국내·외 상지재활을 위한 음악중재연구 내 악기연주 유형 분석

Abstract

본 연구는 국내·외 상지재활을 위한 음악중재연구의 전반적인 연구 특징과 악기연주 유형 및 악기의 활용을 분석함으로서 향후 임상적 적용을 확대하기 위한 자료를 제공하는 것을 목적으로 한다. 이를 위해 국내 및 국외의 전문 학술지에 게재된 연구를 대상으로 2006년 1월부터 2021년 9월까지 발표된 연구 중 포함기준에 따라 총 21편의 연구를 선정하여 분석하였으며 분석 결과는 다음과 같다. 먼저, 선정된 연구들을 수행한 연구자의 학문적 배경과 중재에 활용된 악기연주 활동의 기법 기술에는 차이가 나타났다. 반면, 상지기능 개선을 위한 악기연주 활동의 내용과 사용된 악기의 종류에는 공통된 특성이 나타났다. 국내에서 진행된 악기를 활용한 음악중재 연구는 주로 음악치료 분야였으며, 국외에서 진행된 연구는 주로 의학 분야에서 시행되었다. 의료 환경에서 음악활동이 보조수단으로 사용되었던 것에 비해 상지기능 개선을 위해 직접적인 재활도구로 악기가 사용된 것에 의의가 있다고 할 수 있으며, 재활훈련은 주로 여러분야가 협력하여 집중적인 치료를 실시하기에 타 분야와의 교류가 지속적으로 시행되어야 할 것으로 사료된다. 분석된 연구의 연구대상자로 뇌졸중 환자가 대부분이었기에 실제 임상현장에서 적용될 수 있는 다양한 신경학적 질환을 대상으로 한 임상연구가 확대되어야 함을 시사한다. 마지막으로 대근육 움직임 또는 소근육 움직임의 기능별 목표에 따라 진행된 연주내용에 차이가 나타났다. 대근육 움직임을 목표로 한 연구는 소근육 움직임을 목표로 한 연구에 비해 비교적 적은 수의 연구가 진행 되었으며 목표기능에 따른 악기연주의 활용에 대한 설명이 더욱 요구됨으로 악기연주 및 악기활용에 범위를 넓히기 위한 다양한 기능을 목표로 한 연구의 확대가 필요함을 시사한다. 본 연구의 결과는 상지 기능 재활을 위해 악기연주 활동을 사용하고자 하는 치료사들에게 실제 음악중재에서 사용되는 악기연주 활동에 대한 정보와 임상현장에서 적용 할 수 있는 관련 정보를 제공한다는 점에서 의의가 있다.;The purpose of this study is to extend the clinical applications of the use of musical instruments in music intervention studies for upper extremity rehabilitation. The overall research characteristics of music arbitration research, the types of instruments played, and the utilization of instruments was analyzed. A total of 21 studies published from January 2006 to September 2021 were selected and analyzed according to the inclusion criteria, targeting studies published in domestic and foreign specialized journals. The analysis results are as follows. There was a difference between the academic background of the researchers who conducted the selected article and the technique of playing the musical instrument used for music intervention. On the other hand, general characteristics appeared in the content of musical instrument playing activities for improving upper extremity function and the type of musical instrument used. Music intervention studies using musical instruments in Korea were conducted mainly in the field of music therapy, and music intervention studies in other countries were conducted mainly in the medical field. It represents rehabilitation training should be carried out continuously with exchanges with other fields, mainly in order for you to cooperate and provide intensive treatment. Most of the participants were stroke patients, suggesting that clinical studies targeting various neurological disorders that can be applied in the actual clinical setting should expanded. There was a difference in the performance progressed by the functional goals of the movements of gross motor or the movements of fine motor. It is necessary to expand research aimed at various functions in order to expand the scope of instrument performance and the use of musical instrument in music intervention studies for upper extremity rehabilitation.
