Abstract
The positive impacts of providing written prescription medicine information to patients have been well recognized. Written medication information can increase patient knowledge, satisfaction and compliance. Consumer-friendly written patient information is available in many countries and provided to consumers by regulation or voluntarily, although not yet in Korea. This study was undertaken to provide an overview of the US voluntary written drug information program for consumers, to describe current status of written drug information in Korean hospital pharmacy and to find what is needed to provide useful written medication information to Korean patients. Written medication information materials were collected from 6 hospital pharmacies in Seoul, All 95 materials were analyzed by information type (contents) they provide. Contents include brand name, generic name, drug class, strength, appearance, indications, contraindications, precautions, how much to take, how often to take, how to administer. action in case of overdose, action in case of a missed dose, adverse reactions, storage instructions, hospital name, hospital phone number, and date of revision. The number of contents in each material is also calculated. Usefulness of atenolol, glyburide, atorvastatin, nitroglycerin leaflets was measured by criteria used in 2001 US national study. US voluntary program was introduced in 1996 by Public Law 104-180 which called for voluntary distribution of leaflets through private-sector efforts. The law allowed six months for a private-public collaboration to develop an action plan to achieve goals, that is, distribution of useful written information to 75% of individuals receiving new prescriptions by the year 2000 and 95 % by the year 2006. The collaborative process has been completed and the long-range private-sector plan, "Action Plan for the Provision of Useful Prescription Medicine Information" was accepted in 1997 by Secretary of Health and Human Services. The voluntary program seems to have some advantages over government regulation. For example, off-label use is allowed in customized information and many private-sector initiatives are encouraged to develop the more effective leaflets. 2001 US national study, commissioned by FDA, revealed that 89% of patients received written information and the overall usefulness of information was about 50%. Public meeting is scheduled in July 2003 to discuss how to improve the quality of patient prescription medicine information in order to meet the year 2006 goal. There was a high variability in type and quantity of information by route of administration and by therapeutic class of drugs across the study hospitals. Mean number of contents included in materials is 7 and the contents most often missed were action in case of overdose and date of revision. Materials for external use medicine and respiratory tract medicine were focused primarily on dosage and administration, with little risk information. For cardiovascular drugs. only one hospital provided action in case of a missed dose. The usefulness was compared with US mean points by criteria. The scores for individual criteria of Korean materials were frequently lower than US