Abstract
본 연구는 일 대학병원에서 근무하는 외래간호사의 업무를 분석하고, 그 업무에 대한 중요도를 파악함으로써 외래간호사의 역할을 재정립하기 위한 기초자료를 제공하고자 시도되었다. 본 연구의 대상자는 일 종합병원 외래간호사 53명 전수를 대상으로 하였으며, 연구 도구는 문헌고찰과 사전조사를 통해 작성된 외래 간호업무 측정도구를 사용하여 2007년 4월 17일부터 4월 19일 3일 동안 자료를 수집하였다. 수집된 자료는 SPSS 12.0 program을 이용하여 외래간호사의 업무내용 및 소요시간, 업무 중요도를 평균과 백분율을 이용하여 분석하였다. 본 연구의 결과는 다음과 같다. 1) 대상자의 평균 연령은 32.7세이고, 기혼자가 64.2%이다. 직위로는 일반간호사 62.3%이고, 4년제 간호대학교 졸업자 50.9%였다. 임상경력은 평균 10년이고, 외래 근무경력은 평균 3년 2개월이다. 외래 간호사의 간호직에 대한 만족도는 만족 56.6%, 외래 근무지에 대한 만족도는 만족 47.1%이다. 2) 대상자의 10개 영역 업무에서 소요시간이 가장 많은 업무는 외래통상업무로 299.94분이었다. 소요시간이 작은 업무는 업무개선활동업무(8.17분), 환자옹호업무(15.15), 물품 및 환경관리업무(17.91분)였다. 세부 간호업무에서 접수 및 절차(44.21분), 예진(33.17분), 재진예약 및 설명(30.44분), 처방입력 및 확인(27.70분), Chart review(26.46분), 건강상담(24.66분)의 순으로 시간을 많이 소요하는 것으로 나타났다. 심폐소생술(0.14분), 검체물 접수(0.64분), 검체체취(0.83분)는 외래 간호사의 업무로 거의 이루어지지 않는 것으로 나타났다. 3) 대상자가 중요하다고 인식하는 업무는 환자 상담 및 교육업무가 평균 4.42점으로 가장 높게 나타났고, 물품 및 환경관리업무(3.64점), 업무개선활동업무(3.64점), 서류업무(3.80점)는 중요도가 낮았다. 세부 간호업무에서 투약 교육 및 상담이 중요도(4.76점)가 가장 높게 나타났으며, 다음으로 질환관련 설명(4.53점), 검사ㆍ시술ㆍ수술관련 설명(4.50점), 건강상담(4.40점), 검사결과 관리(4.29점), 예진(4.08점)의 순으로 나타났다. 4) 대상자의 간호업무 소요시간은 연령, 직위, 임상경력, 외래경력에 따라 업무 중요도는 학력에 따라 통계적으로 유의한 차이가 있었다. 5) 간호업무 소요시간과 중요도는 환자상담 및 교육업무에서만 상관관계가 있는 것으로 나타났으며, 소요시간이 많을수록 중요도가 높게 나타났다. 이상의 연구결과를 통해서 다음과 같이 제언하고자 한다. 본 연구는 일 종합병원에 국한되었으므로 국내의 다른 병원을 대상으로 반복 연구를 실시하여 우리나라 외래간호사의 간호활동의 실태를 전반적으로 파악할 수 있는 연구와 외래간호사가 중요하다고 인식하는 업무를 수행하지 못하는 이유를 분석하는 연구가 시도되어야 한다. 또한 외래간호사의 역할을 소요시간과 함께 기술에 따른 업무분석에 관한 연구도 시도되어야 할 것이다.;The present study purposed to analyze outpatient nurses’ works at a university hospital, measure the important of the works, and to provide basic materials for reestablishing outpatient nurses’ roles. The subjects of this study were all the 53 outpatient nurses of the university hospital, and data were collected for thee days from the 17th to 19th of April, 2007 using a questionnaire on outpatient nursing works developed through literature review and a preliminary survey. Collected data were analyzed using SPSS 12.0, and the analyses were focused on the contents, time, and importance of the subjects’ works using means and percentages. The results of this study are as follows. 1) The average age of the subjects was 32.7, and 64.2% of them were married. Of the subjects, 62.3% were ordinary nurses, and 50.9% were graduates from four-year nursing colleges. The average length of clinical experience was 10 years, and the average length of service as an outpatient nurse was 3 years and 2 months. The subjects’ satisfaction with their job was 56.6%, and their satisfaction with their workplace was 47.1%. 2) Among the subjects’ 10 work areas, routine outpatient works took the longest time (299.94 minutes). Work improvement activities took the shortest time (8.17 minutes) and, next, in order of patient advocacy (15.15), supplies and environment management (17.91). Among detailed nursing duties, reception and processing took the longest time (44.21 minutes) and, next, in order of preliminary examination (33.17), appointment arrangement for revisiting and explanation (30.44), prescription input and confirmation (27.70), chart review (26.46) and health counseling (24.66). Cardiopulmonary resuscitation (0.14), sample reception (0.64) and sample collection (0.83 minutes) were hardly done by the subjects. 3) The work considered most important by the subjects was patient counseling and education (4.42 points) and, next, in order of supplies and environment management (3.64), work improvement activities (3.64) and paper works (3.80). Among detailed nursing duties, education and counseling for medication was considered most important (4.76 points) and, next, in order of explanation about diseases (4.53), explanation about tests, treatments and surgery (4.50), health counseling (4.40), test management (4.29) and preliminary examination (4.08). 4) The length of time that the subjects spent in nursing works was statistically significantly different according to age, position, clinical experience and experience as an outpatient, but the importance of nursing works was statistically significantly different only according to academic qualification. 5) The length of time for nursing works was correlated with the importance of nursing work only in patient counseling and education. That is, the importance of a work was higher when time consumed for the work was longer. Based on the results above, suggestions are made as follows. First, as this study was limited to a general hospital, there should be comprehensive surveys on the general condition of outpatient nurses ’ nursing activities with other hospitals in Korea. Second, research should be made on the reasons that outpatient nurses cannot perform works that they consider important. Third, outpatient nurses’ works should be analyzed not only according to their roles and required time but also according to required skills.
