Abstract
Alzheimer’s disease (AD) is a neurodegenerative disease neuropathologically characterized by extracellular deposit of amyloid protein and intracellular formation of neurofibrillary tangles in the brain. The accumulation of cytotoxic Aß is known to be the main cause of AD (called a ß-amyloid hypothesis). However, several lines of evidence suggest that other mechanisms such as free-radical related changes, inflammatory responses, mitochondrial dysfunction, impaired synaptic plasticity, reduced hippocampal neurogenesis, and altered epigenetic mechanisms, also play a role in concert with Aß, or independently, in the pathogenesis of AD. Numerous studies from academic and pharmaceutical fields have attempted developing therapeutic strategies for treating AD by targeting the ß-amyloid hypothesis in the past decades. Unfortunately, however, none of the methods and trials attempted, including the phase 3 clinical trial of solanezumab, a humanized monoclonal IgG1 antibody directed against Aß peptide and the phase 1 trial of Verubecestat, a BACE1 inhibitor, were successful. The lesson from these failures raises the question that AD is a neurodegenerative disease involving the complex changes more than the ß-amyloid hypothesis. Considering the complex neuropathology of AD and an increasingly urgent demand for therapeutic means, alternative untried approaches to test for therapeutic potentials for AD are highly encouraged. In the present study, therapeutic potentials and underlying mechanisms of hyperbaric oxygen (HBO) treatment were investigated using an animal model of AD. Tg-APPswe/PS1dE9 mice were treated with HBO by applying the pure oxygen at 2 ATA for 1 h daily for 14-28 days from the 7 months old of age when the plaque deposition and the cognitive dysfunction commenced in this AD model. Tg-APP/PS1 mice subjected to HBO treatment exhibited increased cognitive performance comparable to that of wild-type control animals in the spatial learning and memory test, as well as independent contextual recognition memory tests. Systematic molecular, biochemical and histological analyses indicated that HBO treatment in Tg-APP/PS1 mice suppressed amyloid ß plaques formation, brain inflammatory activation, radical oxygen species (ROS) levels, while increased hippocampal neurogenesis and expression of synaptic plasticity markers in the prefrontal cortex and hippocampus. Particularly, the hippocampal neurogenesis was markedly increased after HBO treatment along with increased expression of neurotrophins, including BDNF, NT3, and NT4/5. To gain insights into the mechanisms that produced such drastic and diverse effects of HBO, the expression and activity profiles of basic epigenetic factors in the hippocampus and prefrontal cortex were explored with focusing on the expression of neurotrophic factors. These analyses indicated that the HBO-dependent expression of neurotrophic factors proceeded with the increased activity of epigenetic factors. Overall, these results of this study suggest that HBO treatment has a great potential to counteract AD pathology and behavioral deficits of Tg-APP/PS1 mice and the HBO-induced therapeutic effects proceed with epigenetic-related genomic responses.;알츠하이머 병 (Alzheimer’s disease, AD)은 신경 세포 밖에 아밀로이드 단백질의 침착 및 세포 내 신경섬유 다발이 과다 축적되어 신경세포의 기능 장애 및 인지행동장애를 보이는 대표적인 신경 퇴행성 질환이다. 현재까지 알츠하이머 병의 주된 원인으로 신경세포의 아밀로이드 베타 (amyloid beta, Aß) 생성 증가와 그로 인한 신경병리 현상으로 설명되는 ‘베타 아밀로이드 가설’ 이 중심이 되어왔다. 수십년 동안 많은 연구자들과 제약회사들이 이 가설을 기반으로 AD 치료 약제들을 개발하였으나, 그 동안의 모든 시도들은 최종 임상연구에 이르는 동안 실패하였다. 여러 증거들에 의하면 아밀로이드 베타가 알츠하이머 병 유발에 상당한 원인 물질이지만, 이와 함께 수반되어 발생되거나 독립적으로 발생하는 신경 병리학적 변화들, 예를 들어 염증 반응 증가, 미토콘드리아의 기능 장애, 신경 조직 발생의 저하, 활성산소 증가 등이 병의 진행에 부수적이지 않고 매우 중요한 역할을 한다는 가능성이 제시되고 있다. 알츠하이머 병은 단순히 베타 아밀로이드 가설로 설명될 수 없는 복잡한 질환임을 의미하고, 따라서 앞으로 알츠하이머 병의 치료제 개발은 기존의 관점과 달리 전체적인 병변들을 억제하는 방향이 되어야 한다는 것을 시사한다. 더욱이 현재와 같은 노인 인구 및 알츠하이머 병 환자의 지속적인 증가로 볼 때 새로운 치료방법의 개발 연구가 불가피하다. 본 연구는 알츠하이머 병와 같은 복잡한 기전의 병변을 한 번의 시도로 다양한 기전에 영향을 줄 수 있는 치료 기술로서 고압산소 (hyperbaric oxygen, HBO)의 처치효과를 규명하고 그 작용 메커니즘을 조사했다. 고압 산소는 마우스가 자유롭게 움직일 수 있도록 전용 케이지에 담아 고압 산소 탱크에서 하루에 1 시간 동안 100% 산소를 2 기압 상태에서 2-3 주간 노출하는 방법으로 처치했다. 알츠하이머 병의 동물 모델인 Tg-APP/PS1 생후 7개월부터 뇌에 플라크 침착이 일어나고 7.5개월에 이르면 뚜렷한 인지 행동 저하를 보인다. 이에 생후 7개월부터 2-3 주간 고압산소 처치를 받은 Tg-APP/PS1 마우스는 공간 지각 및 기억 능력과 공포 기억 능력 검사에서 대조군 수준의 인지행동을 보였다. 고압 산소 요법에 의한 인지 행동 변화의 신경학적 메커니즘을 규명하고자 생화학 및 조직학적으로 체계적인 분석을 수행했다. 조사 결과 고압 산소가 처치된 동물에서 베타 아밀로이드 축적으로 생성된 플라크, 활성화된 뇌 염증 반응, 그리고 활성 산소의 증가를 억제하는 양상을 관찰했다. 고압 산소 처치는 또한 Tg-APP/PS1 동물에서 나타나는 미토콘드리아의 기능 저하를 회복하였다. 고압 산소 처치를 통해 전두엽 피질과 해마에서 신경 발생 및 시냅스 가소성 관련 유전자들의 발현이 증가함을 확인하고, 이는 BDNF, NT3 및 NT4/5와 같은 신경성장인자 (neurotrophin)의 발현 증가를 통해 일어날 가능성을 시사했다. 고압 산소 처치에 따른 신경성장인자들의 작용과 연관된 후생유전인자들의 발현과 조절 정도를 조사했다. 그 결과, 고압 산소 처치로 인해 CBP, P300, HDAC2 (histone deacetylase 2), REST (RE1-silencing transcription factor), CREB (cAMP response element binding protein), MeCP2 (methyl CpG binding protein 2) 등의 여러 후생유전인자들의 발현이 증가하거나 감소했다. 이러한 알츠하이머 병 모델 동물을 이용한 연구의 결과에 근거할 때, 고압 산소 처치는 뇌 가소성을 조절하는 다양한 유전자들의 발현 조절을 통해 알츠하이머 병의 병리학적 및 행동학적 결함을 회복시키는 것으로 이해된다. 이와 같은 연구 결과는 고압 산소요법이 알츠하이머 병의 새로운 치료법으로 큰 잠재력이 있다는 것을 강하게 시사한다.
