뇌졸중 노인의 우울, 수면의 질과 수면 만족도의 관계

Abstract

The purpose of this study was to investigate the relationship between depression and sleep in community dwelling older post stroke patients. The subjects were 100 stroke patients who had been discharged, were taken follow-up care at the outpatient department, and were older than 60 years. The data were collected by interview through with questionnaires from August to October 2008. For measuring depression, the SGDS tool (Yesavage, Brink & Rose, 1983), which was translated into Korean by Cho (1999), was used. Quality of sleep was measured with Korean sleep scale of Oh (1998). The satisfaction of sleep was measured using the survey of questions that answered from 0(not satisfaction) to 10(much satisfaction). The data were analyzed by frequency, percentage, mean, t-test, ANOVA, Pearson's Correlation Coefficient. The results were as follows : The means of depression was 8.25±4.33 and the means of quality and satisfaction of sleep were 41.11±5.12 and 6.64±1.91. The result showed significant differences between general characteristics and depression, gender(t=2.882, p=.005), spending money(F=4.415, p=.006), antidepressants(t=3.010, p=.003), sleeping pills(t=3.785, p=.000), and physical activity level in daily life(r=-.396, p=.000). The quality of sleep of patients showed statistically significant differences according to in whether they had a spouse or not(F=4.232, p=.017), living with there family(F=6.247, p=.003), antidepressants(t=-2.822, p=.006), sleeping pills(t=-6.725, p=.000), and physical activity level in daily life(r=.346, p=.000). The satisfaction of sleep shown statistically significant differences according to in whether they had a spouse or not(F=5.506, p=.005), living with there family(F=5.699, p=.005), antidepressants(t=13.440, p=.000), sleeping pills(t=30.762, p=.000), and physical activity level in daily life(r=.556, p=.000). There were significant negative correlations between stroke patients' depression and quality of sleep(r=-.358, p=.000) and depression and satisfaction of sleep(r=-.460, p=.000). Based upon these findings, which indicated correlation with depression, quality of sleep and satisfaction of sleep, it is necessary to provide nursing intervention to improve stroke patients' depression and quality of sleep.;본 연구는 뇌졸중을 진단받은 노인들이 경험하는 우울, 수면의 질과 수면만족도를 파악하여 환자들의 삶의 질을 유지 증진시키기 위한 기초 자료를 마련하고자 시도된 서술적 상관관계 연구이다. 연구 대상은 뇌졸중을 진단받고 S시에 소재한 A병원의 신경과 외래를 방문하는 60세 이상의 노인환자 100명으로 하였다. 우울 측정을 위한 연구 도구는 조맹제 등(1999)이 번역한 단축형 노인 우울 척도(Short Form Geriatric Depression Scale, SGDS)를 사용하였다. 수면의 질 정도는 오진주, 송미순과 김신미(1998)가 개발한 수면측정도구를 사용하였으며 수면만족도는 10점 척도의 1개 문항을 사용하였다. 자료수집기간은 2008년 8월부터 10월까지였으며 자료 분석은 SPSS/WIN 12.0 프로그램을 이용하였다. 본 연구의 결과는 대상자의 우울 정도는 평균 8.25±4.33으로 나타났으며 SGDS의 우울 점수 분류에 의하면 정상이 25%(25명), 우울 의심이 27%(27명), 우울이 48%(48명)로 나타났다. 대상자의 수면의 질은 평균 41.11±5.12로 중간 정도였고 수면만족도는 평균 6.64±1.91로 보통 이상으로 만족하였다. 우울은 월 평균 용돈(F=4.415, p=.006)이 많을수록, 일상생활 수행능력(r=-.396, p=.000)이 높을수록 우울 정도가 낮았으며 뇌졸중 병력(F=3.329, p=.040)이 많을수록 우울 정도가 높았다. 또한 성별(t=2.882, p=.005), 항우울제 복용(t=3.010, p=.003), 수면제 복용(t=3.785, p=.000)에 따라서 유의한 차이가 있었다. 수면의 질은 배우자의 유무(F=4.232, p=.017), 동거가족(F=6.247, p=.003), 항우울제 복용(t=-2.822, p=.006), 수면제 복용(t=-6.725, p=.000), 일상생활수행능력(r=.346 p=.000)에 따라 통계적으로 유의한 차이가 있었다. 이는 배우자가 있고 부부나 자녀와 함께 살고 있는 경우와 일상생활 수행능력이 높을수록 수면의 질이 높게 나타났다. 수면의 만족도는 배우자의 유무(F=5.506, p=.005), 동거가족(F=5.699, p=.005), 항우울제 복용(t=13.440, p=.000), 수면제 복용(t=30.762, p=.000), 일상생활수행능력(r=.556, p=.000)에 따라 통계적으로 유의한 차이가 있었다. 대상자의 우울 정도가 높을수록 수면의 질(r=-.358, p=.000)과 수면의 만족도(r=-.460, p=.000)는 낮았다. 수면의 질이 높을수록 수면의 만족도는 높은(r=.586, p=.000) 것으로 나타났다. 이상의 연구 결과에서 뇌졸중을 진단받은 노인의 우울, 수면의 질과 수면 만족도는 상관관계가 있으며 우울 정도가 낮을수록 수면의 질과 수면 만족도가 양호함을 알 수 있었다. 이에 따라 우울을 감소시키고 수면의 질을 증가시킬 수 있는 간호중재의 제공이 필요하리라 본다.
