신경 손상 성인의 손 기능 향상을 위한 우쿨렐레 활용 TIMP 중재 사례

Abstract

The purpose of this study is to investigate how TIMP intervention using ukulele affects hand function in adults with nerve damage. The subjects of this study were 3 adults with neurological damage diagnosis and hand dysfunction at S Hospital in Seoul, and a total of 12 sessions were conducted 2-3 times a week for 30 minutes for about 7 weeks. The change in hand function was investigated using the pre-post-Post Box and Block Test, 9-Hole Peg Test, and Jebsen & Taylor Hand Function Test, and post-interview interviews were conducted individually to find out the participation experience through ukulele playing. As a result of the study, it was confirmed that all three subjects showed improved results in BBT and 9-HPT, indicating a positive effect on coordination and agility. In particular, in the JTHFT, all three subjects showed improvement in the 'writing' and 'card flipping' domains, confirming that it was effective for fingertip coordination and fingertip pinching. As a result of the post-interview, ukulele performance gives the subjects an opportunity to try something new, higher motivation for treatment, and more frequent hand use in daily life, so positive feedback can be confirmed. means that Therefore, it suggests that in the future, it is necessary to develop and expand an intervention method so that the intervention using the ukulele can handle more diverse functions.;본 연구의 목적은 우쿨렐레를 활용한 TIMP 중재가 신경 손상 성인의 손 기능에 어떠한 변화를 나타내는지 알아보는 데에 있다. 연구 대상자는 서울특별시 소재 S병원에서 신경학적 손상 진단 및 손 기능 장애가 있는 성인 3인을 대상으로 약 7주간 주 2~3회 30분씩 총 12회기를 진행하였다. 본 중재 활동은 대상자의 기능과 증상에 맞춰 적합한 난이도의 활동으로 제공되었다. 사전 사후 Box and Block Test, 9-Hole Peg Test, Jebsen & Taylor Hand Function Test를 사용하여 손 기능의 변화를 알아보고자 하였고, 우쿨렐레 연주를 통한 참여 경험을 함께 알아보기 위해 사후 인터뷰를 개별적으로 진행하였다. 연구 결과는 3인의 대상자 모두에게 BBT와 9-HPT에서 개선된 결과가 나타나 협응력과 기민성에 긍정적 효과를 나타냄이 확인되었다. 특히, JTHFT에서는 3인의 대상자 모두‘글씨쓰기’와‘카드 뒤집기’영역에서 개선이 나타나 손끝의 협응력과 손끝 집기 동작에 효과적임을 확인하였다. 사후 인터뷰 결과로 우쿨렐레 연주는 대상자들에게 새로움을 시도할 수 있는 기회와 높아진 치료 동기, 일상에서의 손 사용 빈도가 잦아짐을 나타내어 긍정적 피드백을 확인할 수 우쿨렐레가 치료적으로 추후 다양하게 활용될 수 있는 악기임을 의미한다. 따라서, 향후에는 우쿨렐레를 활용한 중재가 더 다양한 기능을 다룰 수 있도록 중재 방안의 개발 및 확장된 연구가 필요하리라 시사하는 바이다.
