우리나라 만성질환자에서 우울증 동반 여부에 따른 의료비용과 삶의 질의 차이

Abstract

Depression is most common type of mental illness and significant health issue both for world population and Korean. Furthermore, due to its negative impact on health behaviors and socioeconomic status, chronic diseases are one of the main risk factors of depression. However, only few studies focused on this relationship in terms of quality of life (QoL) limited to several chronic disease which is hypertension, diabetes, and arthropathy. This study analyzed difference of health related QoL and medical expenditure of Korean chronic diseases patients aged above 40 years according to depression performing propensity score matching. Using Korea Health Panel Survey 2016~2018 provided by the Korea Institute for Health and Social Affairs and National Health Insurance Service, 5,019, 4,930, 5,068 were respectively selected as study subject before propensity score matching. After the matching, study subject was 1,044, 1,010, 1,100 respectively. QoL was assessed by EQ-5D-3L and then converted into score between 0 and 1 using values suggested by Lee et al. (2009). As a result, the study showed that depressed chronic diseases patients had lower QoL compared with non-depressed chronic patients. Those with both problems had lower QoL according to age, marry status, education, economic status, income level, exercise, and number of chronic diseases and the difference was statistically significant. (α<0.05) Additionally, age, marry status, exercise, and depression were regarded as factors that influence QoL of chronic diseases patients. On the other hand, medical expenses of depressed chronic patients were higher than non-depressed in terms of outpatient and outpatient medical expenses. However only the result of 2018’s outpatient medical expenses was statistically significant. Taking into consideration of the findings, chronic patients with depression are more vulnerable in qol. Therefore, they need different treating guideline other than those with single problem and integrated program should be designed considering both problems. This will further help develop new programs and policies for managing these individuals.;정신질환과 만성질환은 전 세계와 우리나라 모두에서 건강을 넘어 사회, 경제적인 부분에도 상당한 영향을 미치고 있는 질병이다. 정신질환자와 만성질환자의 수는 지속적으로 증가하고 있으며 두 질환을 동시에 보유한 환자의 수도 증가하고 있다. 하지만 우울증과 만성질환을 동시 보유한 환자를 대상으로 건강관련 삶의 질과 비용을 살펴본 연구는 거의 없었다. 이에 본 연구는 우울증 여부에 따른 만성질환자의 건강관련 삶의 질 및 의료비용 차이를 분석하고 건강관련 삶의 질 영향요인을 파악하는 것을 목적으로 한다. 이때 만성질환자는 한국의료패널조사 자료 중 상위 3위 질환에 해당하며 고령화로 인하여 그 중요성이 대두되고 있는 고혈압, 당뇨병, 관절병증 환자를 만성질환자로 정의하여 살펴보았다. 2016년, 2017년, 2018년 한국의료패널 자료를 이용하여 만 40세 이상 만성질환자를 대상으로 우울증 유무에 따른 건강관련 삶의 질과 의료비용을 확인하였다. 교란 요인을 통제하여 우울 유무에 따른 건강관련 삶의 질과 의료비용 차이를 확인하기 위해 성별, 연령, 배우자 유무, 교육수준, 의료보장유형, 민간보험 가입여부, 소득수준, 경제활동 여부, 만성질환 보유개수를 교란 요인으로 선정하여 성향 점수 매칭을 실시하였다. 그 후 SAS 9.4와 STATA 17.0을 이용하여 카이제곱 검정, 대응표본 t-검정, 일원분산분석, 다중회귀분석을 실시하였다. 그 결과 건강관련 삶의 질의 경우 우울증이 있는 만성질환자는 우울증이 없는 만성질환자보다 모든 요인에서 낮은 것이 확인되었다. 또한, 우울증이 있는 경우 연도별로 공통적으로 연령이 증가할수록, 배우자가 없는 경우, 교육수준이 낮을수록, 경제활동을 하지 않는 경우, 소득수준이 낮을수록, 신체활동을 하지 않을수록, 만성질환 보유개수가 많을수록 낮은 것이 확인되었다. 또한, 다중회귀분석 결과 만성질환자의 건강관련 삶의 질에 영향을 미치는 요인은 연도별로 조금씩 차이가 있었으나 공통적으로 연령, 혼인상태, 경제활동 상태, 신체활동, 우울증으로 확인되었으며 통계적으로 유의한 영향을 미쳤다. 의료비용의 경우 3개년 모두 외래와 외래처방 약제비에서 우울증을 가진 만성질환자가 우울증이 없는 만성질환자보다 높은 것으로 나타났으나 2018년에서만 통계적으로 유의한 차이를 보였다. 만성질환자는 우울증에 이환 될 확률이 높으며 만성질환과 우울증을 동시 보유하는 경우 건강관련 삶의 질이 감소한다. 최근 만성질환과 우울증을 보유하는 환자의 수가 점점 증가하고 있지만 아직 우리나라에서는 만성질환과 우울증을 동시 보유한 환자에 대한 통합 관리사업이 미비한 실정이다. 이에 따라 만성질환자에 있어 우울증 이환 전 이를 예방할 수 있는 교육이 필요하며 두 질환을 동시에 보유하는 환자를 대상으로 한 관리사업 및 제도 마련이 필요하다.
