간호사의 보건사업 수행 과정

Abstract

Public health nurses are important in their community, for they serve in various programs of home health care, smoking cessation, health behavior improvement, cancer control, cardiovascular disease control, family health, psychological health and contagious disease control related paper work. There are some problems with public health nursing in personnel shortage, education, role transition. But, little has been studied on the role of public health nurses. Thus, this study utilized a grounded theory approach in order to disclose the public health nursing process of nurses' perspective and develop a substantive theory based on these findings. This study surveyed 19 nurses at 5 public health centers. The data were collected through in depth interviews, participant observation, and other related data. The collected information was analyzed through the grounded theory of Strauss and Corbin(1990). As a result, 49 concepts, 23 subcategories, and 11 categories were deduced from an open coding process. 23 subcategories are ＇difficulties of results-based administrative work ＇, ＇hard managing＇,＇playing a community health protector＇,＇realized necessity of broad health management of residents＇,＇realized residents vulnerability＇,＇checking risen the standard of public health level＇,＇making use of community resources＇,＇planning of concord program＇,＇having practical relationships with co-workers＇,＇having emotional relationships with co-workers＇,＇active serving of public health＇,＇finding alternative with flexibility＇,＇mediating services with client-tailed＇,＇expanding of participating opportunities of clients in public health center＇,＇increased professionalism＇,＇strengthening information＇,＇strengthening drafting＇, ＇understanding of management of practical work＇,＇designing of methods of case management＇,＇setting a program direction＇,＇contributing to program development＇,＇following on existing works＇,＇attaching importance to work responsibility＇. 11 categories emerged from the data :＇facing difficulties of practical administration＇,＇attaching importance to health support for residents＇, ＇assessing of residents' needs＇,＇searching for participation methods of community ＇,＇forming of support system＇,＇working with positiveness＇, ＇improving of resident's health and medical accessibility＇,＇striving for self-development＇,＇carried out promptly for field-based problem solving ＇, ＇mastered of successful program planning methods＇,＇settled for themselves to existing style＇. As for the participants' time-based process, four stages had surfaced, namely, program performing period, program method probing period, program strategy developing period and program competencies application period. The core category,＇strengthening of practical·planning work competencies＇, incorporated the relationship between and among all categories and explained the process of working performance. And properties of＇strengthening of practical·planning work competencies＇are＇understanding＇,＇flexibility＇,＇creativity＇. Concretely, according to degree of the properties, there are differences.