Abstract
목적: 미숙아의 기관지 폐이형성증(BPD)은 저해된 혈관 발달과 폐포 발달로 특징지어진다. 저자는 혈관내피성장인자 수용체 억제제로 유도된 BPD 생쥐 모형에서 혈관 내피 세포에 작용하는 Retinoic acid (RA) 가 정상적인 폐 발달을 유지할 수 있을 것이라는 가설을 증명하고자 하였다. 실험 방법: 실험 동물인 생쥐를 대조군 (n=5), RA군 (n=5), SU1498군 (n=8), SU1498+RA군 (n=6)등. 4군으로 나누었다. SU1498 (30 ㎎/㎏, 피하)는 생후 3일에 일회 주사하였고 RA(2 ㎎/㎏, 복강 내) 는 열흘간 계속 주사하였다. 실험동물은 생후21일에 희생시켜 폐조직을 고정하였다. 기도와 폐 조직의 용적 밀도, 폐포 내면적 및 폐포 평균 용적을 계산하고 전자현미경 소견을 비교 분석하였다. . 내피세포의 정량적인 분석을 위해서 PECAM (platelet endothelial cell adhesion molecule) 면역 염색을 시행하였다. 결과: SU1498를 투여 군에서는 폐포 발달이 저해되어 있었으나 RA를 병용 투여한 군에서는 폐포 용적과 내피세포의 양이 정상으로 유지되었다. SU1498을 투여한 군에서는 폐포 용적이 3배로 증가되어 있었으나 (P<0.05) RA 병용투여 군에서는 대조군과 유의한 차이가 없게 회복되었다 (P<0.05). 결론: 폐포 발달이 저해된 환경에서 RA는 폐포 용적이나 내피세포의 양을 정상적으로 유지시킬 수 있었다. 향후 기관지 폐이형성증의 치료와 예방에 적용될 수 있을 것으로 생각한다.;PURPOSE: Bronchopulmonary dysplasia (BPD) in premature infants is characterized by inhibited vasculogenesis. and inhibited alveolarization. The role of retinoic acid (RA) on endothelial cells in maintaining lung development, was investigated in a BPD mouse model induced with a vascular endothelial growth factor receptor (VEGFR) inhibitor. METHODS: Three-day old mice were allocated to four study groups; control (n=5), retinoic acid (RA) (n=5), SU1498 (n=8), SU1498+RA (n=6). A single dose of SU1498 (30 ㎎/㎏, SC), a commercially available compound that inhibits VEGFR, and concomitant doses of RA (2 ㎎/㎏, IP) for 10 consecutive days were injected respectively. Lungs were inflation fixed on postnatal day 21. Tissue sections were mounted and morphometric analysis was performed to determine the volume density (VD) of air space, tissue, alveolar surface area, mean alveolar volume, and electron microscopic (EM) analysis. PECAM (platelet endothelial cell adhesion molecule) immuno-staining was done to compare the endothelial cell population. RESULTS: Alveolar development was attenuated in the SU1498 treated mice. In contrast, with the addition of retinoic acid treatment, mean alveolar volume and endothelial cell numbers were maintained. The mean alveolar volume, which was three fold increased in the SU1498 treated mice was normalized, with subsequent RA treatment (P<0.05). CONCLUSION: RA maintains mean alveolar volume and endothelial cell numbers in conditions that otherwise result in an inhibition of alveolarization. Thus RA and endothelial cells may be potential targets for developing strategies in the prevention and treatment of BPD in premature infants.
