아동의 보완대체요법 이용실태에 관한 연구

Abstract

보완대체요법은 그 효과와 안정성이 과학적으로 완전하게 증명되지 않았음에도 아동의 안녕과 건강을 위해 보완대체요법에 대한 관심과 이용률이 증가하고 있다. 한편 외국에서는 아동의 보완대체요법에 대한 연구들이 활발히 진행되고 있으나 우리나라의 경우는 거의 이루어지지 않은 실정이다. 이에 본 연구는 어머니를 대상으로 아동을 위한 보완대체요법 이용과 종류, 그에 관련된 요인 및 보완대체요법에 대한 인식을 파악하여 아동 건강관리자로서 어머니들에게 보완대체요법 시행과 관련된 정확한 정보를 제공하고 아동 간호중재로서 보완대체요법을 개발, 적용하는데 기초 자료를 제공하고자 시도하였다. 연구방법은 2006년 4월 1일부터 4월 30일 사이에 서울 E 대학병원 소아과 병동과 소아과 외래, E 어린이집, S 스포츠 센터에 방문한 아동의 어머니를 대상으로 질문지를 이용하여 조사하였으며, 수집된 322명의 자료는 SPSS 12.0을 이용하여 실수와 백분율, χ²-test로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 아동이 건강할 때 이용한 보완대체요법은 생물학적 기반요법, 심신조정요법, 한의학치료, 수기 신체요법의 순이었으며 질병이 있을 때는 생물학적 기반요법 다음으로 한의학 치료가 많았다. 또한 지난 1년간 지속적으로 이용한 방법은 생물학적 기반요법, 심신조정요법, 수기 신체요법, 한의학 치료의 순으로 아동들은 생물학적 기반요법, 특히 잡곡류, 사골, 종합비타민, 종합영양제 등을 가장 많이 이용하는 것으로 나타났다. 아동과 어머니의 특성별 보완대체요법 이용은 아동의 나이가 많을수록, 현재 호흡기 질환을 앓고 있는 경우, 어머니의 나이가 많을수록, 경제상태가 좋을수록 보완대체요법의 이용이 많았다. 2. 보완대체요법에 대해 어머니들은 ‘질병치료에 도움이 된다’, ‘병원치료와 병행하면 좋을 것’이라고 인식하였다. 또한 아동이 첫째인 경우, 종교가 있는 어머니, 학력이 높은 어머니가 보완대체요법이 질병치료에 도움이 된다고 생각하고 있었다. 3. 보완대체요법 이용률은 86.6%이며, 아동이 건강한 상태에서 시작하는 것으로 나타났다. 보완대체요법을 접하게 된 경로는 친구, 주변사람의 권유가 가장 많았으며, 사용해본 사람이 효과가 있다고 추천해서 이용한다고 하였다. 4. 보완대체요법을 이용하면서 건강증진을 가장 많이 기대하였고, 이용 후 48.4%에서 심리적 안정이 되었다고 하였으나 소수에서는 부작용을 경험하였다. 지출비용은 5만원 미만이 가장 많았고, 이용 후 다수가 만족하였다. 5. 보완대체요법을 중단한 이유는 비용부담과 효과가 없어서이며 한약을 가장 많이 중단하였다. 보완대체요법의 이용에 대해 62.0%가 의료진과 상의하지 않은 것으로 나타났으며 상의하였을 때 의료진의 반응은 긍정적이었다가 53.8%였다. 현재 보완대체요법을 이용하는 어머니의 다수가 지속적인 이용의사와 권유의사를 보였다. 이상의 연구결과를 통해 간호사는 병원에 입원한 아동의 대부분이 건강유지 및 증진, 질병예방을 위해 보완대체요법을 이용하고 있다는 것을 인식하고, 간호사정시 아동의 보완대체요법 이용에 대해 묻고, 간호중재에 적용할 수 있도록 해야 한다.;Complementary and alternative medicine for children have been increasing in their usage, but limited studies in this area have been found in the literature. The purposes of this study were to describe the use of the complementary and alternative medicine, and to examine mothers' perception of the use of such therapies for their children. Findings from this study are expected to be useful for improving parents' knowledge of the complementary and alternative medicine as well as for developing and applying complementary and alternative nursing interventions for the young population. The subjects for this study were the mothers of children (N=322) who visited pediatrics department of 'E' University hospital in Seoul, 'E' Children's House and 'S' Sports Center for the period between April 1 and 30, 2006. Descriptive statistics and chi-square test were employed for analysis using the SPSS 12.0 The major findings of this study are: 1. The most frequently used CAM was the "biologically-based therapies" such as mixed grains and cow ethmoid, followed by the "mind-body therapies" (e.g., exercise, prayer), "oriental medicine", (e.g., chinese medicine, acupuncture) and the "manipulative and body-based methods" (e.g., massage, acupressure). Study also presented that the presence of respiratory disease of children and both mother and children's age were positively associated with the use of complementary and alternative medicine. 2. Mothers were found to perceive that the complementary and alternative medicine were 'helpful to the treatment of diseases' or 'useful if in parallel with hospital treatment'. Mothers who had a religion and higher educational background showed a favor to the complementary and alternative medicine for their first children. 3. A total of 86.6% of the mothers used the complementary and alternative medicine. Most of them had tried such therapies since their children were healthy, and were informed by friends or neighbors who already had experienced such therapies. 4. Study results presented that mothers most expected general health improvement of their children through the complementary and alternative medicine. Also, 48.4% of them perceived that their children became emotionally stabilized by using the therapies, while a few of them experienced some side-effects such as vomiting, rash. Mothers spent less than 50 thousand won for the therapies, and the majority of them were satisfied with their results. 5. Mothers discontinued the complementary and alternative medicine due to no apparent effects and financial burden, and the oriental medicines were most easily quitted. Approximately one-thirds of the mothers (62.0%) had not consulted the medical staff about their uses of the complementary and alternative medicine, and 53.8% of those who had consulted the medical staffs, heard their favor response to the therapy. The majority of the mothers using the therapies were willing to use such therapies continuously(N=252, o=90.3%) and to recommend them to others(N=238, o 85.3%). Study findings present that most hospitalized-children were using some of the complementary and alternative medicine, and suggest that nurses need to include the complementary and alternative medicine in their nursing checklist for their care plans.
