Three-dimensional comparison of the occlusal contact area between Class I and II molar relationships

Abstract

Introduction: The aims of this study were to evaluate the consistency of the molar relationships from the buccal and lingual aspects by using a three-dimensional (3D) system and to compare the occlusal contact area of the Class II and Class I molar relationships. The null hypothesis is that there is no difference in the occlusal contact area among the different subtypes of Class II molar relationships and Class I molar relationship. Materials and Methods: The definitive sample comprised of 232 pairs dental casts from 232 subjects (166 women and 66 men) and a total of 380 sides (one or both sides). The molar relation on the study casts was evaluated from both the buccal and lingual aspects, and sorted according to the classification defined by Liu and Melsen. The occlusal contact area analysis was performed using 3D digital models, which were generated through surface scanning of the study casts. The areas of maximum intercuspation of the upper and lower casts were calculated by Boolean function in Rapidform XOR3 software (INUS Technology Inc., Seoul, Korea). Results: The majority of buccal Class I group (89.6%) and Class II-m group (86.7%) had cusp-to-central fossa relationship at the lingual aspect; however, Class II-s group had cusp-to-mesial triangular fossa or marginal ridge relationship. The mean occlusal contact area of Class I group was significantly greater than that of Class II groups. And Class II-s group had the lowest value of mean occlusal contact area. However, there were no statistically significant differences in values between the Class I and Class II-m groups (P > 0.05). From the lingual aspect, the mean contact areas between cusp-to-central fossa and other relationships were statistically significant (P < 0.05). However, there was no statistical significance between the cusp-to-mesial triangular fossa and cusp-to-marginal ridge relationships (P > 0.05). Conclusions: These results suggest that the buccal and lingual molar relationships were not always consistent. And there were statistically significant differences in the mean occlusal contact areas between the Class I and Class II molar relationships. Therefore, a more differentiated diagnosis of molar relationships may help us recognize the true nature of malocclusion, on the basis of which, suitable treatment approaches can be applied.;이 논문의 목적은 Class I 과 Class II 구치 관계를 가진 모델을 삼차원적 분석 프로그램을 이용하여 협측과 설측에서 각각 교합 관계를 세분화하고 그 연관성을 평가하는 것이다. 또한 이들의 교합 접촉 면적을 조사하여 비교하는 것이다. 귀무 가설은 Class I 구치 관계와 Class II 구치 관계의 교합 접촉 면적은 통계학적으로 유의한 차이가 없다는 것이다. 232명(여성 166명, 남성 66명)의 선별된 대상자에서 232쌍의 교정 치료 전 진단 모형을 얻었고, 표본은 총 380 측면(우측 또는 좌측)으로 구성되었다. 이들의 구치 관계를 Liu and Melsen 분류에 따라 협측과 설측에서 각각 분류하였다. 진단 모형을 모두 삼차원적 모델 스캐닝을 통해 삼차원적 데이터를 얻었고 이를 Rapidform XOR3?? program 을 이용하여 상악과 하악 모델의 제 1소구치부터 제 2대구치까지의 교차점, 즉 교합 접촉의 면적을 얻었다. 이번 연구 결과를 통해 다음과 같은 결론을 얻었다. 1. 협측에서 I급 (89.6%) 과 미약한 II급 (86.7%) 구치 관계를 보인 표본의 대부분은 설측에서 cusp-to-central fossa (교두-중심와) 접촉 관계를 보였지만, 심한 II급 구치 관계를 보인 표본은 대부분 설측에서 cusp-to-mesial triangular fossa (교두-근심소와) 또는 marginal ridge (교두-변연융선) 접촉 관계를 보였다. 2. I급 구치 관계를 보이는 군은 II급 구치 관계를 보이는 군들의 평균 교합 접촉 면적보다 통계학적으로 유의하게 더 큰 값을 보였다 (P < 0.05). 그리고 심한 II급 구치 관계를 보이는 군의 평균 교합 접촉 면적이 통계학적으로 유의하게 가장 낮은 값은 보였다. 3. I급 구치관계를 보이는 군의 평균 교합 접촉 면적은 미약한 II급 구치 관계를 보이는 군과 통계학적으로 유의한 차이를 보이지 않았다 (P > 0.05). 4. 설측에서 교두-중심와 접촉 관계를 보이는 군의 평균 교합 접촉 면적은 (73.51 ± 8.37 mm2) 다른 군에 비해 통계학적으로 유의하게 가장 큰 값을 보였다 (P < 0.05). 이번 연구를 통해 협측과 설측의 교합 관계가 항상 일치하는 것은 아님을 확인할 수 있었다. 또한 교합 접촉에 있어 상악 제 1 대구치의 회전이 큰 영향을 미침을 알 수 있었다. 따라서 구치 관계에 대한 협측에서 뿐만 아니라 설측에서 좀 더 세분화된 임상적 진단을 통해 부정 교합의 본질을 정확히 인식하고, 정확한 치료 접근을 해야 한다고 사료된다.