mean points and high variable by hospitals. Written materials are expected to be used widely as an adjunct to verbal medication counseling due to benefits of written information and inadequacy of verbal counseling in Korea. Guidelines are needed to assure the quality of written information considering the high variability observed in Korean materials. US quality evaluation process based on its own guidelines, collaboratively developed, may be helpful to improve the quality of Korean materials. The useful written information can promote patients' health and save health care costs for preventable drug-related illness by helping patients use drug products safely and effectively. Korean guidelines for written medication information should be developed in collaboration with consumers and continuing quality evaluation of the written information is needed to provide useful information to Korean patients.;환자에게 처방의약품에 관한 서면 자료를 제공하는 것은 환자의 지식, 만족도, 순응도 등을 증가시킬 수 있는 장점이 있다고 알려져 있다. 우리나라에서는 아직 널리 쓰이지 않고 있으나 많은 나라에서 정부규제에 의해 또는 자발적으로 처방의약품에 대한 환자친화적인 서면 자료를 환자에게 제공하고 있다. 본 연구는 미국의 자발적인 서면 자료 제공 프로그램을 조사하고 현재 우리나라 종합병원에서 사용하는 복약지도용 서면 자료의 현황을 파악하여 우리나라 환자에게 유용한 자료를 제공하기 위해서 필요한 점이 무엇인지 알아보기 위해 시행되었다. 미국의 자발적 서면 자료 제공 프로그램의 현황은 문헌고찰을 통해 조사하였다. 우리나라 현황을 알아보기 위해 서울에 위치한 종합병원 6곳의 약제부에서 사용하는 복약지도용 서면 자료를 수집하였다. 수집된 자료 95건이 상품명, 일반명, 약물군, 함량, 성상, 적응증, 금기, 주의사항, 투여용량, 투여횟수, 용법, 복용을 잊었을 때 취해야 하는 행동, 과량복용시 나타날 수 있는 증상 및 대처법, 부작용, 보관방법, 병원명, 병원전화번호, 자료의 최종수정일 등 총 18개 항목에 관한 정보를 제공하는 지 여부를 조사하였다. 자료에 포함된 항목의 수 및 특정 항목을 표시한 자료의 수가 투약경로별, 약효군별, 병원별로 차이가 있는 지 분석하였다. 또한 2001년 FDA의 위탁연구에서 사용한 기준에 따라 우리나라의 Atenolol, Glyburide, Atorvastatin, Nitroglycerin 자료의 유용성을 평가하였다. 미국의 자발적 서면 자료 프로그램은 1996년 Public Law 104-180에 의해 도입되었다. 이 법은 민간부문이 자발적으로 환자에게 처방의약품에 대한 서면 자료를 제공하도록 하였으며 2000년까지는 새 처방을 받는 75%의 환자가, 2006년까지는 95%의 환자가 처방의약품에 대한 유용한 서면 자료를 받는 것을 목표로 하였다. 이 법에 따라 민간-공공 부문은 6개월간 협의하여 이 목표를 달성하기 위한 계획안인 "Action Plan for the Provision of Useful Prescription Medicine Information"을 마련하였으며 1997년 1월 보건부장관의 승인을 받았다. 2001년에 실시된 전국 현황평가에서 89%의 환자가 약국에서 처방의약품과 함께 서면 자료를 받았고 자료의 전체적인 유용성은 50%로 조사되었다. 2006년 목표를 달성하기 위해 자료의 유용성을 향상시킬 방법을 논의하고자 2003년 7월 공청회가 예정되어 있다. 서울에 위치한 6개 종합병원 약제부에서 사용하는 복약지도용 서면 자료를 분석한 결과 수집된 자료들은 총 18개 항목 중 평균 7개의 항목을 포함하고 있는 것으로 조사되었다. 약물의 투여경로별 및 약효군별, 병원별로 자료에 포함된 항목의 개수 및 종류에서 차이가 많았다. 호흡기약물이나 외용제 자료는 사용법에 관한 정보를 주로 제공하는 반면 약물의 위험성에 관한 정보는 거의 제공하지 않았다. 세 병원의 순환기약물 자료 중에서 복용을 잊었을 때 대처법은 한 병원의 자료에만 포함되어 있었다. Atenolol, Glyburide, Nitroglycerin, Atorvastatin 자료의 기준별 점수는 전반적으로 미국의 평균값보다 낮았고 국내 자료간에도 병원에 따라 차이가 있는 것으로 조사되었다. 최근 우리나라 환자를 대상으로 한 연구에서도 서면 자료의 이점이 보고되고 있고 구두설명만으로는 환자들의 약물정보에 대한 수요와 필요를 충족시키기 어려운 것으로 보이므로 서면 자료가 복약지도의 보조 도구로 사용될 가능성은 더 높아지고 있다. 하지만 6개 병원의 자료에 포함된 약물정보가 병원별로 또는 약물별로 차이가 있었다는 결과를 고려하면 우선 자료의 품질을 보장하기 위해 우리에게 적합한 서면 자료의 기준을 정하는 것이 필요하다고 판단된다. 미국의 제도는 허가사항 외 용도를 개인별 자료에 표시하는 것을 허가하고 민간 업체가 더 적합한 자료를 개발하도록 동기부여를 한다는 점에서 정부가 제조업체를 통해 획일적인 설명서를 제공하는 방법보다 우수한 것으로 생각되며 소비자단체 등 관련 단체가 협의하여 서면 자료의 기준을 정하고 이 기준에 따라 자료의 질적 평가를 하는 미국의 방식은 우리나라 서면 자료의 문제점을 해결하는 데 도움이 될 것으로 보인다. 유용한 서면 자료는 의약품을 안전하고 효과적으로 사용하기 위해 필요한 정보를 환자들에게 이해하기 쉽게 전달함으로써 환자의 건강증진과 불필요한 의료비 감소에 기여할 수 있다. 우리나라 환자에게 이러한 서면 자료를 제공하기 위해서는 우선 소비자와 협의하여 우리나라에 적합한 가이드라인을 정해야 하며 이를 기준으로 자료의 질적 평가가 지속적으로 시행되어야 할 것이다.
