Abstract
Amikacin is an aminoglycoside antibiotic which is widely used in the management of serious infections caused by gram-negative organisms in patients with solid or haematological malignancies, and as empirical therapy for febrile episodes in neutropenic patients. Determination of the drug concentration in serum is necessary in this kind of patients due to the life-threatening nature of infections in neutropenic patients and the narrow therapeutic index of amikacin. Increased aminoglycoside distribution volumes and clearances have been noted in patients with malignancies. However, these kinetic alterations are not uniform in all subgroups of malignancies and cannot be attributed to any single variable such as malignancy type, degree of neutropenia, exposure or type of chemotherapy. The purpose of this study was to describe pharmacokinetics of amikacin in Korean patients with solid or haematological malignancies, to evaluate the effects of several clinical and physio-pathological factors on the disposition kinetics of this drug, and to find potential predictive factors for dosage individualization. Amikacin pharmacokinetic parameters were studied retrospectively in 109 inpatients with an underlying malignancy and who were admitted to National Cancer Center in Korea, between January 2006 and June 2007. Almost patients received amikacin in combination with beta-lactams. Serum drug concentrations were monitored by our Clinical Pharmacokinetic Service. A total of 288 blood samples, including 144 samples containing both peak and trough concentrations, taken at steady state were analyzed. The observed amikacin volume of distribution, clearance and elimination rate constant were significantly greater than predicted (0.37 ± 0.12 versus 0.25 L/kg [P = 0.000], 0.084 ± 0.047 versus 0.048 ± 0.018 L/hr/kg [P = 0.000], 0.24 ± 0.17 versus 0.19 ± 0.07 hr^(-1) [P = 0.000], respectively). Patient factors found to correlate significantly with amikacin clearance and elimination rate constant were age, weight, % of ideal body weight, serum creatinine, blood urea nitrogen(BUN) and creatinine clearance. Age, serum creatinine, BUN, creatinine clearance, albumin, daily dose were correlated with amikacin half-life and albumin correlated with volume of distribution. According to stepwise linear regression to examine further the association between the factors analyzed by correlation analysis and the measured amikacin pharmacokinetic parameters, a decrease in albumin was associated with a larger volume of distribution and a longer half-life. An increase in age, an increase in weight or an increase in serum creatinine were associated with a decrease in clearance. Amikacin volume of distribution was significantly increased and clearance and elimination rate constant were significantly decreased with increasing age. Patients were divided into 3 groups based on % of ideal body weight(IBW), an underweight group(patients weighing < 90% of their IBW), an ideal-weight group(patients weighing within 10% of their IBW, and an overweight group(patients weighing > 110% of their IBW). Clearance and elimination rate constant were significantly different among the three groups and were significantly decreased in overweight group. However, volume of distribution and half-life did not differ significantly. A decreased creatinine clearance was associated with