Abstract
The cutaneous silent period(CSP) is a phenomenon that anterior horn cells are temporarily inhibited by the painful stimulus on afferent cutaneous nerve. Its Clinical interest stems from potential usefulness for evaluating small diameter sensory fibers that are not well assessed by current electrodiganostic methods, and for better understanding central nervous system disorders that perturb motor or sensory processing. But as it varies with the individual responsiveness, the degree of voluntary muscle contraction and the kind of electrical stimulation, it has not been standardized. So, in this study, we address the effect of electrical stimulation intensity and duration on the CSP. Subjects were 20 healthy adults in all. The digital cutaneous nerve of right index finger was stimulated and CSP was measured from the right opponens pollicis muscle. The voluntary contraction of opponens pollicis muscle was maintained in 50% of maximal voluntary contraction, and digital cutaneous nerve was stimulated using 0.1, 0.2, 0.5, 1.0 msec stimulation duration and progressively increasing intensity from 30 mA to 80 mA by 10 mA interval at each stimulation duration. There were interstimulus resting periods of 30 sec to avoid refractory period and muscle fatigue. As a result, with increasing stimulus intensity and duration, CSP duration increased, then it plateaued at some point. This maximal CSP duration was a specific value individually and varied with subject. It ranged from 34.7 to 71.0 (51.2±9.5) msec. To obtain the maximal CSP duration, required stimulation intensity and duration also varied with subject, but the intensity and the duration were above 40 mA and 0.5 msec, respectively. In conclusion, the maximal CSP duration is a specific value individually, and the CSP duration increases with the increment of stimulation intensity and duration before it plateaus. Stimulation duration of 0.5 msec and intensity of 40 mA are required to elicit the maximal CSP duration at least.;경피성 무반응기(cutaneous silent period, CSP)는 척수전각세포의 흥분도가 구심성 경피신경의 전기자극에 의해 일시적으로 억제되는 현상으로, 일반적인 근전도 검사 방법으로는 평가할 수 없는 가는 감각 신경에 대한 평가나 여러 종류의 중추 신경계 병변에서 그 병태생리를 이해하는데 이용할 수 있으나, 그 반응은 개체별 반응도, 수의적 근수축 정도, 전기자극의 종류 등에 따라 변화를 보여 정량화가 되어 있지 않아 임상적 이용에 어려움이 있다. 본 연구의 목적은 경피신경자극의 강도 및 기간 증가에 대한 CSP의 변화를 관찰하는 것이다. 건강한 성인 20명을 대상으로 제 2 수지의 경피신경을 자극하고 우측 모지대립근에서 CSP 지속기간을 측정하였다. 전기자극은 0.1, 0.2, 0.5, 1.0 msec의 전류를 이용하여 각각의 전류에서 30 mA의 강도부터 10 mA씩 강도를 증가시키면서 80 mA까지 자극하였으며, 모지 대립근의 수의적 수축은 50% MVC로 유지시켰다. 말초신경과 척수전각세포의 불응기, 수의적 수축의 근피로를 피하기 위하여 자극간 간격을 30초 이상 두었다. 그 결과 모든 대상자에서 어느 한도까지는 자극 강도와 기간이 증가함에 따라 CSP 지속기간이 증가하다가, 자극 강도와 기간을 증가시켜도 더이상 CSP 지속기간이 증가하지 않는 고평부 현상이 나타났다. 대상자 각각에서 최대 CSP 지속기간을 구할 수 있었는데, 이 값은 대상자 별로 특정 값을 가지며 변이도가 커서, 그 범위는 34.7 ~ 71.0 (51.2±9.5) msec이었다. 최대값에 도달하기 위한 최소 자극 강도와 기간도 개인별 차이가 컸으나, 적어도 0.5 msec, 40 mA 이상의 전류가 필요하였다. 이상의 결과로 최대 CSP 기간은 개체별로 일정한 값을 가지며, 최대 CSP를 유발시키는 자극 이하에서는 전기자극의 강도와 기간이 증가할수록 CSP 기간이 증가하는 것을 알 수 있었다. 또한 젊고 건강한 성인에 있어서 CSP를 구할때는 적어도 0.5 msec, 40 mA 이상의 전류를 사용하여 일관된 반응을 얻을 때까지 자극을 증가시켜가며 검사를 반복해 보아야 할 것으로 생각한다.
