하고, 시행하기 쉬우며 시간이 적게 걸리면서 방사선 장애가 없다는 장점을 가지고 있다. 저자는 정상 한국인의 비장크기에 대한 연구를 위하여 임상적으로나 혈액및 생화학적 검사상, 비종대를 일으킬 원인질환이 없다고 확인된 정상성인 남녀 100명을 대상으로 하여 다음과 같은 결론을 얻었다. 1. 총 100명중 남자가 49명, 여자가 51명이었고, 연령은 30대가 26명으로 가장 많았다. 2. 정상남자의 비장크기는 폭이 6.85±1.31cm, 두께가 4.93±1.27cm, 높이가 6.33±1.45cm이었고 정상여자에서는 각각 6.60±1.23cm, 5.17±1.26cm, 6.32±1.42cm이었다. 그평균치는 폭이 6,73±1.27cm, 두께가 5.06±1.27cm, 높이가 6.33±1.39cm이었다. 3. 비장체적지수(SVI)의 정상범위는 남자에서 8.20±3.94 여자에서 8.41±4.08이었으며 그 평균은 8.31±3.99이었다.
