임상간호사의 전문적 거버넌스와 간호업무성과의 관계 연구

Abstract

병원조직은 구성원들이 의사결정을 할 수 있도록 임파워링하는 방향으로 변화되고 있어 간호사가 의사결정 시 더 큰 자율성과 책임을 가지도록 지원하는 전문적 거버넌스에 대한 관심이 높아지고 있다. 이에 본 연구는 간호사의 전문적 거버넌스와 간호업무성과의 관계를 파악하여 간호현장에서 전문적 거버넌스의 중요성을 확인하고, 간호업무성과를 향상시키기 위한 방안을 마련하는데 필요한 기초자료를 제공하고자 수행되었다. 자료 수집은 서울의 일개 상급종합병원에 근무하는 1 년이상의 병동 및 중환자실 간호사 145 명을 편의표집 하였다. 연구자가 소속된 기관의 연구심의위원회에서 승인을 받은 후 구조화된 설문지를 통해 2021 년 11 월 1 일부터 11 월 21 일까지 자료를 수집하였다. 본 연구는 간호사의 전문적 거버넌스(Index of Professional Nursing Governance, IPNG)와 간호업무성과 도구를 사용하였으며, SPSS/Win 28.0 프로그램을 이용하여 빈도분석, 기술통계, t-test, ANOVA, Scheffe’s test, Pearson’s correlation coefficient 로 분석되었다. 본 연구의 결과를 요약하면 다음과 같다. 1. 대상자의 전문적 거버넌스, 간호업무성과 대상자의 전문적 거버넌스는 평균 168.58±36.85 점으로 ‘전통 거버넌스(86 ~ 172점)’ 수준이었다. 하위영역으로 자원 영향력, 실무통제, 목표갈등해결은 ‘공유 거버넌스’를 인력통제, 정보접근, 위원회 참여는‘전통 거버넌스’를 나타냈다. 대상자 의 간호업무성과는 평균 4.07±.42점(5점 만점)이었으며, 하위 영역으로 간호업무수행능력 4.19점, 간호업무수행태도 4,07점, 간호과정적용 4.06점, 간호업무수준향상 3.78점 순이었다. 2. 일반적 특성에 따른 전문적 거버넌스, 간호업무성과의 차이 대상자의 일반적 특성에 따른 전문적 거버넌스는 임상 경력(F=4.36, p=.006)에서 5~10년 미만의 대상자가 3~5년 미만보다 전문적 거버넌스가 유의하게 더 높게 나타났다. 간호업무성과는 연령(F=4.09, p =.019). 최종학력 (F=-2.41, p=.017), 결혼상태(F=-2.27, p=.025), 임상경력(F=4.72, p=.004), 부서이동횟수 (F=5.80, p=.001)에 따라 유의한 차이를 보였다. 즉 40세 이상이 29세 이하보다, 석사 학위대상자가 학사 학위 대상자보다, 10년 이상의 대상자가 1~3년 미만보다, 3회 이상의 부서이동 경력이 있는 대상자가 부서 이동이 없는 대상자보다 간호업무성과가 유의하게 더 높은 것으로 나타났다. 3. 전문적 거버넌스와 간호업무성과의 관계 본 연구에서 전문적 거버넌스와 간호업무성과 간에 유의한 상관관계가 없는 것으로 나타났다(r=.094, p=.263). 본 연구의 결과 간호사의 전문적 거버넌스는 간호관리자에게 의사결정권이 있는 전통 거버넌스 수준으로, 임상경력에 따른 차이를 나타내 조직의 의사결정에 참여하는 공유 거버넌스가 이루어지도록 경력간호사의 보유를 높이려는 조직적 전략이 필요함을 시사한다.;The purpose of this study was to identify whether or not there is a relation between the professional governance of clinical nurses and the outcome of nursing performance. Using a convenience sampling method, 145 registered nurses who had more than one year of experience in nursing and who worked for tertiary care hospitals located in Seoul were recruited as participants in the quantitative phase. Data were collected from November 1, 2021, through November 21, 2021, using a questionnaire. Quantitative data were measured using the through the SPSS program, the collected data was analyzed in terms of percentage, average, t-test, ANOVA, Scheffe test and Pearson correlation coefficient. The key results of this study are as follows: 1. The mean score of professional governance was 168.58±36.85, indicative of traditional governance. Among the subscales of professional governance, “influence over resources” , “control over professional practice” and “goal setting and resolving conflicts” reflected shared governance, while “control over personnel,” “access to information” and “participation in committee structure” reflected traditional governance. The mean score of nursing performance was 4.07±.42. 2. Significant differences in professional governance were seen based on clinical experience (F=4.36, p=.006), and less than 5 to 10 years of clinical experience was higher than 3 to 5 years. 3. Significant differences in nursing performance were seen based on age (F=4.09, p=.019), final education (F==-2.41, p=.017), marital status (F=-2.27, p=.025) clinical experience (F=4.72, p=.004), the number of department movements (F=5.80, p=.001). 4. There was no significant correlation between professional governance and nursing work performance (r=.094, p=.263). This study is meaningful in analyzing the factors affecting the professional governance of clinical nurses and preparing basic data for improving nursing work performance. According to the study, the higher the clinical experience, the higher professional governance tends to be, so institutional and structural changes are needed to change the working environment and expand decision-making.