＇Strengthening of practical·planning work competencies＇was described as a substantive theory, defined as the process each nurse experienced to function as a public health nurse. This study has described public health nurses' characteristics and working performance style in Korea. These findings have important implication for the practice of public health program and must be considered to develop the competencies for planning and practice of public health nurses.;보건소에서 간호사는 지역주민의 건강수준을 향상시키고자 방문건강관리, 금연, 건강행태개선, 암 관리, 심혈관계질환 예방관리, 정신보건, 모자보건, 전염병 관리 등 사업과 행정업무를 수행하는 중요한 위치에 있다. 이에 보건간호사는 지역특성을 반영한 보건사업을 계획, 수행, 평가하는 업무를 담당하고 있다. 그러나 보건사업 영역별 전담인력의 부족과 직무교육과정의 불충분성 및 시립병원과 보건소 간의 순환근무로 인한 직무 유동성으로 초래되는 보건사업에 대한 간호사의 전문성 부족이 문제점으로 보고된 바 있다. 이에 본 연구는 근거이론적 접근을 사용하여 간호사의 보건사업 수행과정을 밝히고 그에 대한 실체이론을 개발하고자 하였다. 연구의 참여자는 보건사업을 수행하는 간호사 19명이었으며, 심층면담을 통해 자료를 수집하였다. 수집된 자료는 Strauss와 Corbin(1990)의 근거이론방법을 적용하여 분석하였다. 연구결과 개방코딩 과정에서 49개의 개념과 23개의 하위범주, 11개의 범주가 도출되었으며, 23개의 하위범주는 ＇실적중심으로 인한 행정업무의 버거움＇,＇감당하기 힘듦＇,＇지역주민의 보호자 역할을 감당함＇,＇지역주민의 포괄적 건강관리의 필요성을 깨달음＇,＇지역주민의 취약성을 파악함＇,＇지역주민의 서비스 기대수준이 높아짐을 확인함＇,＇지역사회 자원을 활용함＇,＇지역주민에게 호응받는 사업을 구상함＇,＇동료와 업무적 유대관계를 가짐＇,＇동료와 정서적 유대관계를 가짐＇,＇능동적인 자세로 사업을 수행함＇,＇융통성 있게 대안을 찾아줌＇,＇지역주민 맞춤형으로 서비스 조정하기＇,＇지역주민의 보건소 참여기회 확대하기＇, ＇전문성 따라잡기＇,＇정보력 강화하기＇,＇기안력 강화하기＇,＇실무관리 방법을 터득함＇,＇현장중심의 사례관리 방법을 고안함＇,＇사업방향 설정을 주도함＇, ＇새로운 사업개발에 기여함＇,＇기존 업무내용을 답습함＇,＇업무분장을 중요시함＇이었다. 축 코딩에서 간호사의 보건사업 수행 구조는＇실무행정의 어려움에 직면함＇, ＇지역주민의 건강지원을 중요시함＇이라는 인과적 조건과＇지역주민의 요구를 파악함＇의 맥락적 조건에 영향을 받아 나타나는＇지역사회 참여의 사업방법을 모색함＇의 중심현상에 대해＇지지체계를 형성함＇,＇적극적으로 보건간호서비스를 제공함＇의 중재적 조건에 영향을 받으면서＇지역주민의 보건의료 접근성을 높여줌＇,＇자기계발을 위해 노력함＇의 전략을 사용하여＇현장중심의 문제해결을 위해 신속히 처리함＇,＇성공적인 사업기획을 위한 방법을 터득함＇,＇기존업무방식에 안주함＇이라는 결과를 도출하였으며, 간호사의 보건사업 수행 과정은＇사업수행 갈등기(葛藤期)＇,＇사업방법 탐색기(探索期)＇,＇사업전략 개발기(開發期)＇, ＇사업역량 적용기(適用期)＇의 4단계로 구분할 수 있었다. 즉,＇사업수행 갈등기＇는 간호사가 보건사업에 대한 부담감으로 사업수행의 갈등을 경험하지만 지역주민의 건강지원을 중요시 하는 시기이고,＇사업방법 탐색기＇는 간호사가 지역주민의 요구를 파악하여 지역사회 참여방법을 모색하는 시기이며,＇사업전략 개발기＇는 지지체계를 형성함과 적극적으로 보건간호서비스를 제공함에 영향을 받아 지역주민의 보건의료 접근성을 향상시키는 방법과 자기계발을 위해 노력함으로 전략을 개발하는 시기이며, 마지막으로＇사업역량 적용기＇에서는 간호사의 유형에 따라 실무행정 유형은 현장중심의 문제해결을 위해 신속히 처리함, 기획행정 유형은 성공적인 사업기획을 위한 방법을 터득함 및 소극적 중립 유형은 기존업무방식에 안주함의 결과를 초래하여 간호사의 역량을 사업역량으로 적용하는 시기이다. 이러한 과정은＇실무·기획 역량 강화하기＇이었다. 이에 선택코딩에서 간호사의 사회·심리적 과정으로 드러난 핵심범주는＇실무·기획 역량 강화하기＇이었다. 즉, 지역주민의 보건의료 접근성을 높여줌을 주요 전략으로 사용한 간호사는 현장중심의 문제해결을 위해 신속히 처리함을 나타냈고, 자기계발을 위해 노력함을 주요 전략으로 사용한 간호사는 성공적인 사업기획을 위한 방법을 터득함을 나타냈다. 또한 전략의 사용이 미흡한 초보 간호사나 팀조직의 업무에만 몰두한 간호사는 기존업무방식에 안주함이라는 결과를 가져왔다. 따라서＇실무·기획 역량 강화하기＇의 속성은 보건사업에 대한 이해도, 사업수행의 유연성, 사업계획의 독창성이었다. 구체적으로 보건사업에 대한 이해도가 강한지와 약한지, 사업수행의 유연성이 높거나 낮은 정도, 사업계획의 독창성이 높거나 낮은지의 차원을 달리 하였다. 이상의 결과는 간호사의 보건사업 수행 과정과 구조를 분석하여＇실무·기획 역량 강화하기＇의 실체이론을 제시하였으며, 간호사의 보건사업 수행 과정에 따른 직무특성을 이해하는데 도움을 주는 기초자료를 제공하였다.