an increase in half-life and decrease in amikacin clearance and elimination rate constant. Amikacin volume of distribution and half-life were significantly increased and clearance and elimination rate constant were significantly decreased in hypoalbuminemia(< 3.0 g/dL). No association with type of malignancy(solid versus haematological) was found but volume of distribution was greater in lung cancer or colorectal cancer patients. Amikacin clearance and elimination rate constant were significantly elevated in the presence of febrile episode. The relationships between amikacin pharmacokinetic parameters and total bilirubin, BUN-creatinine ratio, exposure or type of chemotherapy, neutropenia, concomitant medication(penicillins or cephalosporins, vancomycin) were not statistically significant. Therapeutic concentrations(peak 20 - 30 ㎍/mL, trough < 5 ㎍/mL) of amikacin were achieved in 67 patients(46.5%) and sub-therapeutic concentrations were achieved in 55 patients(38.2%). Because of the wide interindividual variability in amikacin pharmacokinetics in patients with malignancies, combination factors such as age, weight, serum creatinine and albumin should be considered for optimizing the dosing regimen.;Aminoglycoside계 항생제는 암환자에서 그람 음성균에 대한 중대한 감염의 치료와 호중구감소 환자의 열성 반응의 경험적인 치료에 널리 사용되고 있다. 호중구감소 환자에서 감염은 치명적이고 약물의 유효혈중농도 범위가 좁아 암환자와 같은 고위험군의 환자에서 개별적으로 적정 용량을 설정하고 모니터링 하는 것이 중요하다. 특히 암환자에서 aminoglycoside의 분포용적과 청소율이 증가하는 등 약동학적 차이가 보고되고 있다. 하지만 이러한 약동학적 변화는 암환자의 특성에 따라 동일하지 않고, 암종별, 질병 상태, 호중구감소, 항암화학요법 시행여부와 같이 어떤 한 인자에 의한다고 볼 수 없다. 또한 이러한 인자들에 대해 용량 설정시 고려할 수 있는 투여 지침이 제시되어 있지 않다. 본 연구에서는 한국 암환자에서 amikacin의 약동학적 매개변수를 설명하고, amikacin의 약동학에 영향을 미치는 요소를 평가하여 용량 개별화를 위한 잠재적이고 복합적인 예상 요소를 파악하여 치료기간 최적의 용량 지침을 설정할 수 있도록 방향을 제시하고자 하였다. 2006년 1월부터 2007년 6월까지 국립암센터에 입원하여 amikacin을 투여 받고 임상약동학실에서 혈중농도 모니터링이 실시되었던 고형암 및 혈액암 환자를 대상으로 후향적인 연구가 실시되었다. Amikacin의 평균 투여용량은 14.6 ± 4.3 mg/kg/day 이었고, 투여간격은 14.6 ± 5.7 hr 이었다. 대부분 beta-lactam계 항생제나 vancomycin과 병용되었다. 109명의 환자로부터 144쌍(최고혈중농도 및 최저혈중농도)의 혈중농도를 분석한 결과 amikacin의 분포용적, 청소율, 소실속도상수가 예상 추정치에 비해 유의성 있게 증가하였다(각각 0.37 ± 0.12 versus 0.25 L/kg [P < 0.0001], 0.084 ± 0.047 versus 0.048 ± 0.018 L/hr/kg [P < 0.0001], 0.24 ± 0.17 versus 0.19 ± 0.07 hr^(-1) [P < 0.0001]). Amikacin의 약동학에 영향을 주는 인자를 조사하였을 때, 청소율 및 소실속도상수는 체중, 표준체중 백분율, blood urea nitrogen(BUN), 혈청 creatinine, creatinine clearance와 유의적 상관성을 보였다. 선형회귀분석 결과 연령, 체중, 혈청 creatinine이 amikacin의 청소율과 음의 상관관계를 나타냈다. Amikacin의 분포용적은 albumin과 유의적 상관성이 있었고, 반감기는 albumin, creatinine clearance, aspartate aminotransferase, 투여량과 유의적 상관성이 있었다. 나이에 따른 amikacin의 약동학적 변수의 차이를 살펴보았을 때, 나이가 증가할수록 반감기는 유의성 있게 증가하고 청소율과 소실속도상수는 감소하는 경향을 나타냈다. 체중에 따라 저체중, 정상체중, 과체중 3개의 군으로 나누어 amikacin의 약동학적 변수의 평균을 비교했을 때, 분포용적과 반감기는 유의적 차이가 없었고, 과체중군에서 amikacin의 청소율과 소실속도상수가 감소하였다. Creatinine clearance에 따라 보면, amikacin의 반감기는 creatinine clearance가 40 - 60 ml/min인 경우 정상에 비해 2배, 40 ml/min 미만인 경우 정상에 비해 3배 가량 연장되어 있었고, 청소율과 소실속도상수도 각각 2배, 3배 가량 감소하였다. Albumin에 따라 보면, 저알부민(< 3.0 g/dL)에서 분포용적과 반감기는 유의성 있게 증가하였고, 청소율과 소실속도상수는 감소하였다. 암종별로 살펴보았을 때, 폐암, 직장결장암 환자에서 분포용적이 증가하는 경향을 보였고, 열성 반응을 동반하는 환자에서 청소율과 소실속도상수가 증가하는 경향을 보였다. 항암화학요법 시행여부, 호중구감소, 병용 약물, 고형암 또는 혈액암에 따른 약동학의 차이를 살펴보았을 때 유의적 차이가 없었다. 이처럼 암환자에서 amikacin의 약동학은 정상인구에 비해 분포용적 및 청소율이 증가하는 경향이 있고, 이러한 차이는 여러 가지 변수의 영향을 받으므로 환자의 연령, 체중, 신기능, albumin 수치와 같은 변수들을 고려하여 용량을 조절하고 약물혈중농도 모니터링을 실시하여 효과적인 치료가 이루어지도록 해야 할 것이다.
